Provider to Coach, Why I Changed My Title/Identity
18 Oct 2025 · 13 min
Kelly Alhooie explains why she moved from the provider role into coaching: treating patients one-on-one felt rewarding but constrained her time and business growth. She realized many client scenarios share the same underlying solution and systematized that approach into her signature LSR method (Lengthen, Strengthen, Return). This led her to favor group coaching over individual sessions—group formats scale better, build community (a key patient need), preserve standards through systemization, reduce provider burnout, and address common practical problems (time, documentation, low reimbursement). She cites pricing examples (patients charged $250/hour; she charged $1,000/hour for one-on-one consulting but stopped selling it) and offers her personal phone number for questions.

Why I gave up my provider title to become a coach.
So when I was a provider, I realized I.
Love treating patients at first. And then I became, as the business was growing, I became more and more frustrated with being stuck in a 8 by 10 room with the patient because a, I realized I had so many other things I could be doing to run the business, like to keep it afloat. I needed to support my employees, but I was busy treating. I needed to, you know, get back to patients over the phone, email, whatever. But I again was in the treatment room and I started to realize this actually pretty early on, that I didn't like to be stuck in a room with a patient. However, what I did like about it is I could connect like one on one with this patient. I can com. I can really get deep with them. I felt like every single patient was my best girlfriend or my best guy friend. Right. And I just absolutely loved connecting with people and then also seeing the outcomes that we got were just amazing. When you have so many stories of changing people's lives, it that that is the best part. That is what actually keeps you going and being like, yeah, I'll stay stuck in a 8 by 10 room for another eight hours today. Oh, and then go home and do notes at night. Yeah, I'll do that. It's because of those stories. It truly is. And so from the provider heart inside, it's like, I love that because when you get those wins, that's every single day like what you're like, yes, I'm so grateful I'm going to keep doing this like forever, because this is awesome. But then at the same time, like I said, as the business starts to grow, you just start to see things from a different light and you start to really audit your time. And for me, I was like, wow, I am treating this patient for one hour. We were charging 250.
And at the same time, I was also growing pelvibiz. And for those that don't know, I charge $1,000 an hour for, for consulting and one on one. And by the way, I don't even sell that because I don't again want to do that because to me it's the equivalent of being stuck in a treatment room for an hour period of time. And I don't believe people need that even though they think that they want that same thing for patients. And so I started to make this correlation. Patients believe, some agents believe that they only can see a doctor one on one. And that's how it is. And I don't believe that that's actually true. Just like some people in the coaching space believe that they have to see someone one on one to get the results. And I don't believe that that's accurate either. I can tell you. You know, being on I used to do, when I started Pelvibiz, I used to do over 200 coaching calls on Zoom for an hour. Back to back to back to back to back. I couldn't even think straight by the time I was done. And I recognized and realized while doing that and is, oh my God. The person just had a little bit different scenario. But guess what? The solution that I gave was actually the same thing. And then I get on the next call, the solution I gave was actually the same thing. The solution I gave was actually. And then I started to realize the solution is the same thing, even though the scenarios are slightly different.
And it. I was like, wow. And then I was like, so why am I doing these one on ones? I should be doing this group. Because when you have someone to lean on and support you, it's like so much freaking better. And then I started to think about.
And this is only after the fact, this is only after I sold my practice, when I actually had time to ponder and not just be in the weeds all day long. I really started to ponder and be like, how do I incorporate coaching, which I am doing at Pelby Biz, into provider land? And then this is when it dawned on me. I was like, ultimately, we have a method. We have a method for.
Physical therapy. We have a method. I've created my own method. It's called the LSR method. It's my signature method. It is my method that I've created the length and strengthening return phase. So then that had me thinking, okay, lengthen strength and return phase. Well, there is a signature of what I actually do with people. Now some of y' all right now are going to be thinking, why does she do the same stuff with the same people? Don't they have different problems? Yes, they do have different problems. But guess what? The thing that you're going to do and whether or not you realize this or not, because you actually do the same thing too. You're going to give them the same solution whether or not they come in with this problem, this problem, or this problem. You just don't realize that it yet because you haven't systematized it yet. Everything I do, I put a system to it because when there's a system to it, you're able to reproduce it and keep the standards High. That's why at my brick and mortar, we had high standards. No matter who I brought in, it could have been a new grad or it could have been myself that has been practicing for 15 years. I know, and I am comfortable with new grad trading because they. The standards are still at the level because of the method that we all follow. But if someone were to come in and they're like, hey, I don't do any of those Pilates exercise you guys do, which has happened before, okay? And oh, by the way, I'm not going to do that. Okay, well, that's not part of our method. And so you are either going to learn it or you're just. That's not part of this culture. Okay? And so. And yeah, they can throw in their own things of like, oh, my God, well, I took this TRX course. Okay, yeah, let's throw that in there. No problem. So open to that, right? And it, by the way, I don't make. I'm not making every therapist have to treat the exact same. But what I am creating is a standard so that anyone that comes to the practice, this is the standard. And these are the results. Because the standard is the actual result.
Quick interruption. For those that have questions on today's episode, please text my personal number. It's 703-783-2388. I'd be happy to answer any questions that you have to get you unstuck. Let's get back to the episode. It actually gives you guidelines to go off of, and then you can throw in some cherries on top, some, you know, TRX movements and whatever else. But at least we know the basics are getting hit so that the outcome that the patient wants does actually happen.
And that is called a method. And so what I realized is because I had a method.
Then I was like, well, I'm doing the same method for chronic pelvic pain person, a leakage person, a, um.
What. What else? A pregnancy person. I'm doing the same method. It doesn't mean I'm not. I'm doing the same thing, guys. It's a. I'm not doing the same thing. I'm doing the same method. I'm not doing the same treatment. It's different. Okay? So I started to realize, well, if I'm doing the same method, then everyone follows the method. So then if everyone follows the method, then why don't I group people together? Ding, ding, ding, ding, ding. That's when I was like, ah, group coaching. Those words came to my head. Group coaching, okay. Well, let me put them together.
And this is then when I started to do the math and started do the problem solving and started doing the time audit and all that stuff, I was like, oh my gosh, all the stars freaking align. Like, this is freaking amazing. Like, this is amazing.
And now I just need you all to do it because I figured it out. I feel like I broke the code of changing healthcare and I'm on a mission to freaking change healthcare for real. I really am. Because I feel like we. Everyone complains about time, everyone complains about notes, everyone complains about low pay, less reimbursement. So. So why don't we change it? Why don't we change it? And group coaching is the way it's different. They have answers now. Patients have answers now in their pocket from chatgpt and perplexity. They can literally solve complex problems through an app. Why the heck would they want to come see you? Why? Because they want certainty. And guess what else they want.
Community.
Can you provide community when you have one on one in your box or one on one online?
I mean, you could maybe online through like a Facebook group, right? But you're not getting community while you're in the session. They don't have anyone else to talk to and relate to. So they again, feel that they are the only one that have that has this problem. Everyone always thinks they're the only one that has this problem. And the reality is we know that's not the case.
Because I've treated 10 plus thousands of patients. Okay. I could tell you the method has worked over and over and I very, very, very high success rate. And I'm not anyone special. It's just because I've implemented the method. And when you implement the method, then you get the results of what the patient is wanting. And when the patient gets what they want, the patient is a very satisfied customer, is very happy.
And the method does not just consist of exercises, lengthening, strengthening. No, the method consists of putting all the pieces of the puzzle. When it talks about stress, diet, nutrition, supplements, partners, relationships, activities they've been doing throughout the day, what their work is like, what their kids are like, all of that matters. It all matters. And that is part of the method.
So I think where some resistance comes with some of this stuff of changing from actually being a provider to a coach is a. People are fearful. Y' all are fearful. And I'm not blaming you. I get it too, because you're like, oh my God, am I going to the state lines? I have to cross them. Do I Need to get a compact license. No, you don't need any of that anymore. You don't. You really don't. Patients don't care what degree you have. They are literally asking an app chat bot how to solve their problem. Do you really think they care what letters you have behind your name? They. They don't. They're asking a bot that's not even a human, hey, can you help solve my problem? They are just looking for someone that they can connect with, that they have seen has proof of concept. The chats don't even have proof of concept. They don't. They don't have chatgpt, doesn't have a testimonial box of, hey, someone that asked me this pudendal neuralgia question. Um, I'm going to give them the testimonial of how I. The chat has helped this person. They don't have that. We as humans actually do. But you have to have a method to show people, this is the path that I'm going to take you down so that you. This is the result that you're going to get. And, oh, and by the way, this is what this person wrote about how we do things here.
And so. So that is huge. Like, so don't be afraid to go outside the boundary. Besides, what they taught in school, it's just different. And I get it when, when things are different, people get very weary of that. And if you're very weary, then get a lawyer and just ask them a question. It's probably gonna cost you, like, 500 bucks, but then that would help you get over it and be like, okay, I could do this, you know, or ask me. I'd be happy to help. You can always text me at 703-783-2388. Be happy to answer any questions from you guys.
That being said, you. You have to. You got to do it differently. You just got to do it differently. Go help people in. In, I don't know, in Alaska or whatever. Like, go help people there. They probably need our help.
Go help people. That's what we're like. That's why we got into this profession to begin with.
And so I'm, I'm just kind of asking you guys to get uncomfortable and do things differently than how we were trained so that you can get the outcome that you're looking for and that the patient gets the outcome and everyone is happy, excited, not burnt out, and ready to push forward. And so that's all I'm asking today is do something that's slightly uncomfortable and just give it a try. All right, guys, until next time. Bye. Bye. Thanks guys for joining and thank you, Jane, for sponsoring Pelby Biz Podcast. We can't thank you enough so that we can help more providers across the world. Thanks. Thanks so much for listening.
