This is a transcript excerpt from Drive Podcast AMA #73, where Peter Attia and colleagues answer rapid-fire listener questions about preserving brain health, optimizing exercise programming (zone 2, VO2 max, HIIT, lactate testing), and body composition strategies. In this chunk Attia frames dementia risk as modifiable vs non-modifiable: age, sex (women ~2x higher Alzheimer’s risk) and genetics (APOE4) are non-modifiable; key modifiable drivers include metabolic disease—obesity (≈60% relative increase in dementia risk), type 2 diabetes (≈50% increase; ~25% additional risk per 5 years of diagnosis), hypertension (≈30–60% increase depending on duration/severity), and LDL cholesterol (each 1 mmol/L / ~40 mg/dL increase ≈8–10% higher all-cause dementia risk). He states RCT and mechanistic evidence support causal links and notes APOE4 carriers are substantially more susceptible when metabolic disease is present (speaker: five to 5.5-fold higher risk in one comparison).

Questions this episode answers

About a 60% relative increase in risk.

About a 50% relative increase; additionally, for every five years with type 2 diabetes, risk increases by almost 25%.

About an 8–10% increase in all-cause dementia risk.

Statin use in RCTs is associated with about a 20% decrease in all-cause dementia risk and about a 30% reduction for Alzheimer's disease in some analyses.
