In this solo presentation, Kelly Alhooie explains how clinicians—physical therapists, chiropractors, and occupational therapists—can transition from traditional in‑person practice to a profitable online healthcare business in 2026. She argues that success online hinges on becoming a "massive digital marketer" and shifting from selling static courses to offering implementation‑focused services that deliver measurable results remotely. Alhooie outlines the three viable practice models (online‑only, hybrid, and in‑person) and stresses that each requires distinct skill sets, especially the ability to generate results without hands‑on contact. She discusses how clinicians can build brand awareness and community regardless of geographic location, even for mobile professionals such as military spouses. Finally, she contrasts two pathways for remote service delivery: influencer‑style virtual coaching, which operates across state lines with minimal regulatory burden, and regulated teletherapy, which must comply with HIPAA, documentation, and state licensure requirements. Throughout, she provides practical guidance on marketing, community building, and navigating regulatory constraints to create a sustainable, location‑independent online practice.

All right, today I'm gonna talk about how to build an online business in the healthcare space in 2026. So if you were thinking about going online, this is what we help people with to be able to actually get patients, not just to sell a course, but to actually have a business that thrives, that you can make money, you can be anywhere you want, and you can also get patients the results. So if you are a physical therapist or chiropractor or occupational therapist, this video is for you today. So here we go. Let's get into it. Number 1, if you want to be online, you have to think of yourself as a massive digital marketer, period. You have to be the best digital marketer to be known online. I've seen it over and over again. People just assume that they're going to create this course because you guys are amazing clinicians and you believe that because I'm so amazing, I've gotten people results, that immediately people are going to buy the stuff. And that does not happen. And so I am seeing this, uh, switch from courses to actually implementation and certainty, moving more into the online space, which I think needs to be had for more consumers to actually want to do, um, online. And by the way, I should say this, I have people in my program currently that strictly just do online businesses and they work. I have people that do hybrid, they work. I have people that do just in person, they work. Every model works. It's just that you're going to have to have different skills for different models. And so if you want to be online, you have to be the best at marketing, and you have to be good at, um, getting the results online. And I think most clinicians feel not confident in getting someone results because in the schools just taught us hands, hands, hands, hands, and they did not teach us online. So not only do you have to learn a new skill of making sure that you're getting the result without touching the patient. But you also have to learn how to get your brand and awareness and your name out there and build community all online. And by the way, if you do online, that doesn't mean you also can't do in person. Meaning, especially if you like move— you're a military spouse and you move every year and a half— well, guess what? You got a year to freaking build, even if you're in a place for like 3 months. Guys, when I would come out here to California and I wasn't here full time, it's like, cool, I got an opportunity to build a community out here, you know, in person, because I can like meet people. Like, that's cool. And by the way, you can build a community— you don't have to be in person, you can be anywhere. I also coach people that live here in California and are building a business in Hawaii. I coach people that lived in Virginia and then also are building a business in Miami, Florida. So you don't have to be in the location to build the business, if that makes sense. Um, and so I'm going to break down— I have it on my whiteboard here— I'm going to break down what needs to be had that you have to do if you want to do teletherapy or you want to do virtual coaching. Coaching you can do all across state lines. You're basically doing what influencers do. They help, they help people and inspire people to get the results that they are looking for. Cross state lines, there's no HIPAA, there's no notes, nothing. Highly recommend doing that. Or you can be under the strict rules of teletherapy where there's HIPAA, you have to do notes, um, and it just, you know, is teletherapy.
