In this episode of The Evidence Based Chiropractor, host Dr. Jeff Langmaid reviews a 2026 pilot randomized sham‑controlled trial that examined whether thoracic spinal manipulation combined with trigger‑point therapy could alter inflammatory cytokine and neurodegenerative biomarker levels in people with relapsing‑remitting multiple sclerosis (MS). The study enrolled 21 stable, ambulatory participants (11 receiving chiropractic care, 10 receiving a sham activator procedure) who were treated twice weekly for four weeks. Blood samples were taken at baseline, 20 minutes and 2 hours after the first and final interventions, measuring 21 inflammatory cytokines/chemokines and three neurodegenerative markers, alongside clinical outcomes (pain, fatigue, mood, sleep, walking speed, hand dexterity, cognitive processing speed). Results showed moderate‑to‑large effect‑size reductions in six pro‑inflammatory cytokines favoring the manipulation group, with evidence of cumulative biomarker shifts across repeated treatments. Neurodegenerative markers showed a modest decrease, while clinical outcomes changed only minimally. Dr. Langmaid interprets these findings as early mechanistic evidence that chiropractic manual care can influence systemic immune signaling, beyond local pain pathways, and discusses how such data can broaden the narrative of chiropractic care toward neuroimmune modulation. He also highlights practice‑growth opportunities (hybrid programs, shockwave devices, new tables, OA recovery software) and urges clinicians to communicate these mechanistic insights to patients without over‑hyping the results.
