YouTube video (Jenn Simmons, MD) arguing against underdosing bioidentical hormone replacement therapy (HRT). The speaker states that titrating hormones to symptoms alone results in correct dosing only ~30% of the time, leading to overdosing or underdosing, and emphasizes the need for testing and hormone-literate providers for peri- and postmenopausal women.

When you give bioidentical hormone replacement and all you do is titrate to symptoms, you're only going to be right about 30% of the time. And 30% of the time you're going to be overdosing people. And 30% of the time you're going to be underdosing people. Right. And this is a problem. And this is why it's so important. I don't believe in just giving people a little bit. Like that whole theory of give the least amount for the shortest amount of time. That is absolutely wrong. That should be malpractice. They think that they're doing something safe by just giving a little bit. Right. And it's not helping women. It's not helping them at all. And under dosing does not serve them. But the only way that you know is to test. And most women who are being prescribed hormones are not being tested. That's why it's so important. If you're going to go on hormone replacement, and I believe most women who are peri and postmenopausal should go on hormone replacement. But if you are going to go on hormone replacement, it should be with a hormone literate provider.
