Intravenous administration of umbilical cord lining stem cells

PDF · 12 Feb 2025

The article reports the first-in‑human, Phase Ia, open‑label, 3 + 3 dose‑escalation pilot trial (uSTOP LVAD BLEED) evaluating intravenous umbilical cord lining stem cells (ULSCs) in patients supported by HeartMate 3 left ventricular assist devices (LVADs). Nine outpatients received escalating single doses of 50 × 10⁶, 100 × 10⁶, or 200 × 10⁶ ULSCs administered over 45 minutes and were monitored for 30 days (with a 90‑day follow‑up for immunogenicity). The primary endpoint—safety and tolerability—was met: no infusion‑related adverse events, no thromboembolic events, no change in lactate dehydrogenase, no blood transfusions, and no development of panel‑reactive antibodies. Exploratory secondary outcomes showed that in four patients angiopoietin‑1 (Ang‑1) increased and angiopoietin‑2 (Ang‑2) decreased from baseline to day 30, indicating partial angiogenic stabilization. Quantitative fecal immunochemical testing (qFIT) demonstrated a reduction in mean and median blood content of stool from baseline to day 30, persisting to day 90. The study concludes that IV ULSCs are safe and tolerable in LVAD patients and provide an early efficacy signal that warrants larger, powered trials. Limitations include the small sample size, lack of standardized concomitant medication dosing, and short‑term follow‑up. The authors disclose financial ties to RESTEM LLC, the cell supplier, and funding from the Gatorade Foundation.

Topics

LVAD‑Associated Angiodysplastic Bleeding: Pathophysiology and Clinical Burden

Overview of how continuous‑flow LVADs predispose patients to gastrointestinal bleeding via platelet dysfunction, acquired von Willebrand syndrome, and dysregulated angiogenic factors (Ang‑1/Ang‑2 imbalance).

Umbilical Cord Lining Stem Cells (ULSCs) as a Vascular‑Stabilizing Therapeutic

Rationale for using off‑the‑shelf ULSCs, including pre‑clinical evidence that they raise Ang‑1, lower Ang‑2, and improve endothelial barrier function more effectively than bone‑marrow‑derived MSCs.

uSTOP LVAD BLEED Pilot Study Design and Patient Cohort

Details of the Phase Ia, single‑ascending‑dose 3 + 3 design, inclusion/exclusion criteria, dosing schedule (50, 100, 200 × 10⁶ cells), infusion protocol, and baseline demographics of the nine participants.

Primary Safety and Tolerability Outcomes

Presentation of safety data: absence of infusion reactions, thromboembolic events, LDH changes, transfusion needs, and lack of immune sensitization measured by panel‑reactive antibody testing.

Exploratory Efficacy Signals: Angiopoietin Modulation and Stool Blood Reduction

Findings that four patients showed increased Ang‑1 and decreased Ang‑2 at 30 days, and qFIT testing indicated a decrease in stool blood content persisting to 90 days, suggesting early vascular stabilization.

Discussion of Clinical Implications, Limitations, and Future Directions

Interpretation of results in context of prior stem‑cell LVAD studies, acknowledgment of small sample size, medication confounders, and proposal for larger Phase II trials with pre‑implantation dosing and broader biomarker panels.
