057- Neck Pain: Adjustments Crush Mobilization
11 Nov 2025 · 0 min
Dr. Jeff Langmaid reviews a study comparing three neck pain treatments. Spinal manipulation and mobilization both reduced pain at rest, but manipulation produced the biggest gains in range of motion and pain relief.

Hello and welcome to the Evidence Based Chiropractor. I AM your host, Dr. Jeff Langmaid. Happy New Year. I know that we have been looking forward to the turn of the year here at the Evidence Based Chiropractor because I have a ton of awesome things on Tap for 2017 and I hope it's the same for you as well. Hope you had an opportunity to sit back, recharge the batteries, hang out with your family and get ready for 2017. One thing I'd like to talk about right at the top is I've had a ton of requests for more patient front material. So you guys, I know love the research and love what we can do for chiropractic and chiropractors but we also would love to get some or I've had a lot of requests for some patient fronted or patient centered material and I am happy to say today on the podcast that is coming in 2017. So you hear a lot more about that coming up. Also, big shout out to Dr. Bo Pearce and Circle of Docs. I was humbled, honored. I was included in their list of the top 10 chiropreneurs to watch in 2017. So if you don't already check out Circle of Docs, I recommend doing so. They're pumping out tons of content each and every day and I was very humbled and proud to be a part of that. And today on the Evidence Based Chiropractor podcast, we are back to the research. We are going to talk about a study titled Mobilization versus Manipulations versus Sustained Apophyseal Natural Glide Techniques and Interaction with Psychological Factors for Patients with Chronic Neck A Randomized Controlled Trial. Wow, that was a mouthful. I'm gonna say that one more time. It is titled Mobilization versus Manipulations versus Sustained Apophyseal Natural Glide Techniques and Interaction with Psychological Factors for Patients with Chronic Neck A Randomized Controlled Trial. This was released in April 2015 by the European Journal of the Physical and Rehabilitation Medicine. So again, brand new article April 2015 European Journal of Physical and Rehabilitation Medicine. So this is cool because it really pits mobilization, Manipulation and apophyseal glide for neck pain. So there can be a lot. Sometimes there is a lot of question on which is best. So this takes a look at all three and puts it with that so many individuals suffering from chronic neck pain experience a significant reduction in quality of life. We know that patients with neck pain have a tough time with their quality of life and this paper is going to show us which of the techniques or which of the interactions yielded the best results. So before we get started, I'll say a few words about Rocktape Roc Tape, makers of the world's best kinesiology tape are proud to introduce completely redesigned Rocksauce and Roxas Chill powerful topical muscle and joint pain relief now available in 3 ounce roll ons and 32 ounce clinical pump tops. Give long lasting pain relief to your pat. Learn more at rocktape.comtopicals Again that is rocktape.comtopicals so with the research, the resulting disability from neck pain can affect both the professional and personal lives of patients. We see it all the time, every day in practice. For many healthcare providers, the decision on appropriate treatment modality can be difficult. Obviously we know chiropractic is underutilized, there is no question about that. When 11% of only 11% of primary care doctors have referred to chiropractors in their career, that is truly a travesty of the greatest kind. So research like this is very important to be able to showcase. We showcased this through the Evidence Based Chiropractor membership. If you are an active member, you received this in March of 2016. If you're listening to this and you are not an active member, which there are thousands of you listening as not active members, I'd encourage you take a look at the evidencebasedchiropractor.com, click the join tab. See what we have to offer. If you're serious about growth in 2017, the opportunity is ripe and the opportunity is to build referral relationships with other providers in your community. Nobody does it better than us at the Evidence Based Chiropractor and we give you all the tools A to Z, everything you need to know to get started with absolute efficiency. So I'll kind of hold that and we'll go dive back into the research. So this study evaluated the efficacy of mobilization, spinal manipulation and natural glide for patients with chronic neck pain. They found researchers that spinal manipulation provided the greatest breadth and depth of improvement. Manipulation was able to increase the overall cervical range of motion while also providing the greatest reduction in pain intensity at rest. Research like this continues to prove manipulation to be an extremely safe treatment option. So let's take a look at a few quotes directly from this paper. The researchers found all the manual therapy techniques studied produced a decrease of pain during active movement and an increase of cervical range of motion and local hypoalgesic effects. But only the manipulation and mobilization groups experienced pain relief at rest. So this is very important. So when they looked at all three techniques, all three techniques had some improvement. But manipulation and mobilization were the only ones that had that improvement. With the addition of pain relief at rest, they also found. The researchers also found under low anxiety levels, a better prognosis is expected after manipulation and snag technique intervention. So that's the apophyseal glide. So under low anxiety levels, a better prognosis when you take manipulation or apophyseal glide technique intervention. So that's also good. They also found Quote Here we go. HVLA high velocity low amplitude manipulation group showed more reduction of pain intensity in rest and more increase of cervical range of motion than the others. High velocity low amplitude. Again, high velocity, low amplitude manipulation group showed more reduction of pain intensity and rest and more increase of cervical range of motion than the others. A little bit of interesting wording from the researchers there, but the bottom line is this paper showcased mobilization technique, manipulation technique or an apophyseal glide technique for chronic neck pain patients. And the truth of the matter is, with this paper, manipulation was able to get the best overall increase of cervical range of motion, which makes sense. If you're manipulating the joint, you should get into that paraphysiological space. We should see a better increase in overall range of motion. And they do. They also found that on the VAS scale, when they looked at high velocity low amplitude adjustment and rotational score at rest or active high velocity low amplitude manipulation, an adjustment received the greatest amount of pain relief, the lowest pain intensity during activity, and the lowest pain intensity at rest. So this is an awesome paper because it showcases mobilization techniques which are gonna be common in physical therapy offices, manipulation techniques, which are the core adjustments, chiropractic adjustments, the core of what we do as chiropractors, and some natural glide techniques, which are really more in the physical therapy realm. Now, we as chiropractors also might choose to do more than one technique, but this paper really puts a framework that showcases that cervical manipulation provides greater range of motion and provides greater pain relief than the others for patients with chronic neck pain. And also that's an interesting facet of this study as well. When we look at chronic neck pain, that tends to be the most difficult to treat. Sometimes that is when patients may think, oh, well, I don't know if a chiropractor is going to work for me. I've been dealing with this so long, it must be so bad. The arthritis is so bad. But the bottom line is with chronic neck pain, there perhaps is no better, no more efficient and effective and no more safe technique than a chiropractic adjustment. So I love this paper. I'd love to know about what you think about this paper. Shoot me a comment, Jeff the evidencebasedchiropractor.com Let me know of any papers that you'd like me to highlight as we continue to move forward and let me know what you think would how good of a job are we doing so far? I absolutely love to interact with you guys. I love to have feedback. I'm going to be on the road a ton coming up here in 2017. I'd love to meet as many chiropractors as possible in 2017. Continue to serve at the highest level. So shoot me an email jeff the evidencebasedchiropractor.com let me know any studies you think we should highlight. Give me some feedback regarding the show and certainly when you get a moment, as soon as this ends momentarily, go to itunes. Give us some feedback. Give me a rating. That helps us get found by more and more chiropractors which only helps continue to elevate the profession. There are thousands of you listening out there as I speak and I have a couple dozen reviews. I'd love to get up to 25, 35, 50 reviews let's say by the end of the year. So if you want to be one of those, I would love your feedback. Give us an honest feedback. Give us some honest feedback at itunes and a rating. So I hope that again you had a great holiday week. I cannot wait to hit the ground running in 2017. Big thanks again to Bo and everybody at Circle of Docs for highlighting me as a top 10 chiropractic entrepreneur in 2017.
