The 90-Day Cash-Based Pivot: What Nobody Tells You Before You Quit Insurance
13 Jun 2026 · 10 min
In this solo coaching podcast, Kelly Alhooie walks clinicians through a concrete 90‑day plan to transition from a salaried insurance position to a cash‑based private practice. She emphasizes starting with proof of concept before any legal or branding investments. The first action is to recruit five initial clients by reaching out to personal contacts (phone, email, text) and Instagram followers, offering a single free session to demonstrate value and convert them into paying patients. She advises operating virtually or mobile from home, using simple payment methods like Venmo or checks, and postponing LLC formation, business banking, and branding until a client base exists. Kelly then explains how to calculate the income needed to replace a $4‑5k monthly salary (roughly 8‑9 patients per week) and recommends reducing hours at the day job as the cash‑based client load grows. She highlights the mental bandwidth cost of juggling two jobs, urging a rapid, aggressive pivot rather than a slow, fear‑driven approach. Throughout, she stresses the advantages of a cash‑based model—instant payment, pricing freedom, and steady cash flow—and invites listeners to contact her via @pelvibiz for personalized coaching.

You want to leave your job and you're just freaking scared? Well, guess what? Today I'm going to show you how to do a 90-day pivot so that you can actually leave your job once and for all and you're not in this limbo of like, I'm kind of in, I'm kind of out, and then all of a sudden you don't have money and now you're living on the streets. Okay, so here's the game plan. I'm going to break it down from start to finish of what you need to do. Number 1, you do not need to go get a business. Name. Last thing you need to do, okay? You got to have proof first, so don't waste your time, effort, money, energy in doing that. Who can you right now, from your actual cell phone, who can you right now text, call, email? Basically, you can do it for all your contacts. Email all these people in your contacts. You guys have a lot of emails, you guys got a lot of phone numbers. Text every single person and say, hey, are you struggling with back pain? Hey, are you struggling with leakage? Hey, are you struggling with whatever? I know this is a random message, but I really just want to start helping people for free and I'm taking on 5 people. That's it. Anyone that responds, guess what? That's your client. Now, what did I just say? I'm treating them for free. So you're not going to get paid necessarily. This is why we still have to have the job. Okay, the 9-to-5 job. But once you treat them one time for free— one, not 20, one— when you treat them one time for free, what I would like to see happen is they say, damn, that was freaking amazing, I gotta have more of this. And in that session, you've got to be able to communicate how you're going to help them from point A to point B and what that pathway looks like. And if you can do that effectively, they will say, here you go, I just became your first client. And you're going to do this out of your house. You're going to do this virtually. You're going to do this mobile. You do not need a space. Okay, if you— as long as you have a computer, or a table, you're good to go. Okay, that's key. And so you're just going to try to get clients, guys. When you have about 5 clients, then I would go actually apply to become a business, because all right now you're doing is just trying to see if a business is going to occur. Okay, I would not go making shirts. I would not go get a business bank account. You can't anyways unless you have an LLC or, you know, a PLLC here in California. Um, so get the clients first. This is the first step that everyone misses because the clients are the thing that's going to pay you, not your LLC. Once you get your LLC, it doesn't just start paying you, okay? You're losing money then if you don't have clients. So the first step is to say, what contacts do I have? Start just using the contacts you have. The next step I do is I go to Instagram. So once I've I've done email, text, and my phone. Okay, then I go to Instagram and I'm like, hmm, how many followers do I have? Great. I'm going to message every single one of those followers the exact same message. Okay, so, hey, I'm just seeing if you have leakage, low back pain. I'm just giving away 5 free spots for free. Are you interested? Like, literally. And people, people have trust issues now. Okay. So you have to say what you want to get out of it. So I would love to treat you for free to help you get out of pain. And in return, I just get to build up my Google reviews if you like my service, or I just get to have more experience underneath my belt or whatever you want to say. But say like what you are trying to get out of it too, so it doesn't feel salesy and then no one responds to it. I go through all that. And just the clients' contacts that you have, guys, do you believe that you can actually get 5 clients? Yes, that's before you do any marketing, any marketing, just by your network. Unless you're a hermit and you live underneath like a little rock, you— I guarantee you have contacts in your phone, guaranteed. Maybe a family member, whatever, whoever it may be. Great. And then if you can film it, not internal exams, but if you could film it, great, even better. You then have you live in action, what it's going to look like to work with you. Fabulous. This is what I did. I did a mock eval on my sister-in-law, went over to her house, set up a video camera so that my clients could see what it was like to work with me. And I just did a mock eval because, A, it was like a practical. I just wanted to see, like, if I could even get through it. And how it was going to go and like, how do I take payment? Well, guess what? How you take payment? It's called Venmo. Everyone has Venmo. Venmo, check, Venmo or check again. And it's just written out to you personally, not, not the business, because you don't have a business at this point if you listen to what I was saying. And that's okay because the accountants will know that when you get your business started, things get a little mixed up. And then long term, I'm not saying do this long term, But what I'm saying is this is how you start to then leave your job. And basically the third step I do then is to say, how much do I need to replace my income? For most of us, it's $4,000 to $5,000. And so that is equivalent, depending on how much you charge, between 8 to 9 patients a week. Not a day, a week. That's so doable. We see that in a freaking day. And so the goal is to consistently get up to that and you drop your hours. And the fourth thing I would say is if you can drop your hours when you have 5 clients to 20 hours a week, highly recommend doing that because that is when you will see a massive return and it will grow your business. Because the problem is not only the time aspect with you spending 40 hours at your job and then trying to grow the side hustle— it is possible, I did it. Okay, um, and/or, you know, it's the mental bandwidth that you have to have because you have to go to this job, they want you to do it this certain way, they want you to look at the charts a certain way, they want you to progress a certain way, and then you have to go to your own business and figure out, well, how are you doing it? And it's going to be different. And, and the mental bandwidth of just going back and forth will be the biggest self-sabotage of you getting your business started in 90 days or less that I've ever seen. It's the number one problem I see. It's the time and it's the mental sabotage that happens between working two different— basically having two different lives, working two different jobs in two different environments. And so people want to like really go slow through this phase. They tell me this all the time, and it confuses the heck out of me of why. I think they're just fearful, but like, they're like, well, I'm not trying to go fast. Like, if it just happens, it happens. And I think they're just telling themselves that, like, because they don't want to disappoint themselves. It's like a joke. It's like really like a joke, because the longer that you're in both jobs, the more torture you are doing to yourself. So I'm just going to be clear here. The more you're torturing yourself and the more that you are just creating more pain— and I don't know why people want to do that— you want to try to move as fast as possible out of that job that you, you know, don't like, or what more— you want more for yourself. So you want to try to get away from that job, and you, you know, do you and your business as fast as you possibly can. I don't know why you would want to move slow. I really don't know why. I hear it all the time. People tell me this all the time, and I believe it's just fear because they don't know. And so they believe that slow is better because it's not going to be overwhelming, it's not going to help, like, take away from their health. And I'm like, no, it's actually going to murder your health because mentally you're going to break down and physically you break down because you're so freaking exhausted. And so it's the opposite of what people think. And so you really want to try to do everything possible, follow up with the leads, follow up with your friends, follow up with whoever is saying yes to you as fast as possible, because that's the thing that's going to move you out of the job. Don't make it a long process. It makes no sense. So that's what you got to do is you got to quickly pivot into cash, and cash is the only way to go. Like, it really is. Cash-based model, you can— your hands are not tied behind your back. You can charge what you're worth. You get paid right there on the spot, so your books are not messed up for 6 weeks because you're waiting on potentially getting paid or not. You know you can stay in business because you got cash flow coming in. Like, there is, there is no negative. There's literally no negative. So if you're scared and you're like, I don't know how to do this, like, I really want— if you know you're meant for more, you know, you're like, damn, like I need to do this, but I'm freaking scared. Reach out to me, message me @pelvibiz, P-E-L-V-I-B-I-Z, Pelvi Biz. Message me, say, Kelly, I'm scared to make this jump. Can you create the path specifically for me? I'd be happy to do that.
