086- Chiropractor at a Surgical Conference
11 Nov 2025 · 0 min
Podcast episode (host Dr. Jeff Langmaid) describing his presentation about chiropractic research to a largely surgical audience at his company’s all-physician retreat. He summarizes four research-backed mechanisms/structures affected by spinal manipulation: multifidus recruitment (stability), lateral thalamic neuron thresholds (nociception/pain relief), facet (Z-joint) gapping (mobility/adhesion breaking), and central motor system facilitation (motor evoked potentials). The episode also includes promotional content for Rockstock 2017 (discount code "groovy").

Hello and welcome to the Evidence Based Chiropractor. I AM your host, Dr. Jeff Langmaid. This week is all about the research and it is about how chiropractic adjustments work. And the story behind this is this weekend. Just yesterday I had the opportunity to present to neurosurgeons, physiatrists, orthopedic surgeons, executive team members at my company's annual all Doctors retreat. And yes, they asked a chiropractor to present about chiropractic at this event. I was humbled and honored to do so and I wanted to share a few, really four of the key points that I shared with them regarding the research behind chiropractic care. Before I do, I will say a few words as I do each and every week about Rock tape. Rock stock 2017 coming up September, Huntington Beach. Come hang out. I'll be interviewing people all weekend long. The continuing education is unbelievably awesome. The location is killer. It is an event that you do not want to miss this year. So check it out. Rocktape.comrockstock Pick up your tickets. There are still if you enter the word and I'm going out on a limb here, you can enter the word groovy G R O O V Y and at the link in our show notes and you'll get 100 bucks off your ticket. 100 bucks off your ticket. That'll get you like halfway there on a plane, if not round trip if you find a good deal. So rock stock 2017. Check it out. But this week is really all about research and it's about four structures. Again. I had the opportunity to speak at our annual all physician conference, really a surgical conference by and large. And they asked me to talk about chiropractic care and chiropractic research. The response that I got even more interesting. Not only asked to do so, honored, flattered, massive gratitude. The second component is I had multiple surgeons coming up saying thank you. That was interesting. I didn't know that the adjustment did that. That's cool. I had a video that showed what it's like in a chiropractic office and it was really eye opening. A lot of these guys and gals just don't, don't have the exposure to what we do. They just don't know it's outside of their wheelhouse. So it was a really great opportunity to kind of jam with them and highlight what we do at the highest level each and every day in practice. And it comes down to four structures. Multifidus, lateral thalamic neurons, facets and central motor system. So multifidus, we know multifidus, big time stabilizer in the lower lumbar spine. Fatty infiltration makes the multifidus work like garbage. So what they found, what they and I say they Spine journal, top tier research paper in 2011. They had an article that came out titled Preliminary investigation of the mechanisms underlying the effects of manipulation. And what they found was a SMT force, a adjustment force, a manipulative force has been quote, A manipulative force has been shown to stimulate peripheral afference, Altering central nervous system input and enhancing motor neuron excitability. They also found, quote the underlying mechanisms explaining the benefits of SMT appear to be multifactorial. Both spinal stiffness characteristics and lumbar multifidus recruitment changes appear to play a role. Very, very important because multifidus recruitment, that's big. That's recruiting the muscle fibers in the low back that are essential for stability. And if somebody's decontamina, if there's fatty infiltration, you look at that axial slice on an mri, if you have it, it's white, not dark. That means the multifidus is not functioning well. So you need the best recruitment of those fibers possible. Chiropractic adjustments do the trick. I'm gonna keep moving. Number two. We talked about lateral thalamic neurons JMPT in 2014 paper titled Effective spinal manipulation Thrust magnitude on trunk mechanical activation thresholds of lateral thalamic neurons. And what they found was Relative to control the 85% body weight thrust magnitude adjustment Significantly increased mechanical thresholds of lateral thalamic nociceptive specific neurons to dorsal ventral mechanical testing of the trunk. What does that mean? Well, this was an award winning paper in 2014. And what they're basically telling you through this is, hey, that's how pain relief is found. There's a stimulation to certain neurons and a alteration of the threshold of other neurons. And when there is an alteration on the thalamic neurons, that can give better, so to speak, breathability or breathing room before pain ensues. So that's lateral thalamic neurons. The adjustment impacts the nervous system. Number three facets. This is the easy one. This was spine journal in 2002. The effects of side posture, positioning and spinal adjusting on the lumbar Z joints. Bottom line, they opened up the Researchers found, quote, hypomobility may be the result of injury, inactivity or repetitive asymmetrical movements. We talk about that all the time. They also found, quote, unquote gapping breaks up adhesions, thus helping the motion segment reestablish a physiologic range of motion. What these researchers also found, not coincidentally, was the patients receiving chiropractic adjustments in the lumbar spine had better gapping than the control group. So when a chiropractic adjustment, we know this, but when a chiropractic adjust proved it, when a chiropractic adjustment's given in the lumbar spine, the Z joint, the facet joint gaps and that is great for breaking up adhesions, decreasing pain and establishing a better range of motion. We also talked about central motor system jmpt again in 2002, central motor excitability changes after spinal manipulation. A transcranial magnetic stimulation study. And what they found was this data suggests that there is a post synaptic facilitation of alpha motor neuron called corticomotor neuron or both that may be specific to the high velocity low amplitude thrust. Another award winning of pieces of research examining the physiological effect of an adjustment via central motor facilitation. And this research article highlighted the mechanism of action based on the alteration of ultimately motor evoked potentials after the chiropractic adjustment. They're talking again about it being multifactorial, which makes sense. The multifactorial impact of an adjustment is probably why patients with a variety of complaints respond well to treatment. It impacts multiple structures, multiple tissues across multiple mechanisms, thereby helping multiple people get well and stay well. We also talked a little bit as we wrap about Golgi tendon organs. This was a spine journal paper again top tier 2004 effective spinal manipulation duration on low threshold mechanoreceptors in lumbar paraspinal muscles. And what they found was in addition, spinal manipulation both increases the excitability of motor pathways in the spinal cord and depresses the inflow of sensory information from muscle spindles assessed during transcranial motor evoked potentials and H reflex, respectively. So this paper found that load time or impulse duration of manipulation produces a unique activation response on the Golgi tendon organization. This unique activation is thought to contribute to the therapeutic effect. So that's a lot of information in a short, short amount of time. But I wanted to give you the inside track on what I have found to be very powerful research that is very clear on the chiropractic adjustment affecting and influencing very specific structures. So this is a presentation that I used again just this weekend, literally yesterday, speaking at my company's annual All Physician retreat. And I'll tell you, I was not moving as fastly through the presentation as I did right now. We went into detail on the structures, we told the story of chiropractic history and at still with osteopathy and DD with chiropractic and short lever versus long lever and how over time chiropractic has evolved and some of our core beliefs, some of our core statutes and principle principles remain the same and that many have evolved with the knowledge that we've gleaned. But ultimately the story came back to something that I talk about all the time. The breadth and depth of chiropractic research has never been better and we are just at the tipping point. So this is the tip of the iceberg. We have not even barely sort of scrape the surface in a matter of speaking in terms of what's out there and what a chiropractic adjustment does. And to date the research has been profound, the research has been supportive and the research has gotten behind what we have known for a long time as practicing chiropractors that many people can get well with what we do, that it is safe, that it is effective and that people like it, that there is high patient satisfaction. So those are a couple take home messages that I had. I would absolutely love it if you took a moment give us some feedback on itunes. I just checked in. We're hovering on the top 50 alternative health podcasts in the world on itunes. So thank you for checking it out. Thanks for listening. Each and every week. I'll continue to deliver marketing and research in the time it takes to get to your office. And every Monday morning at 6am Give us a little feedback. Give us some love over on itunes that helps keep everything fresh and relevant from the itunes charts perspective and I would greatly appreciate that. I hope you have a fantastic week in practice and I will talk to you soon.
