In this solo YouTube presentation, Kelly Alhooie outlines the seven core systems required to grow a physical healthcare practice to $1 million in revenue and beyond. She begins with the foundational "organizing the business" system—covering legal entity, NPI, EMR, document management, the PelviBiz CMR platform, call logs, and continuous website/SEO audits. She then details the "offers" system (wellness programs, apps, group coaching, tele‑therapy, memberships, packages, and clinical upsells like shockwave and dry needling). The third system is "marketing," emphasizing regular messaging audits, social media themes, ads, SEO blogs, workshops, DM outreach, email campaigns, and AI‑driven search visibility. The fourth system is "sales," describing automated DM workflows, a 12‑step phone script, funnel pages, objection handling, and client retention coaching. The fifth system covers "hiring," from job posting and interviewing to contracts, payroll, onboarding, and expectation setting. The sixth system focuses on "leadership," including hard conversations, team building, communication, and developing staff toward their own goals. The seventh system is "profit," outlining tax payment routines, debt payoff, investment strategies, and overall money management. Kelly recaps that a $1 M practice can be run with roughly five employees (2‑3 therapists, a manager, and the owner) in 2‑3 treatment rooms (~1,500‑2,000 sq ft). She promotes her "Power Circle" program—a ready‑made suite of the described systems for a modest price—and invites viewers to contact her via Instagram @pelvibiz for more information.

The seven systems that you need to grow your brick and mortar to one million dollars and beyond. So here we go. Let's get into it. Number one, you must have systems that organize the actual business. So let's start there. So do you have your entity? Do you have your NPI? Do you have your NPI Type Two? Do you have an EMR system? Okay. So there's going to be systems and and Systems inside of systems here, and each system inside of a system can be very detailed or not, just depends. So do you have your own EMR? Have you set that up? Do you have your own Google Docs? Have you set that up to share with your admin or potentially an employee or just yourself? Do you have PelviBiz Solution, which is a CMR, which is everything that is already built out for you that you need to run a brick-and-mortar? From emails to text to cancellation notifications to reviews, like all of that is one whole system that we have in place that you don't have to build out. Do you have a call log? Do you have, um, you know, do you have a system for your website? How often are you auditing your website? Most of y'all just think, let me put up a website and then it's done. And especially these days, that's not the case. You must be always auditing because the words on your website matter for AIO and SEO, and that's really important to stay in the game so that your competitor doesn't show up more than you do. Number 2, what offers are you actually presenting? So are you doing wellness programs, which I highly recommend anyone in the health and wellness space do wellness programs? Do you have an app potentially that you develop for your clients? Are you doing group coaching? Are you doing teletherapy? Are you doing memberships? Are you doing packages? Are you doing one-on-one? What does that actual look like? So what offers do you have? What upsells and downsells? Do you have shockwave? Do you have dry needling? Like, what are the things that you have that you can enhance someone's outcome? That's huge. The 3rd system, marketing system. This is the one that everyone focuses on and forgets all the other things, but Marketing is just one of the systems. So how often are you auditing your messaging? Every 3 months, guys, I recommend you auditing your messaging because your population changes over time. When I started orthopelvic, I was known for hips, that, and then that was pelvic floor, and then I was known for CrossFit athletes, athletes with leakage, and then I was known for male pelvic floor, and then I was known for perimenopause and menopause ladies. So like the messaging always shifted. It wasn't always just the same, and if you stay the same, you will die. So I would highly recommend not doing that. What's the messaging on social media that you need to put out every— I would say at least month to 2 months there's a new theme coming out on social media. There's always something. So are you keeping track of that? Are you running ads or not running ads? Are you putting out blogs to help with SEO or not? Are you doing workshops? Are you DMing people? Do you even know how to talk to people in the DMs? Are you sending out emails? Are you doing AIO so that when people search for something, it recommends you in the AI search bar? Like, what are you doing for marketing? So that's a whole— marketing is lots of systems inside of that. And do you have those systems in place? Number 4, do you have a sales system? This is huge. So when someone DMs you, do you have an automated workflow for that? When someone calls you, do you have a 12-step processes that someone follows over the phone so that you make sure that you get them as a client? Do you have an upsell or downsell? Do you have sales funnels where it's like pages explaining what your product is or your service is? Can you handle any objection, whether or not that's in person, phone, or online? Can you handle all that? How do you also keep clients in or patients in for the entire plan of care. How do you do that? Do you have a system for that? How do you coach that to someone else? Number 5, there's a hiring system. This one took me a while to get, um, but how to put up a job post, that's a whole system. How to interview, that's a whole system. How to do contract— how to, you know, do a contract, it's a whole system. How to do payroll, system. Onboarding. It's a whole processes of how to onboard, how to set expectations. Always you are doing that, but how to continuously set expectations. All of that is an actual system. Okay. The sixth thing is leadership. How to have hard conversations. They always say a big business is built on the amount of hard conversations that you have. And I think that's very, very accurate. Okay. Team building. Do you know how to lead a team? Do you know how to communicate well? Do you know how to develop people so that you develop them to get there to their goals instead of your goals? And indirectly you'll get to your goals, but like you have to get them to their goals. The— it's about them. It's not about you. The seventh thing on the list is profit. People love to talk about it. People hate to talk about it, but profit—that's the last system. What's the system of you paying your estimated taxes? What's the system of you paying off your debt? What's the system of you investing in the money that you make? Where are you putting that to make you more money? How to manage your money. So many people will be like, "Oh, you got money." So now what do I do with it? Oh my god! And it's like, no, you have to manage the money now. So let me recap here. There's 7 systems that you will need, and there's systems inside the systems, just so you know. Some are larger, some are smaller, that you will need to scale a $1 million practice. It's organizing the business first. That's huge. Offers, marketing, sales, hiring, leadership, profit. Basically how to handle money. That is what you will need to have a successful brick-and-mortar practice. And so if you guys have any questions on that, of how to actually build a— and by the way, I think so many people think or scared of like a big brick-and-mortar, they got to have a building and I have to blah blah blah. Guys, you just have to have basically 4 to 5 employees including yourself, and you can have a million business with 5 employees. Okay, 5 employees, 2.5 therapists, a manager, and you. That's it. That's literally it. Okay, that will literally bring you a million-dollar business and your time back. So like, don't be scared to create. And by the way, you need 2, maybe 3 treatment rooms at that point. Maybe you could probably honestly get away with 2. Okay, and like, that's it. It's not a large space. That's probably— you need like 2,000 square foot of space, maybe even less than that, maybe even 1,500. It's not that big. So like, if you're like, oh my God, I can't build a million-dollar business, like, that just sounds too crazy— it's not. It's really, really not. And this could be all cash clients, all cash. So if you're struggling with that and you want help, this is why we have Power Circle, because it is literally all of our systems that I just named out. So if you're like, this sounds so overwhelming, literally for a very small price, you literally get handed the systems and say, here you go, put your logo in it, there you go, and the systems are there.
