Episode hosts Peter Attia and Andrew Huberman jointly run a journal-club-style discussion. Two main papers are discussed: (1) Keys et al. (retrospective cohort, Danish registry) re-assessing the 2014 Bannister et al. finding that metformin-treated type 2 diabetics have a survival advantage versus non-diabetics. Keys et al. used larger samples (≈500k registry), singleton matched controls and discordant twin analyses, and sensitivity checks for informative censoring; their results do not replicate a protective effect that overturns the mortality disadvantage of diabetes, and hazard ratios favored higher mortality in diabetics on metformin after adjustments. (2) A bioRxiv experimental fMRI study (Gu/Perl et al.) testing belief effects: smokers inhaled identical low-dose nicotine via vape but were told they received low/medium/high doses; subjective reports and thalamic activation (and thalamus→vmPFC connectivity) scaled with instructed belief, showing dose-dependent belief effects on brain responses despite identical pharmacology. The hosts also discuss how to read papers (figures-first approach, check methods, supplements, power and confidence intervals), limitations (confounding, informative censoring, lack of aging biomarkers), and implications for geroprotective claims and clinical decisions (TAME trial, ITP animal data, metformin/rapamycin/acarbose outcomes).
