Podcast interview (recorded Nov 6, 2023) of Dr. Harold Burstein by Peter Attia covering breast cancer fundamentals and clinical practice. Key points: US lifetime risk for women ~1 in 8 (~12%); ~250–300k invasive breast cancer cases/year in the US with roughly 38k deaths/year (speaker cited ~275,000 cases/38,000 deaths); overall cure rate for newly diagnosed disease ~80–85% (implying ~15–18% fatality risk). Major biologic subtypes: ER-positive (most common, ~70–75%, peaks ~age 65), triple-negative (~10–15%, skews younger and higher in Black women and BRCA1 carriers), HER2-positive (~10–15% of cases; trastuzumab is transformative). Precursors: atypical hyperplasia (ADH/ALH) and in situ carcinomas (DCIS stage 0; LCIS as a field-risk marker). DCIS: often mammographically detected (calcifications); ~15–20% of DCIS diagnosed on core biopsy are upstaged to invasive cancer at excision; majority (~80%+) of DCIS are ER+. Radiation after breast-conserving surgery lowers local recurrence but treating DCIS has not clearly reduced mortality; anti-estrogen chemoprevention (tamoxifen) reduces incidence modestly (absolute gains small) without survival benefit. Staging follows TNM (tumor size, nodal status, metastasis); node involvement is a major prognostic factor. Stage IV median survival cited ~5 years (varies by subtype). Genetics: ~8–10% of breast cancers related to high-risk germline mutations; BRCA1/2 are high-penetrance (substantially increased lifetime risk), PALB2, CHEK2, ATM and others account for additional hereditary cases. Multidisciplinary care and pathology/radiology review are emphasized.

Questions this episode answers

About 1 in 8 (≈12%).

Speaker cited roughly 275,000 cases and roughly 38,000 deaths per year (ballpark 250,000–300,000 cases).

Speaker estimated curing about 80–85% of women with breast cancer, implying ~15–18% mortality/recurrence risk overall.

ER-positive ~70–75%; triple-negative ~10–15%; HER2-positive ≈10–15% (HER2 collectively ~15%).
