Joanne Smith questions why an extra minute of activity triggers severe 9-10 pain flare-ups with no visible injury. She argues that simple activity explanations don't match what patients actually experience, and that careful assessment is needed to find what's really driving the pain.

So realistically, does an extra minute or an extra 5 minutes, or you thinking that you've done too much, does that really explain the severity of the flare-up that you might have on the back of that? Realistically, when we spend time being curious and really assessing what's really going on, we find huge, huge inconsistencies in what's possible and what's not. Um, because often people's flare-up, the severity of the symptom is, is it can be up to like 9, 10 out of 10 if you're going to score it on a, you know, on 10 being the worst it could be. It doesn't actually explain the severity, and we've not had any obvious knocks to the body or trauma or injury. Um, so physiologically, when we actually get to the nitty-gritty, these things don't always make any sense.
