Non Surgical Rhinoplasty Washington DC, Chevy Chase Maryland, Virginia - Dr. Naderi

Video transcript of Dr. Shervin Naderi demonstrating and explaining non-surgical rhinoplasty using Restylane. The video covers candidate selection, technique (micro-droplet injections, molding), product choice rationale (preference for Restylane over Juvederm, Radiesse, Artifill, silicone), aesthetic goals (bridge elevation to narrow appearance), and brief aftercare advice; includes a live demonstration on a female patient.

Hi. This morning we're going to talk about non surgical rhinoplasty with a beautiful young female who all she wants is just a little bit more height to her bridge. Now, non surgical rhinoplasty is a term that came into the medical literature a few years ago. Some surgeons and some dermatologists overdo this. They fill the nose to a point where it's not aesthetically pleasing. But not everybody's a candidate for a non surgical rhinoplasty. The number of patients that are actually a candidate for this procedure are very small. So essentially in her, what she wants is from the front to be able to have a little bit more lift and a little higher bridge. And as the bridge elevates a little bit, it makes it look narrower. So lifting the bridge creates a slimmer, narrower look from the front and from the side, it creates a little bit more of a natural looking appearance. And this is very common in Asian patients, African American patients, Latino patients, but.
It'S even common in patients who want.
Revision, non surgical rhinoplasty, people who may have had 1, 2, 3 previous surgeries. And there are a few little areas that could benefit from a little bit of volume. Classically, we're doing this with cartilage and a surgical rhinoplasty. I'll be placing either rib cartilage or septal cartilage to lift that area, but in a non surgical fashion. The same thing can be done and can be repeated once or twice a year. So we're going to do that with Restylane, which is a very safe compound compared to some of the other stuff that's out there.
So I start by sterilizing the area.
With a little bit of alcohol, just.
To kind of prep the skin.
So what I'm going to start doing is using Restylane. Restylane is a clear gel. It's a hyaluronic acid. Compared to some of the other fillers, it's one of the safest fillers. I like Restylane because it's a hyaluronic acid, but it has a little higher G pressure. G prime allows a lift and it's structurally more sound than Juvederm. Juvederm creates more fullness, but it doesn't give structure the way Restylane does. I'm not a fan of Radiesse in the nose. I'm not a fan of Artifil or silicone in the nose.
That's not safe.
They can cause lots of scarring. I've had a lot of rhinoplasty patients who've had injections with those in the past. As you go to surgery and I have to remove all that.
It's just a mess.
But Restylane works nicely. I'm going to show how that's done. So when I start, I'm basically lifting.
The area between my fingers and I'm having my bevel down. I just enter and again, it's a lot of micro droplets. I'm placing a little bit right at the bridge. I'm walking it down. As you walk it down, this does have lidocaine. It'll start numbing up and you can.
See very little bleeding so far.
You can have some bleeding. If you do, you just stop, hold some pressure. We don't want to get a lot of bruising. You can see it lifts the area up. Just slowly. I'm walking it down. I'm lifting my finger away so you can see how that's done. Then I take my fingers and I mold it. Now what I tell patients is I don't want heavy sunglasses on the nose. I don't want them getting facials. I don't want them massaging their nose, popping pimples on the nose, anything that puts pressure on the nose for a few days. Because I don't want the substance molded after I've done it. You can see how that's done. Little by little by little, it already gets raised. And then there are some areas on the middle vault that you can fill up. So it's not just the bridge. Even the tip could be done. The side walls can be done. And then I like to check it again. As a rhinoplasty specialist, as a rhinoplasty surgeon, the aesthetics of the nose are very important to me. It's not just about adding volume and filling the nose up. With my rhinoplasty patients, I do computer imaging and I take their pictures. And using the computer, I show them what is possible and what is not possible. I do the exact same thing with non surgical rhinoplasty patients just so they can see again. When a lot of people go to dermatologists, dermatologists are very good on injections, but they don't necessarily deal with the aesthetic anatomy of the nose. The projection, the rotation, the proportions of the nose. You can see immediately what it looks like. If you can actually bring the camera above, you can kind of show it right from the center, bring it above.
And you can see it really well right there.
You can see her bridge was lifted nice and symmetric right down the middle. And I'm gonna have her sit up for a second so you can go to the front. She'll sit up for me just a second.
And you can see how the bridge.
Is raised up and looks like a.
Piece of cartilage was placed there nicely. And it just took a matter of a few seconds.
And this is something that she's gonna.
Do once or twice a year.
That's all it is.
Go to Times Square. Take a picture of me with a Kodak. Took my life? From negative to positive. I just want y' all to know that. And tonight, let's enjoy life. Pitbull. Naya Neo.
That's right.
I will not love you tonight? Give me everything tonight? For all we know? We might not get tomorrow? Let's do it. Ton.
