Interview (Peter Attia with Rick Johnson) exploring how fructose metabolism drives metabolic disease. Key mechanistic points: fructokinase (KHK) rapidly phosphorylates fructose to fructose-1-phosphate causing transient intracellular ATP depletion; AMP is degraded (via AMP deaminase) to uric acid, which induces mitochondrial oxidative stress, inhibits aconitase and fatty acid oxidation, increases citrate (promoting lipogenesis), and impairs AMPK — collectively promoting fat storage, insulin resistance, appetite (via leptin resistance) and metabolic syndrome. The body can also generate fructose endogenously via the polyol pathway (aldose reductase converts glucose→sorbitol→fructose), induced by high glucose, elevated uric acid and increased plasma osmolality (salt/dehydration). Experimental evidence from rodents (fructokinase knockouts, sugar in drinking water, pair-feeding) shows fructose (and endogenously produced fructose) promotes fatty liver, insulin resistance, hypertension and kidney aging; blocking KHK or vasopressin V1B signaling prevents many effects. Practical takeaways: avoid sugar-sweetened beverages and added sugars; whole fruits (small amounts of fructose, fiber, flavonoids) are generally safe; dried fruit and fruit juices are higher-risk. Therapeutic angle: KHK inhibitors (industry programs, some phase II data) and uric-acid–lowering strategies are being explored.
