There's a phenomenon that I call the revolving door of revision rhinoplasty patients. These are some patients that have had four, five, six, seven or even up to eight previous rhinoplasties. They continue to be unhappy with one thing or another part of the nose. At no point has a surgeon really sat down, taken the time to discuss what's realistically possible or not, and they just basically sign him up for another surgery. It ends up being expensive, emotionally tasking, and it's just not healthy. Overall, I spend quite a bit of time talking 20 to 25% of patients out of rhinoplasty, out of revision rhinoplasty. If they come in when I see that, listen, at the end of the day, what you're going to gain is maybe a 5% improvement and it's not worth the risks of it. This is really important, however, you have to distinguish this from patients who've actually had poor results. I've also had patients who've had four to six pre previous really bad rhinoplasties who require the final definitive rhinoplasty to basically get them on with their lives. You just have to basically separate these two from each other. One needs further surgery, the other one does not need further surgery.
