Dysport & Restylane Lyft Injection by Dr. Naderi to Forehead Wrinkles & Liquid Cheek Lift

Dr. Shervin Naderi demonstrates Dysport injections for forehead wrinkles and Restylane Lyft filler for a liquid cheek lift. He explains where to inject (procerus, corrugators, frontalis) while preserving lateral lift, and how filler at the cheek-trough junction smooths under-eye bags and fixes asymmetry.

Hi, good morning. We're doing another video in our injection series today. I'm going to show you some Galderma products. We're going to use Resto Lift as well as Dysport. Dysport, many of you have heard of as the other Botox. It came out a few years ago. It was in Europe under the name Rawoxin. A different name. It's been around for almost as long as Botox. Botox obviously hit the market first, but Dysport is the next most famous botulinum toxin. One is Egobotulinum, one is Ono Botulinum. They're very similar. They're like Pepsi and Coke. They're more similar than they are different. But there's some clear advantages to Dysport. Dysport works faster. It always works faster. So if somebody comes in on a Wednesday and says, I want to go to a party on Saturday and I want to get some Botox, I would usually talk to them getting Dysport so they can see that effect sooner, because Botox typically takes a little bit longer to kick in. But the other advantage of Dysport is that it actually spreads a little bit more. So from each injection site, it actually spreads a little bit more. Gives you a softer, more natural look. I would say about 80 to 90% of our patients who switch to Dysport actually like Dysport. The cost is the same, the injection feels about the same. Most can't tell a difference, but the ones that do tell a difference prefer Dysport. Occasionally people go back to Botox, but most of them prefer Dysport. The other product we're going to do today is a filler called Restland Lift. It's similar to Juvederm Voluma. It's for mid face augmentation. And I'm going to demonstrate on this beautiful patient. So come on up, we're going to demonstrate this. So the most common place for Botox or Dysport on women and men is the Glabella, or the frown lines. The frown lines are created by movement of two muscles. The corrugators here goes up in a V fashion. The procerus is here, which creates horizontal lines. And there's also depressor muscles that pull the eyebrow down. Then there's the frontalis, which goes in this direction and pulls the eyebrow up and creates lines here. You want to be very careful with Frontalis injection. You don't want to over inject the frontalis because you'll paralyze the muscle that lifts the eyebrow and the whole eyebrow Goes down. A lot of people come in and say they've had bad Botox elsewhere where they got the Botox, and all of a sudden the eyebrow just went like this. That's not necessarily bad Botox. It's just bad judgment in how much and where it needs to be injected. When you inject Botox, you can do it in a way or dysport in a way to elevate the eyebrows or keep it more flat. Most women, younger women, prefer elevation. It gives it a nicer look as we get older. The elevation helps the eyes stay more open. But when you elevate the eyebrows, you will see a few lines above it. Because what's happening is that the frontalis muscle is the one that's working, elevating, seeing a few lines. If you want to have a totally smooth forehead, you can't get an elevation. You have to inject the lateral brow as well to make sure it doesn't elevate as much. Now when you go to the filler, there's different places for fillers. You can see that she's starting to form some glabellar furrows. Even with Dysport or Botox injection, what you're going to see is that these lines will not completely go away. So to make them better, I'm going to inject a little bit of filler here as well. You have to be extremely careful in this area. Know the anatomy. When we get to the mid face, there is a couple of little areas of depression right in this area that elongates the lower eyelids. It's the trough cheek junction and it makes people look more tired and older. By injecting it, it lifts it up, plumps it up and does a really nice job. So I'm going to demonstrate that right now. So we're going to do the Dysport injection. First I'm going to show you my pattern. I can do Dysport or Botox in several different ways. We've already been prepped, going to go and demonstrate this technique. My preference is to usually inject the patient from the right side. Sometimes I go behind, sometimes I'm on the left side. But for this demonstration, I'm going to be on this right side. So first is my procerus injection. It's right there. Getting the procerus, getting the depressor, getting the depressor on this side, getting the medial corrugator, getting the lateral corrugator. Coming back again, medial corrugator, sometimes with little tough patches of skin and getting the lateral Corrugator. That's it. That opens up the eyebrows, gives it an arch. Now we're going to go smooth out some of the frontalis lines. There are blood vessels here. They're hard to see, but you can go superficially. Now I'm going to get the middle part of the forehead. So the middle part of the forehead is going to get smoothed out. I'm leaving the lateral frontalis alone so it can elevate so she can have a lift. If she wanted even a further elevation, I would place the drop underneath the brow. I'm not going to do that because we don't want to go too high. But this didn't cause any bruising. This bleeding point will stop. And my last drop right there in the central compartment. Just about a few seconds of pressure, that stops, but no bruising. Now, bruising can happen anytime. You put a needle in the skin, you can have bruising, but bruising from Botox or Dysport needles are small. Their TB needles are extremely small. You can barely see it. And so if any bruising occurs, usually it's a small bruise. So that's as far as that's. Now we're going to go to the filler. The filler we're using is Restylane lift. Again, Restylane lift is FDA approved for the mid face. We're going to open it up. It comes with a special needle of its own. I'm going to go back and start with this side. Okay. So you can see even with her eyes closed, there's a little depression right there. She has nice cheekbones here, fullness here. There's a depression right there. So keeping our fingers crossed, there's no blood vessels under there. Ready? A little injection. And I'm injecting to the point of correction, I don't want to jab too many times. I also don't want to have to massage it too much. I want to go right where I need to go. Inject as few times as possible not to create too much bleeding, bruising and swelling. Injections are deep. And you can see immediately. Look, the fat pad, the lower, lower lid fat pad that she had. The eye bag that she had is gone. It just blends better. I'm going to touch it up a little bit below. It did. Okay? Yes, good. You could say if it's horrifically painful. Okay, good. No crying, no screaming. She's doing well. Look, you can see immediately there's no bruising. It may get a little yellowish greenish in a few days, but just that beautiful, nice round Mound of the mid face was created and I used just barely a few drops, but each drop made a difference and lifted this area up nicely. So that's essentially the injection of rest and lift. I'm going to go to the other side. Now. The two sides, the two cheeks are almost always different on everybody. The two sides of the face are different. Each person has one eye, one cheekbone that's higher, One corner of the mouth is higher. One side is usually narrower, one side is wider. The two sides of the face don't match. If you look at her face, you can see her left side is higher, left eye is higher, left cheek is higher. If you place your fingers on the cheekbones, this side is higher. So I want to put less on this side because I want to create more symmetry and lift this up. I've seen some patients that go home and after getting a procedure, they stare and stare and stare and stare in the mirror until they discover magically all the asymmetries that they always had but they never noticed before. Then they think that the filler or the Botox or the Dysport or the facial or whatever else was done, created that. That's just not true. We all have these asymmetries. No bleeding at all from this side. I'm going to go back to the other side. She's doing really well. Now to create more symmetry, what I'm going to do is I'm going to lift this cheap one up a little bit. I can't lift the eye. I can lift the cheekbone. Keep our fingers crossed to see. Make sure there's no blood vessels under here. If there are, you just hold a little pressure. I'm injecting as I go in. And what it does is it pushes blood vessels out of the way. Look, when you look at the face now, look at the cheekbones now they're getting more symmetric. The mid face is fuller. She just looks more rested. A little bit more here. Now I'm going to do something to show you if I want to improve these lines that have already formed here. You can see the lines, but just doing this, as you press the two sides together, you'll see where it wants to kind of fold it. So what I do, very superficially, these injections are not deep. I'm injecting the glabella to improve these lines, because what happens, and you can tell I'm going to move my finger so you can see, see it superficial. The problem is, even with Dysport or Botox, if these lines have already etched in, they're not going to go away with just neurotoxin. So you need the addition of filler to kind of make it stiffer. So as I press it together now, it's less likely to want to fold in. And women, you want to be careful. You don't want to make them too masculine, give them a really strong masculine brow ridge. So you want to be more cautious. But this is injection. People are afraid of risk of blindness in this area. With injections, it's extremely small. The risk of blindness is less than getting hit by lightning. But if you know the anatomy, it's almost zero. You just can't be occluding blood vessels in that area. That's essentially our injection. So what we did was we did this port, which used to be a Medicis product, then became a Valiant product and now it's Galderma product. And we did Restylane Lift. In the old days, it was labeled as Purlane. It lasted two years in the face. It's now been relabeled as Restylane Lift. It competes with Juvederm Voluma. I like both Voluma and Restylane Lift. The preference for me on wrestling lift is it's a smaller needle, it's a little bit stiffer. So for contouring and creating cheekbones, I think it works better than Voluma. Voluma is still a fantastic substance and I think it does a great job for adding volume. As far as Dysport and Botox, again, we use both substances. I train for both companies, I teach for both companies. I don't really have any particular interest in one or the other. But Dysport does work a little bit nicer in our patients. Works faster, spread treads more, has a more natural look. For more information, you can visit our website and learn more. Thank you.
