How to Build a Profitable Online Healthcare Practice
4 Apr 2026 · 14 min
In this solo instructional podcast, Kelly Alhooie outlines a step‑by‑step framework for clinicians—physical therapists, chiropractors, occupational therapists—to launch and scale a profitable online healthcare business in 2026. She emphasizes that success hinges on becoming a top‑tier digital marketer and mastering remote result delivery. Key components include aggressive cold outreach (100 DMs daily), building and borrowing Facebook groups for community, selecting a hyper‑targeted niche, and deploying a comprehensive tech stack (Zoom, funnels, landing pages, email lists, and a specialized CRM called PelviBiz Solution). Alhooie differentiates teletherapy (HIPAA‑compliant, note‑heavy) from virtual coaching (influencer‑style, cross‑state, no notes) and stresses the need to overcome client objections about non‑touch care by demonstrating confidence and measurable outcomes. She provides concrete revenue benchmarks (US$3‑11K in three months, US$10‑12K per month after a year at a $200 price point) and recommends hybrid growth tactics such as in‑person events and high‑ticket retreats to accelerate lead warming. Lifestyle advantages—location freedom, flexible scheduling—are highlighted, alongside the reality that clinicians must either adopt organic marketing tactics or invest in paid ads. Throughout, Alhooie offers actionable advice, tool recommendations, and invites listeners to DM her for personalized help.

All right, today I'm gonna talk about how to build an online business in the healthcare space in 2026. So if you were thinking about going online, this is what we help people with to be able to actually get patients, not just to sell a course, but to actually have a business that thrives, that you can make money, you can be anywhere you want, and you can also get patients the results. So if you are a physical therapist or chiropractor or occupational therapist, this video is for you today. So here we go, let's get into it. Number 1, if you want to be online, you have to think of yourself as a massive digital marketer, period. You have to be the best digital marketer to be known online. I've seen it over and over again. People just assume that they're going to create this course because you guys are amazing clinicians and you believe that because I'm so amazing, I've gotten people results, that immediately people are going to buy the stuff. And that does not happen. And so I am seeing this, uh, switch from courses to actually implementation and certainty, moving more into the online space, which I think needs to be had for more consumers to actually want to do, um, online. And by the way, I should say this, I have people in my program currently that strictly just do online businesses and they work. I have people that do hybrid, they work. I have people that do just in person, they work. Every model works. It's just that you're going to have to have different skills for different models. And so if you want to be online, you have to be the best at marketing, and you have to be good at getting the results online. And I think most clinicians feel not confident in getting someone results because in the schools just taught us hands, hands, hands, hands. And they did not teach us online. So not only do you have to learn a new skill of making sure that you're getting the result without touching the patient, but you also have to learn how to get your brand and awareness and your name out there and build community all online. And by the way, if you do online, that doesn't mean you also can't do in-person, meaning especially if you like move, you're a military spouse and you move every year and a half. Well, guess what? You got a year to freaking build, even if you're in a place for like 3 months. Guys, when I would come out here to California and I wasn't here full time, it's like, cool, I got an opportunity to build a community out here, you know, in person, because I can like meet people. Like, that's cool. And by the way, you can build a community— you don't have to be in person, you can be anywhere. I also coach people that live here in California and are building a business in Hawaii. I coach people that lived in Virginia and then also are building a business in Miami, Florida. So you don't have to be in the location to build the business, if that makes sense. Um, and so I'm going to break down— I have it on my whiteboard here— I'm going to break down what needs to be had that you have to do if you want to do teletherapy or you want to do virtual coaching. Coaching you can do all across state lines. You're basically doing what influencers do. They help, they help people and inspire people to get the results that they are looking for. Cross state lines, there's no HIPAA, there's no notes, nothing. Highly recommend doing that. Or you can be under the strict rules of teletherapy where there's HIPAA, you have to do notes, um, and it just, you know, is teletherapy. Now, the first thing I need you guys to do is you got to be good at cold outreach. You got to outreach 100 times a day to people. You have to be a wizard at going to find people online. You have to be a wizard. If you're all online, you have to know online. So how are you going to do those cold DMs? You got to know that. And if you don't, message me. I can help you. Okay. Second thing is you need a very targeted niche, especially because online is even a bigger space than if you were in person. So you need a niche and you're going to talk to them daily. Now, if I was you and I was not, you know, 'Oh my God, I gotta DM 100 people a day, that sounds insane.' Well, that's what it takes. If you wanna really grow a nice business online, you gotta do that. The other thing I like to do is Facebook groups. Get everyone to join a Facebook group so that it is very cohesive. And at any point in time, you can go into your specific Facebook group and be like, 'Hey guys, you wanna do this event? You wanna do this? Cool.' That's what I would do. And so, Um, yeah, that's, that's literally what I would, um, try to do. And I did that with like— see, I did like C-section mom group. I like— those were my clients, and I would just pull from them and be like, hey guys, you need help? Okay, cool. And I put a message, and then bam, I'd get like 3 clients from my own Facebook group that I built. I didn't try to sell them at first. I just try to get them into the group. That's it. I'm just trying to build community, and that's group. Okay, number 2, if you don't know how to get people into the group or, you know, whatever, then you need to borrow people's Facebook groups. So go to other people's Facebook groups where your clients are and then talk in there with them until you create your own group. That's, that's key. Number 3, you must know tech. I know most you guys are gonna kill me, but you must know tech. You must know tech. You must know tech. I can't say this enough, okay? You must have a CRM, which we use something called PelviBiz Solution that we have created for all orthopedic and pelvic floor physical therapists and chiropractic, okay? That we have created this, that it tells you how to talk to people. It tells you how to email people. It tells you what to say in a text message if someone's trying to cancel. It asks for Google reviews. It does all your marketing. So if someone doesn't book for you, it does all the backend marketing. It does your newsletters, it does your funnels, it does— okay, you want to try to sell something? Okay, great, you can put a product in there. It does it all. Okay, you must have that if you're online. That's like, uh, it must be built out. Okay, and we already, by the way, have it built out. We give it to you. Okay, now you must know how to work Zoom because you must have things recorded so that you can use that over and over and over again to be able to get X result. Again, you must have funnels, you must have landing pages, you must know how to, how to do this. And if you don't, this is why we're here. We're here to help you because we've already built it out and say, here you go, you can edit ours if you want and put your, you know, words or cute picture on it. But basically we've already come up with the strategy of how to execute the whole system because it's not about just like putting something on a page. It's about the whole pre-launch, the launch, the after the launch, the follow-up. Like it's all the whole system and strategy that all goes together. And I think most people are like, just, oh yeah, I guess I need a funnel. And it's like, but there's so much strategy behind that. Okay, and that's where we come into play. You have to build an email list. An email list, at any point in time, 2 to 5% of people in your list currently wants to buy something from you. But if you're not emailing them, how can they buy anything from you? They can't. So that's key. And then making sure that's very strategic and making sure like, you're actually putting offers in front of the people that want the offers and not every single person because they may not need it. It's a whole thing. Number 4, converting online and showing that there is value into online. Some people love it because it's convenient and they don't leave their house. Some other people are like, no, how do you know you're not touching me? I see the value in touching. And so you have to overcome that objection And you yourself, like I said, have to be so confident that you can get them results without touching them. You can. Why can't you? We have that clearly laid out per condition, per diagnosis, that this is what they do this week. And it's all online because I've done it and I've gotten tons of results. So I know it's possible, but I need you all to think that it's possible. Okay, so With that, um, and again, the biggest thing is you gotta get them results. They need results. That doesn't mean also too that you have to deliver all the results, but you can be the person that creates the team that delivers the results. So maybe you're not so good about talking about, you know, sex with their partner. Okay, cool, refer them to a sex psychologist that you have in your back pocket so that together you guys cohesively can actually make the thing that they want maybe to have, you know, pain-free sex and have an orgasm actually happen. And then every single person wins. That's key. So I would say typically this is how it goes, 'cause you guys a lot of times are like, Kelly, well, what's the timeline? What does this look like? Within 3 months, you should be making $3,000 to $11,000. That's if you do everything I just said to do, okay? $3,000 to $11,000 in 3 months if you do everything I just said to do. Create your groups, deep cold DMs, all the time. That is what you're doing. That is your job. Okay. Um, within 1 year, typically people make anywhere between $10K to $12K months, just letting you know. And that's for if people are at the $200 price point online. This is what we've seen. Um, in-person events do really well to connect, make the connection to then bring them online and do like the treatment or like the protocol online, but connecting with people as quickly as possible in person, they go from a cold lead to a warm lead to a hot lead within 1 hour. Conversely, online, if you're in someone's DMs coldly, it is going to take— it could take a week, it could take months, it could take a year. And so it could be a little bit of a slower process depending on how the you know, depending on the niche that you're targeting and the prospect that you're trying to hit up. Another thing I like to do for online is do retreats, do retreats. So create your community online, but then bring them into an in-person event that— or like, you know, a 3-day or 2-day thing that people want to connect. This is where the trends are going. They want to connect. They want to feel connected with other like-minded people. And if you can provide that community and you charge like $1,000 per ticket, think about the difference. If you charge $1,000 per ticket and you get 10 people in your retreat in 2 days, guys, that's $10K in 2 days. Conversely, if you charge $200 a visit You know how many clients you're gonna have to see? You're probably gonna have to see like— you're probably gonna— if I— again, I'm not doing math in my head, but you probably see 12 clients a week. 12 times 4 is what? 12 times 4, you're going to see probably over 50 hours of patient care. Again, I can't do math, so if you're doing math in your head and I can't do it, great. Um, 12 times— what did I say? 200 times Yeah, wow, I did really good math. That's amazing. So yeah, 12 clients a week. Okay, that's 12 times 4 is what? 12 times 4, that's 48 hours of patient care. Or you can do 16 hours. Which one would you prefer, 16 hours or 48 hours? You see how retreats can get you there faster to what you want? So I think again we have to think about this stuff, um, differently. Uh, the benefit to virtual is your location freedom. You do not have to be somewhere, you know, at a certain state, and you can just go wherever. Um, and you're not stuck in a treatment room. I would say that's number 2. You're not stuck in a treatment room. You can be wherever. You can get the beach, you can be at a table, wherever you want. Um, and you could help people. You can be on vacation and you pause your vacation to just like take a call and then get off and then go somewhere. So that's the benefit, I think, of virtual, and you can still make money. So, um, but the thing about it is you have to be a digital marketer, period. And I think most providers do not want to step into that role or are scared to step into that role because it's so uncomfortable, because they didn't go to school for that. They went to school to help people And I believe that's why some of them struggle to build online, because they're basically starting from ground zero and other people have gotten like degrees in marketing. Um, and so yeah, or you pay, you pay Google and Facebook, right? And you will pay a big penny to them. Um, if you're like, look, I'm just not willing to go and DM people, well then you gotta pay. And that's the other way to build this. So hopefully that helps, guys, give you some insight online and how to do that. If you have questions, please DM me @pelvibiz, P-E-L-V-I-B-I-Z, @pelvibiz. Say, hey Kelly, I need some help. It is me in my DMs, by the way. Um, so would love to talk then. See you guys soon. Bye-bye.
