Why Teletherapy Is the Missing Piece in Your Practice
20 Nov 2025 · 15 min
Host Kelly Alhooie (Pelvibiz podcast) advocates for integrating teletherapy into pelvic/rehabilitation practices. She argues teletherapy is growing, can be as or more effective than in-person care for functional coaching, and can expand reach and revenue via one-on-one virtual sessions, memberships, assessments, and group coaching. Practical advice includes camera positioning, doing home-based functional assessments (e.g., postpartum setup), typical pricing models (session rates, memberships, assessment fees), and an upcoming free webinar to train clinicians on teletherapy.

What's up, guys? Welcome to the Pelvibiz podcast. I'm your host, Kelly Alhui. Today I want to talk about teletherapy. I had someone in one of my CAT programs ask me of, hey, I really want to be able to treat this person. She's two hours away. How do I actually treat her? And I really want to go over the tactile version of this. But I first just want to start out with a story of why teletherapy is important. Teletherapy is important because you are still going to get the patient the results by staying consistent point blank. That is the number one thing of why every single person, whether or not they're online or brick and mortar, this goes to both avenues here. You must do teletherapy, and it is growing in popularity. Just so you guys know, there is more teletherapy coming out. More people are being accepted of it. There are companies right now that, you know, I've spoken to a lot of you guys, and there's companies right now that basically are just contracting therapists. So they do all the marketing. And then they basically say, hey, Steph, I have this, you know, patient in Illinois. Are you a therapist in Illinois? And do you want this patient? You just kind of say, yes. It's almost like an Uber. You know how you booked it and you're like, yep, I'm available, and great. Then that person becomes your client. Better Health is also doing this. I don't know if you guys have heard of Better Health. It's like for psychotherapy. And basically they. You just say your state, you say, you know what your problem is, and they match you with the therapist that has those credentials and is able to handle that. And they're doing it at a very, you know, reasonable price, anywhere between 80 to $150 a session. And they're basically doing membership. So what they're saying is, okay for. Let's just say it's $100 a session for $400 a month, you're going to get to speak to your therapist once a week, and we'll just continue until we, you know, you cancel it. And so that's how they're doing this, and that's how they're positioning this online. So if you guys are not adding this in, you are missing the boat. Okay? AI is here. AI is really going to be everyone's, you know, medical Dr. ChatGPT will be. It's not Dr. Google anymore. It's Dr. Chat BT GBT. And it will be in everyone's pocket. So I need every single one of you guys that are listening to this to really jump on this bandwagon. And if you are uncomfortable with it, if you're confused by it, we've been doing it since the pandemic. It has worked brilliantly. It's actually we find more effective than in person because you become a coach and you have to learn actually how to ask the right questions instead of just using your hands. So and by doing that you actually get more information out of the patient that maybe they didn't disclose in person. So it really becomes such an effective tool not only to save the patient time, but for you to get the information that you need to then accurately guide them for what they want and what they need and what they're looking for and the results that they're looking for. So just to kind of, you know, piggyback off that, teletherapy is going to be the way, and I believe at least probably 50% of clinic owners will be doing it and I know all online business owners will be doing that. Now that being said, that's just in a one on one setting. That's not gearing like you to the person, right? That's not gearing towards group settings which then steers to coaching. That's more of the realm of coaching because you can't technically do like a diagnosis per group. So that would gear more towards coaching. But this will also exercise yalls coaching skills when you're able to do teletherapy, which only will make you more confident, which then will be able to get you guys to do more online things. Because that's the way it's going. And you can not only just serve your community, but you can serve the fricking world. And it just for those that want to create more of an impact, it is going to be your go to thing is going to be teletherapy. So why not start now? Like what are we all waiting on? Why not start now if you're not confident? We have everything to make you confident. So if you need that, just DM us@pelvibiz on Instagram, we would be happy to help. Just say teletherapy and we'd be happy to get you more information on what you need and specifically what that looks like for you. Because everyone needs something slightly different. That being said, pricing teletherapy again, you could do a membership, okay. Or you can do it the same price as if you have in person sessions. Please do it the same price time wise we Always do it for 45 minutes. So that way if you need to write something up afterwards or give instructions afterwards, you can, you have 15 minutes before the next patient or whatever that you can actually do that and it not eat into your time. That's huge. The gist of telehealth that I don't think people realize, I think we over complicate it, is you basically have to take the computer around their home or their iPhone and be able to position it in a way that we can see exactly what they're doing. So if they're a postpartum mom, I want to see how they're breast, when they're breastfeeding, how they're sitting. I want to see how they're carrying their baby. I want to see how they're hinging with their baby. I want to see how they're bending over to pick up the toys. I want to see how they're carrying backpacks, their diaper bag. I want to see how overall they are putting the baby into the car. All those things require something that's immovable. So you cannot do this on a desktop. And it also requires time to position the thing so that you're able to see the client, be able to really get a good glimpse of what's happening. So telehealth, you have to think about it, not just of what exercises I'm going to give them. Can't that just be a trainer? Well, it can be, but here's what makes us different than trainers is we are able to assess functional movement in their homes which allow them to not have the pain that they were having. Because the reason why they have that pain in the first place is because a movement that they have done and that they have created throughout their, you know, years of doing whatever they're doing, which then has caused them the dysfunction. So if you just correct the way that they're doing certain movements like hinging and unloading the dishwasher, all these things, if you just correct it, the pain and the issue goes away. People don't even realize this. I don't know if you guys even realize this. You hinge like 300 times a day to sit, to unload the dishwasher, to put something away, to brush your teeth, know which hinging you're doing. That's why you have back pain, because you don't know how to hinge correctly. Second, you know how to hinge correctly. There you go, you're better. I was, you know, also, you know, talking about telehealth, where we could do just straight up assessments. So for $500, okay, $500, you could do a. When they're, when someone's pregnant, you could preference someone to say, hey, let's do a postpartum setup assessment. And you're like, what's a postpartum setup assessment? Well, what that could be is you over zoom, you zoom into their house and you look at where's everything situated? Most postpartum moms, again, we have to think about this. Most postpartum moms get this, especially first time moms, they get this cute dresser that has the changing table on top of the dresser. We all know what we're talking about from Pottery Barn, right? And the freaking kids room is on the third level of the home. I don't know about you, but a postpartum mom is not going to keep walking up and down the stairs. Well, they shouldn't up and down the freaking stairs to have to go change the baby. Right? So the reality is where can then you put maybe a changing station on the main floor? Are there diapers there? Are there wipes there? Is there certain powders that you need there? Are there extra clothing that's there in the kitchen? Do you have it set up to where you have stations that can easily wash the bottles if you have to bottle feed? Where's your pumping set? Where is that going to go? Do you have stations that are not on the third floor, but maybe on the second floor that you can also pump comfortably? Is there ways of how you're going to feed your baby? Is that set up? Again, not on the third floor, but maybe on the second floor, Maybe you're on the first floor. Depending again on how their house is set up, people want this. They want to know, especially first time moms, they want to know that when they get home that they are set up for the most success for their baby to be able to feed. The number one complaint that I hear in postpartum moms, not even complaint number one issue is that they're most worried about is can I feed my child? That is their number one concern. And if you can do an assessment, and again, you don't have to be a lactation consultant. I mean, if you're a lactation consultant, great. But if you could do an assessment beforehand, that makes it the most optimal way of them having successful with different things around their home and feeling confident that they can do this, that is only going to make their postpartum journey experience so much better. And that's going to take away some of that fear. And anxiety that everyone feels in the postpartum phase and maybe even assess like they're sleeping. Maybe you assess where the baby's gonna be lying. Maybe you assess and you help them create some kind of night routine if they have a spouse or not. Like all these things are part of maybe the $400 assessment that you're going to do virtually over a zoom or over, you know, a Jane. All those things make a big difference and they want that. Or maybe you're going to work with them on week one through three and you have a specific protocol that you're going to do and they buy maybe a group coaching thing from you. That's another way you could hit it. So, again, things to think about that I want you guys to start thinking about, because this is the way it's going. I know I've said this, so I'm blue in the face. I'm telling you, though, technology is here whether or not we want to like it or not. I absolutely used to hate it. Now I have to just work with it. I've learned I have to work with it. We weren't taught it, okay? But we have to run with it. And now is the time to get on this so that you don't fall behind. Because if you're not paying attention right now, look, I'll do all the research for you guys because I do a lot of research myself. So I'm just making these podcasts to make you guys aware of what's. What's upcoming. Just so you know, what is upcoming? If you have a brick and mortar practice, half your clientele will want to do virtual. It will just happen. I know it's going to happen. I'm predicting it right now. Okay? Half your clientele is going to be using ChatGPT to solve their problem. And there's even something, it's called open evidence. Open evidence AI is where they can literally type in any of their medical problems and it tells them a complete solution of what they need to do. I still believe there will be room for in person, okay? Because people want the hands on. I get it. However, I believe that there will be, there will have to be a reduction for in person. And this is why I'm going heavy on teletherapy, because I believe it truly is the new wave of medicine and how it's going to be operating. And people want to be seen right away. They don't want to wait. They want it at a cheaper price point. And that is the way it's going. And so we have to have offers that actually give people what they want at the price point that they want and be able to help serve many people. And so we don't need to be panicking about this, but that just means that you all are going to have to see more people and. Or do it in a group setting and. Or have a product that then serves them so that you don't have to be one on one with them. And if people want more of your time, then they pay for it. It's an upgrade. And they pay to get you one on one to specifically solve their, you know, exact problem. And so I just need y' all to get on the bandwagon and get really confident and comfortable with this so that you don't say crap, you don't wake up, literally. And this is moving so quickly. I think within the next year this is like really going to happen because that's how fast it's moving. And I don't need you to be left behind. I need you to be on the freaking bandwagon. So I wanted to come on today and talk about teletherapy. What's, you know, why is it important and what does that look like? And I do know I am going to put on a free webinar. I want to say I'm putting on a free webinar July 20th. July, no, sorry, July 17th at 7:30. And I will be doing a free webinar on this to get you guys, put it on your calendars. Okay. To get you guys very confident with teletherapy because I need you guys to be dialed in. So if you have questions, please just DM me on Instagram Elvibiz. Okay? Elvibiz. P E L V I B I Z at Pelvibiz and say teletherapy and just ask me your questions there. I'd be happy to answer them. So hopefully you guys learned something today about teletherapy or where I believe things are going and you know, what you should charge and what it's looking like because companies are already doing it. And I wanted to bring this to the forefront. So. All right, guys, talk soon. Bye. Bye.
