Podcast episode titled "HEP's and Memberships" (recorded July 6, 2024) featuring Dr. Kelly Alhooie. Kelly explores whether clinicians should charge for home exercise programs (HEPs), contrasts HEPs (therapy-focused, problem/goal-driven) with personalized performance plans (trainer-style, performance-driven), and discusses memberships/continuity programs and software as ways to create recurring revenue and combine online and in-person services. Welcome to Pelby biz podcast.
I'm Dr. Kelly Alhui.
I have grown and established brick and mortar practice in Northern Virginia called Ortho Pelvic Physical Therapy and also founded Pelvibiz, which is an online business consulting company that helps healthcare providers make more money and be healthy while doing it, especially in the pelvic health realm. So let's get into today's episode.
Today I'm going to be talking about should you pay for an hep? So should you. Well, should you charge, I should say, should you charge for an H E P? So a home exercise program, should you actually charge for that? And this question had me really thinking, traditionally it does take time for us to go to Medbridge. Whatever program you use, you got to select the exercises, you got to be intentionally edited. You got to write in there what specifically is going to be, you know, what they're specifically going to be doing. You then have to email it or print it out for them. Like, it takes a lot of time in a lawyer's perspective. If I was a lawyer, I would 100% be charging for that because it's my time. Right? So, so what makes the difference between why do we as health care workers just say, it's just part of the service versus if I was a lawyer saying, no, we're going to actually charge for this. So I really had this. I've really been thinking about charging for H E PS what would that look like? And you know, what is that? And then if we do that, do we put more responsibility on the patient while in session to be able to take notes or, you know, take initiative for making sure that they are complying with whatever we're saying at home? Right. Why do we as healthcare workers take it upon ourselves? It's like, oh, we'll write everything out for you, we'll get it all organized. It's very nice of us, but at the same time, yeah, why aren't we charging for that? Because that's an increased value and everything is laid out nicely and able to go from here to here. So I was just like really pondering that and thinking like, what is it? And so the other thing I was thinking of is like, memberships. How are we, Are we positioning memberships after clients leave potentially our facility, or maybe even take our aligned program and then they're done with it? Are you having that in some kind of membership where they're doing some kind of community building, or are you providing a tool each month or a resource? Like, what are you actually doing for that that's added value right there. And it will also help stabilize your clinic or online business because it's constant revenue that's actually coming in. So you want something like that. Software is also another thing that takes you from being consistent in the fact that if you have software, you sell it or you make it. You sell it one time and then you can repetitively sell it. So that's, you know, another thing for when it comes to continuity. Now here's the next question I kind of asking myself. And this is kind of just like an open talk to my. To myself really, that I'm kind of letting you guys in on. What's the difference between a HEP and basically a personalized performance program? Or like, what is the difference between that? And. So I was kind of toying back and forth between this and to me, an HEP would be something like a home exercise program based on therapy, based on like an actual goal or problem. A home performance plan is, is more of like a personal trainer where you're giving them really higher level exercises and you're taking them to the next step. Many people these days do have virtual personal trainers now rather than in person. So they could be traveling, they could be, you know, at their home, and the person just tells them, is there to tell them what to do. That is valuable for people. People charge anywhere between $100 a month to maybe $400 a month for that. And, and you know, you can customize that for people. So, you know, are you doing that at all? Have you thought about adding this into your business? Those are the questions I have for you. Have you ever thought about charging for H E P? Like really? That could be something very, very valuable. So, you know, that, that's to me, very intriguing where I'm like, maybe I'll make some changes. You know, I'm always growing and changing. So I'm always looking at things of how can I. How can I change and shift with, you know, everything that's going on? So these are kind of the things that I'm thinking about is how to create more continuity and stability with different outside the box thinking ideas that some people are kind of afraid of because we're traditionally in the model of, you know, let me pay per visit, let me do packages. Which is fine, we do packages as well, but like packages and then we're done once we solve the problem. But the reality is that person is in their lifetime gonna probably have another two to three to four more problems. And you want them to call you back because they Remember how good the customer service was and they're like, yes, I need you to, you know, help me because you helped me the first time. So how can we, you know, how can we do that? And these are kind of ways with a continuity program, performance program, a membership of how you're going to move people and get them going down the right path with a continuous revenue stream coming in the door. You can also look at this from a coaching aspect and the fact of, you know, let's just say you did. We're a triathlete coach, not a pt, a coach. And you were planning out how many miles this person should run, what they should be eating, when they're going to start tapering off, when they're going to start increasing their performance. Really looking at that, that is something that people want and that is on you can do that online. And so how can you do that? How can you do that to where it, you know, is incorporated already into the things that you're doing in person. But it is a compliment to an increase in revenue stream and an add on to people that already know, like and trust you. So I think it's a really interesting concept that I've been kind of toying around with. I don't have all the answers yet coming up with the answers but. But I just want to kind of throw that out there and please comment below if you guys, you know, have any ideas of what potentially you might be doing. But I want to kind of throw that out there because I really like to have online and brick and mortar work together and double dip instead of just hitting it from the brick and mortar side or just hitting it from this side. So just making sure that, you know, we're not running in circles and we're able to, we're able to whatchamacallit, be able to hit it from multi faceted approach and make sure that we're not wasting our energy. So think that's huge. Just want to come on and kind of talk about that and what I've kind of seen there's different apps for this that people have used. There's about four different apps that you can use to help with this that already have potentially exercises built out in there and videos, you can also customize and use your own videos. So there's lots of different things that can be had. But yeah, we've got to start combining online and in person. It's the way it's going and AI is going to even make it even faster happening for all this and be able to you know, have access to whatever people need and have the information. So hopefully this helps. Gives, like, a different perspective on should you charge or not for hep or not. You know, the conclusion here is it's up to you, but how does that look like and what structure actually works for you? I just wanted to kind of brainstorm and give you some ideas here, but that's kind of what I'm thinking on my head of where, like, things are going to be going and how can we continuously keep progressing patients with our knowledge and provide amazing results. So hope you guys have a great rest of the day and we'll talk soon.
