Pelvibiz Teletherapy Webinar Pt. 2
20 Nov 2025 · 35 min
A teletherapy webinar (Pelvibiz Teletherapy Webinar Pt. 2) presented by Dr. Kelly Alhooie covering remote pelvic health care. Dr. Alhooie illustrates multiple case studies (male erectile dysfunction, hypertonic pelvic floor, prolapse screening, post‑op Achilles/abdominal weakness, athlete with leakage) to show how targeted coaching questions, home-environment assessments, and remote diagnostic techniques can solve issues that manual therapy alone may not. She reviews tools and logistics (Jane telehealth platform, mobile desks, mirrors, Intimate Rose products), offers a 12‑week teletherapy careplan product (three diagnostic charts + scorecard + scripts) for $2,997 with 24‑hour bonuses, and shares a promo code (PELVIBIZ1MO) for a free month of Jane. Emphasis is on coaching, consistency, one-action homework, and leveraging the patient’s environment.

Welcome to Pelby biz podcast. I'm Dr. Kelly Alhui. I have grown and established brick and mortar practice in Northern Virginia called Ortho Pelvic Physical Therapy and also founded Pelvibiz, which is an online business consulting company that helps healthcare providers make more money and be healthy while doing it, especially in the pelvic health realm. And let's get into today's episode. Okay, so here's another patient we had, okay? This guy, his name's Ed. Ed. He had erectile dysfunction, okay? Ed is a mechanic. He's stressed out, he's tired. And Ed has been a client of ours for six sessions, but all teletherapy. He has never been in our clinic. Just. Just so you know, it's been all teletherapy. That is the difference between the last client and this client, okay? And for him, my therapists were coming to me and saying, kelly, we've done belly breathing. We've done the stretching, we've done. I've showed him how to use the pelvic wand by Intimate Rose with Amanda. So thanks, Amanda. Shout out to Amanda. She's on. Okay, we've done. I've done everything. Like, I think we should just discharge him because, like, we don't know what else to do. And he's kind of stuck. And I'm like, but wait, guys, you haven't gotten him to his goal yet, which was to be able to have an erection for five minutes with it and be able to have intercourse with his wife and for them to be able to reconnect. And I'm like, no, we cannot discharge him yet. We cannot discharge him. We got to figure this out. And so I was like, okay, let me get on with him to see how I can help, right? And here's what I did differently that I think was a game changer. Number one, I asked three important questions. I said, hey, Ed, what makes you happy? Ed's probably like, what are we going to talk about? Stretching and lengthening and doing. Getting blood flow and all things. But I said, no, Ed, what makes you happy? I can tell Ed was very stressed, and he's like, well, you know, like, you know, not this much work. I mean, I like work, but, like, I can't do this much. I'm, like, overworked. I'm like, okay, well, Ed, besides work, like, what makes you happy? He's like, I don't know. Great question. And by me asking. That's just one question, guys. By me asking one question, it allowed the patient to think when we're using our hands, Most of us, when we're using our hands, and correct me if I'm wrong, okay, but when I'm working on someone, what am I talking about with them? More than likely, I'm probably talking about their weekend. I mean, I'm a very friendly person, so I'm probably talking about their weekend. I'm probably over sharing my life. I'm probably hearing about their life. You know, we just get in conversations. But when it's a teletherapy session, yes, you can get in conversations like that. But I'm asking very direct questions so I can truly figure out how am I going to solve this problem. So that was my first question. My second question was this, Ed, what makes you feel good? That was my second question to him. And he was like, well, you know, my wife and I, when we give each other massages, like that makes me feel good. And like I, you know, like, you know, making her feel good. I'm like, okay, well, good. That, that's awesome. That's really cool. And then I asked them this last question. I think this is the most impactful question. And some of you guys, you may not be comfortable with this. I've treated males for a very long time, so I treated males for like 15 years, so I'm very comfortable with males. But I said, ed, what is your fantasy? I said, because I know for males, a lot of times there could be a mental block happening. I said, what's your. Like, what is your fantasy, Ed? And he said, kelly, I've never thought about that. And I said, well, Ed, do you watch porn? He was like, no, I don't watch porn. I don't like porn. Like, I just don't do it. I'm like, okay, no problem. But I said, you know, what is it that. That you believe is going to give you an erection? And he's like, I don't know. If I figure out. If I figure out this fantasy thing, I really think this is it. So I got off the telehealth with Ed. I'm like, ed, I need this as your homework. I need you to figure out, what do you fantasize about? And I got back on the telehealth next week with Ed. And Ed said, kelly, that was it. I just needed a visual of my wife. And he went into more detail about it. And I said, that's amazing, Ed, that now we're able to solve that. Because what happened with Ed is it wasn't necessarily a muscular skeletal thing, which we were trying to solve. The muscular Skeletal part. But what it really was with Ed is he was so stressed out from work, his to do list was so long that he couldn't take focus mentally off that to do list and the stress. But when he was able to figure out what makes him happy, what. What makes him feel good and what he fantasizes about, then he was actually able to have intimacy with his wife and be able to feel connected with her and get back to having intercourse and not having to worry about the erectile dysfunction. And that right there, he was actually crying on the telehealth to me because he was like, kelly, these questions really changed me and changed the way I was thinking. And I was able to get out of my head because of these questions. And I truly believe if we saw Ed in person, remember, we didn't see Ed at all in person. This is all teletherapy. We saw Ed in person. I don't know if we would have been able to get there with Ed if we would have had that deep of a conversation. And I know some of you guys may not, you know, be comfortable asking these questions, and that's okay, but this is just one example that I'm sharing with you guys of how personal you can get or not get whatever you're comfortable with. Okay. With clients, and then be able to actually share back with you. So you can change people's lives. Because I know some of you guys on here right now think I can't change anyone's life through a computer screen. And I promise you, you really, really can. So how do I do teletherapy? I do it through Jane. For those that know me, I love Jane. It's so easy to switch from. If they're in person on my schedule, then basically all you do is send them a link or you just change it to a televisit and then it sends them a link, which I'll show you in a second, and they just click the button, and all of a sudden, bam. The patient can get on. It's HIPAA compliant. It auto sends them appointment reminders so I don't have to be like, hey, you coming to the appointment today? I already know they're going to come. And then Jane, also, anyone that's on this webinar, if you guys want a free month through Jane, use the code PELVIBIZ1MO and you get a free month with Jane. Okay. PELVIBIZ1MO. Julia can put it in the chat. Okay. If you want a free month. But they're amazing and so I'm going to show you what it looks like. So because I know some of you guys are like, Kelly, how do you do this? Like, what do you use? So again, Jane, is HIPAA compliant, guys, completely safe. It will be like this right here. It says thank you for booking when you say that. It's a teletherapy session. They're going to get something that looks like this. All they have to do is hit this button, begin your online appointment, hit this thing, there's a consent, and then the patient's just waiting there until you hit admit the patient in. And then they come right on your screen and ta da, they're there. Super, super easy. So it doesn't have to be complicated whatsoever. Alright, so another thing is, what devices do I use? You've got to make sure you tell patients that they cannot be using a desktop. I find a desktop to not be helpful at all because I can't move around their environment. So I use an iPhone or, you know, just a normal phone or a laptop that you can adjust the screen up or down that's important so that you can have lots of. And they need lots of room around them so that they can do certain kind of movements or use their house. All right, so here's the next thing. Converting in person leads to virtual. Most of us struggle with this. Who struggles with it? Say me in the chat. Okay, you're struggling with this. Let me know because I'm talking to you. Let me see. All right, Megan said me, Sydney. Yes. Okay, so yeah, I know we struggle with this too. Okay, so here's the thing. You must communicate this prior to this event even trying to happen. So if you guys right today are like, hey, hi. Orthopelic physical therapy, this is Kelly. Hi, I'm sick today. Oh, we want to just switch to a virtual session. They're like, what's a virtual session or virtual therapy? Nah, I really like her when she works on me and I just want to lie there. No, I don't want to do that. Okay, well then I'll see you next week. Click a. You now just lost your consistency, which means your results do what? They go down. Consistency is key. We've seen it in the research time and time again. I've seen it in 15 years of practice. Okay, that's number one. Number two, you now are going to have to get that patient to get kind of caught up and kind of buy. I don't want to say buy back in, but like get them to be back on track. We're losing progress. And now you've lost a visit. So how you do this is number one, you have to have a method and you've got to tell them that on the initial evaluation, this is what we say. At least two of these sessions will be teletherapy because we like to see consistency and we know with consistency there is progress. I think recently there was two research articles that came out. One was on stress, urinary incontinence, a 12 week program. And if they were consistent, don't quote me 100% on the stats on this, but it was like a 75 to 95%. They were actually going to get the results if they were consistent each week. The ones that weren't is like, I don't know, a 15% to a 20%. It significantly dropped. And then for the prolapse protocol, it was 16 weeks of being consistent. So we have seen it in the research. I've also seen it in practice. Consistency is actually key. So that is why when they go, bye bye. It's the same thing when you work out. If I had a trainer, a personal trainer, I'm like, hey, I'll see you next week. I look forward to that. I'm gonna be consistent and stay on it. But if I'm like, you know what? I think I'm gonna eat ice cream with my girlfriends today. And I'm just gonna skip today and I'll see him next week. And I'm like, hey, I'll see you next week. Next week rolls around. Haven't seen him in two weeks. The likelihood of me going to see him again, I'm gonna be like, you know what? I think I just need to take a nap today. Let me just go lie down. And now three weeks go by and then I'm like, I guess I don't need him because I haven't seen him in three weeks. See how this goes. So then now we're not getting the results and now they think that they failed therapy. They didn't fail therapy. It just wasn't communicated properly prior to anything. Any objection that they may have given you? Okay, so we must communicate this on initial evaluation and, or even on your website or even on your social media. Guys, who's promoting, I want to know this, this is interesting. Who's promoting teletherapy on their social media? Anyone? You'll see it. I said orthopelvic. We'll put a computer and, you know, we'll put a therapist there doing whatever and we'll promote teletherapy. So if you guys Aren't doing that. I highly recommend you doing that. Yeah, a lot of us aren't doing it. So promote it so that people know it and that way you can convert them. Okay. And get consistency so you get the results. And again, so there's less chance of having an objection and that they are on board for it and they actually complete their full plan of care. So a lot of you guys are saying, well, Kelly, how do you fill up a 45 minute to an hour treatment session? Well, here's what I do. Number one, I guide them through movements and exercises. Okay. And in a minute I'm going to take you guys through some exercises. So get ready, just FYI. Okay. So I guide them through movement, through the screen. I help them unit using medical devices, guys. Lots of medical devices. I use intimate rose all the time. Amanda, again, thank you for all this. But I use intimate rose products all the time to show my patients what they can do. I build rapport with the patient because I'm actually asking them questions that are open ended and not just like, yep, yes, okay. Like I'm actually asking them questions that I can get more information from them. I give them personalized insight on their progress. So here's where I also think a lot of us miss personal trainers do this. Well, do you ever tell your patients, especially if they're on like the third, fourth, fifth, sixth, seventh visit, do you ever tell them like, hey, you're really progressing and like this is you're doing an amazing job. Like you couldn't do this, but now you're doing this. I think some of us miss the boat. Maybe not everyone, but I think some of us miss the boat on that a little bit. And so when you keep consistency and you're seeing them weekly, you can then see the progress that they're making. And then also this is the biggest thing is educate them on lifestyle modifications. Lifestyle modifications, not exercise modifications. Lifestyle. And the last thing here is don't think that teletherapy is entirely different from in person. It's simply a more accessible format that you can actually help more people in this world. So here's the biggest thing. We all must improve our coaching skills. I say coaching skills, not our manual skills, our coaching skills. So this is why I really believe the best thing I can teach you guys, and I talk about this all the time, is how to communicate better with people and not how to manual therapy them better. So you have to coach them effectively through communication. You got to ask the right questions. This is very important. So hopefully you guys Are taking notes. The right questions lead to the understanding of their actual true why of why they're coming to see you. So I know when a client is coming to see me and they say, kelly, I'm here because of leakage. And I'm like, oh, really? You're here because of leakage? Tell me more about that leakage. Well, I'm here because I have leakage and I want to be able to like run out the door and play basketball with my kids. Oh, okay. So tell me more. Why is that important to you? Well, it's important to me because like, I hate like trying to put on three pads before I have to run out the door and it takes time and I can't catch up with my kids and then I feel old. Oh my gosh. Yes, I completely get that. I get you just want to be able to go out the door and play with your kids and go jump on the trampoline and, you know, go play basketball. Yeah. So tell me, why is that important for you? Well, because, Kelly, I don't want to feel old. I don't want to feel that I have to have this like, oh my God, I can't go play with my kids because now I got to think about where's my pads at and all that. I don't want anything to stop me. And it's really, really important for me to be able to hang out with my kids when I want to hang out with them. And so that's the deeper why guys? That's the deeper why. Not that they have leakage. So if you're not asking the right questions and able to communicate effectively, it's going to be hard for you to do. Coaching people. So this is what I'm going to coach you guys on is how to ask these questions. So here's three important questions I want you guys to write down. Number one, what does your day look like? This is super, super important. So I'm like, okay, Sally, actually show me. So I'm going to take my computer screen here. Okay. Take my computer screen, Sally. Show me. Okay, so you go and you're going to go to sleep, right? Okay, so take me to your room. Okay. Yeah, let me put you right here. Let me actually put you here. I don't know, I didn't even test this out ahead of time. So here we go. Okay, so okay, I'm sleeping. I'm in my bed. Okay, now how do you get out of bed, Sally? Well, I do this. Okay, Sally. Alright, so right there we're gonna make that correction. So then I literally take Sally through how to get out of her bed, and we're using her environment to be able to effectively do that every single day. So now she has no doubt, because she's not in a staged environment. And now she confidently knows that she can do it, because you know why I'm there with her. I'm in her bedroom with her, and she's like, oh, my God, she just saw me do the thing. So now I know I'm good to go. I can get out of this bed this way. I'm giving the clients confidence, so it will take me anywhere around their house, which is amazing. Okay, here's my second question. I like to ask, what would you be able to do if you were. If we were able to solve this problem? So this is me digging. What are you able to do if I was able to solve this problem for you? And here's what they're probably going to say, Kelly. I could go hiking on a hike with my husband and not have urgency and. And not have to be worried that I have to stop every 30 minutes because I got to go pee, and I can't do that right now. And my husband loves hiking, and I can't connect with him. That's a deep why not? I have urgency, and I just got. I got urge. I got to go. When I see a waterfall, that's not a deeper why. Here's the third question. How would you feel if we were able to solve this issue? And I say the word feel because I want to know how it makes them feel, and that's going to help me be able to get them to the finish line. Because we feel if we feel something, we feel that we can do it, then more than likely it's going to be, we're going to get it done. So I had another patient, guys. She was a rugby player. She's really cool, by the way. She's a rugby player, okay? We saw her for 36 sessions in my clinic. She had leakage. We solved that problem. She came back, she fell off a motorcycle. She tore her Achilles. And I. We were working on her Achilles right here because she was playing rugby and she got sick, and the office moved her to a teletherapy session. And I. And I became like, you guys. I said, oh, my God, what am I gonna do with her? I've been really just working at this Achilles, like, what am I gonna do with her? I kind of had a moment of. I kind of freaked out because I was like, wait, what am I gonna do. I've just been really working that Achilles so that we can get back to playing rugby. But here's what I learned. This was really interesting, even myself, but I've been doing this for years. You guys may have these moments, okay? I was like, wait, what else can I do with her besides these things? And I started thinking, I'm like, you know what? Let me test some stuff that I haven't tested in a while. So I got a scorecard out that I created, and a scorecard for those who don't know. A scorecard is how I test people to understand where they're at. So it kind of gives me an idea. And I have them do certain movements to get them to see, like, where they're at, but it also allows the patient to see where they're at. I don't have to sell the patient on anything. I don't sell anyone on anything. I allow the patient to understand where they're at based on me taking them through certain movements so that they can understand what they're lacking or not lacking. Hopefully that makes sense. Okay. So I took her through the scorecard, and she was really weak in her abdominal region. And I was like, oh, I can actually do tons of abdominal stuff with her. We haven't even done that yet, because I was so focused on her Achilles, trying to make her run better and do all the things, but actually her abdominals is causing her hip pain because she was also having hip pain, and I just need to strengthen that. And so the entire teletherapy session, we did abs, and she was so thankful after the fact. She was so thankful. And it was because I had that scorecard that allowed me to get to get her to understand. Crap. I need to really work on this. Oh, and by the way, she was a pole dancer, too, so she's really fun. Okay. She's a pole dancer, and she needed to use her abs a lot too. And so this was amazing. She's like, kelly, thank you so much. Like, we have. We haven't gone over this. This is awesome. And she was so thankful. But if I didn't do a teletherapy session with her, then I wasn't working her abs because I was so focused on using these to get that Achilles to lengthen, which is crazy. Now, here's my general format that I use. Okay, so get ready to move in a second, by the way. Okay, so, number one, I've got to get to the deeper why. Number two, I have them test out and see where their problem is. Like, I just said talked about the scorecard. I use the scorecard to truly see where they're at. So what I mean by that is I have the patient squat. Okay. I have the squat. I don't coach them. By the way, here's Lulu. Okay, everyone say hi to Lulu. Hi, Lulu. Okay, so I have them spot and see. And I'm just observing. I don't coach them. I just see what's happening. What's happening with their knees? Are they coming in? Are they going out? Can they get all the way down? Is their booty doing this? Are they popping out their booty like this? What's going on? And then I just ask them a question. Sally, do you notice that you can't, like, get any lower? And they're like, yeah, that's crazy. I can't even get any lower. I didn't even realize that. The scorecard points out that they got a problem. Number three, I provide hope. I educate them. I say, Sally, this is what's happening. This is what's going on your hip joint. This is what's going on your pelvic floor. This is what's happening. But this is what we can do about that and show them that I got a method that's going to allow them to be able to do the thing. And last thing I do is I give them one tip, not five. One tip. This is also where most people go wrong. Please just give them one thing. When they leave, don't give them five. You overwhelm them again. They don't have time. So if you give them one really good thing, they're going to do it consistently. For instance, tell me how to get up out of my bed better. That's going to be consistent. And I'm going to be able to do that every single day. And I'm going to feel real good about myself. Okay, everyone, I want you to. Hopefully you're on your computer, a laptop or cell phone or something like that. Okay, I'm going to show you guys ways that you're going to diagnose a hypertonic pelvic floor without using your hands. So here we go. Follow me to my floor. Okay, so number one, let me put it here. Okay, I'm here. Angle is down. You're going to come here. I'm going to turn and face like this. Okay? So act like you guys are my patients, by the way. So I'm like, okay, patience. What I want you to do is lie on your side. You're going to take your finger like this, and I want you just to place it on your butt hole. Don't go in it. Just place it on your butthole. Okay? So from here, what we're going to do is I want you now just to breathe. They're going to inhale and exhale, inhale and exhale. And what I'm going to say is, okay, patient, is your finger moving? And if it's not moving, more than likely we got a hypertonic pelvic floor. That's one way that you're going to help diagnose if someone has a hypertonic pelvic floor. Number two, I like to take a mirror, okay? Like so let me show you. I'm gonna put you guys, I'm gonna put you here. Okay? So now patient, if you have a mirror that looks like this, like a makeup mirror that you guys put your makeup on with, you're gonna place that on the floor. So they're now gonna place it on the floor. Okay? Now I'm gonna angle this mirror like this and I'm going to stand over top of it and I'm going to have the patient. Okay, Not. And by the way, I'm not. You're not watching me pull down my pants, okay? This is not that kind of show, okay? Just FYI. But what they're going to do is after the fact, I'm coaching them through on the thing, but they're doing this after the fact. They're not doing this live with me on the video. Let me just make that clear. Okay? So I'm coaching them on how to do it, and they're going to do it with their pants on while I'm on the. While I'm on Jane with them. Ok? So I'm coaching them. I'm standing up over the mirror and I'm here and I'm spreading my legs apart and I'm coughing, sneezing, laughing, and I'm going to see if anything's coming down and if I can see anything, that's how I'm going to diagnose if there is a prolapse. And the patient is going to tell me, okay? And number three, I have the patient sneeze, cough, or laugh. So what that looks like is. Let me pull you up here. So I'm like, all right, patients, let's go ahead. And I want you to lift up your shirt for me and you guys can do this with me, okay? I want you to sneeze for me. Achoo. Achoo. Return to the side. Achoo. Achoo. I'm watching what's happening with their abdominal region. Okay, I'm seeing if it's going in, is it going down, what's happening? Then I have them cough. Okay, Patient cough. Okay, now laugh. Ha ha, ha, ha, ha, ha, ha, ha, ha, ha. And I'm just watching. I'm not coaching yet. I'm just watching because I want to see where is the pressure going. And then from there I can help make corrections on what's happening. And you can see I'm angling here to the side. I'm angling forward. You can change up the angle even more if you wanted to, but it allows you to see what's going on. Like they were, you know, in front of me in person. Okay. Equipment that you may need. And we do have a storefront, guys, if you want the link, let us know. But we do have a storefront that's linked to all this. So I like to use a makeup mirror so you can see. We have. I like to use a mobile desk because watch. You can go down and you can go up very easily. Okay? So I like a desk that can raise and lower. I like some kind of model that you can stick your finger in so patients truly understand. I love the wand by Intimate Rose or the dilators so that we can coach people on what to do and they can really see what's happening. Okay. Or Even just the 3D model. So last thing here, okay, I had another patient. I'm just gonna call him Ed number two. Okay. And this guy had a hyperactive penis. And the fact that he would get an erection if he just saw someone. Okay. And you guys probably think I see a lot. I mean, we see a lot of males, by the way, just FYI. Okay, so this guy had the opposite problem of the first Ed. His. He was very sensitive to anyone coming around him. Different shorts that he was wearing, all the different things. Okay. And with him, we saw him for like 20 something sessions in person. And then we had to do a teletherapy at his home. And so he was having. He would get an erection every single time. He would sit up in a straight chair. Well, the reason why his muscles would activate and contract and it would cause him to have an erection. He would also get an erection anytime he was around a female. And it just that he felt kind of slightly uncomfortable with. And then he would accidentally get an erection. And so it was really embarrassing. It was really life limiting. But what we were able to do is I could see five different chairs around his house that he could sit in, ones that he wasn't having an erection in one that he was having an erection in. And just because I saw the chairs, I was able to coach him through how to not have an erection in a chair that was actually upright. But if I didn't see what chairs that he was sitting in and he kind of just tried to describe them to me at my clinic, I wouldn't have known how to actually solve his problem unless I physically actually saw it. Okay, so that's a great example of, again, why teletherapy is super important. So, you know why I really wanted to do this is to get you guys to be able to truly treat patients and get the results that you want. So I know, and I've created something for you guys that I know with 100% certainty that it's going to get you the results. So here it is tonight, and for 24 hours, by the way, I'm offering this. I'm giving you guys my three teletherapy diagnosis charts, and it's a full plan of care. So like Natalie was saying, she stops at six visits. No, this is a full plan of care for 12 weeks. And what to do each week, I'm giving you guys my pain with intercourse, one of teletherapy, my leakage, one of teletherapy, and hip pain for teletherapy. All of this is $2,997. However, we're adding bonuses for 24 hours for those people that want it. Okay? So if you want to be so certain that you can do this, because even if you just had one patient cancel because they can't come into your clinic, this is going to save you. Okay? And here's what it is. We're going to give you teletherapy checklist for patients, what they need to feel confident. Teletherapy, we're going to give you checklists for providers on what you guys need to feel confident. What do you need to actually effectively communicate with patients? We're also going to give you the full replay. Now, I'm also going to give you my scorecard on how I actually am able to get patients to see that they have a problem. Again, I'm not selling anything. I want people to understand by the scorecard that they have this issue, and this is why they have the issue. I don't really have to point much out. So if you guys are like, I cannot communicate well, the scorecard then is going to be your best friend for those people that are out of state or like, wanting to move to a different state. But they're in a different state. Or if you just want a really effective online marketing plan, this is massive right here, this out of state for patients.
