In this video, Kelly Alhooie outlines the critical internal systems that healthcare providers need to implement to improve patient retention and build a scalable, sellable practice. She begins by asking whether patients are completing their full plan of care and suggests concrete methods—reminders, clear goal setting, and standardized provider protocols—to ensure completion. She stresses the importance of consistent service standards across all clinicians to reinforce brand reliability. Alhooie then moves to internal marketing, describing how providers should position packages and persuade patients to commit to comprehensive care plans, framing these offers as legitimate, results‑driven solutions. Next, she highlights the necessity of robust backend follow‑up systems that continue to engage prospects and non‑paying clients over years, turning them into future revenue streams and adding value to the practice. Finally, she argues that without investing in automation, hiring specialized staff, or building systematic processes, providers will burn out and be outcompeted by larger teams and technology. She notes that only a small fraction of therapists she surveyed understand the importance of such investment, expressing concern for the profession’s future.

Now, when we talk about internal systems, this is what I look at and see, okay, first off, are all the patients completing their plan of care fully? Are they? And if they're not, then what can I implement into the system that actually allows them to complete their full plan of care? Meaning, what are things— can I remind them of their goals? Can I set more expectations and better clarity, right? Do I have a specific method internally that all my providers go by, or, and/or myself, so that no matter who comes in the door, it's the same service that I am providing every single one of my patients, and it's not like off or different, or so that the, the standard is clear? Many of you guys don't have that, and that's what you need to create to make sure that everything is consistent across the board, and then you grow your brand. That's key. Um, the next thing is, how are we talking to customers? Uh, how are— and that's all internal, right? Like, let's just say for a package, if you guys sell packages, whether or not you sell packages or not, um, how are you getting them to commit to a full plan of care and getting them the actual result? That's all internal marketing. How are you marketing to them saying, hey, this is what you need, like legitimate, you know? It's not made up, but legitimate, saying, hey, this is what I really believe you need. Are you comfortable with that? You know, um, and the next thing I would say is, for either yourself or admins, whether you have an admin or not, do you have systems on the back end? This is again internal marketing, where anyone that doesn't book with you, are you actually following up with them for years on end? Following up, providing value to them, helping them out, even if they aren't paying clients. Are you actually doing that? And if you do not have these systems yet, this is key to have to continuously grow your business. Because some of you guys, you don't know if you're gonna sell it in a year. I mean, I never thought I was gonna sell, but I was gonna have my company forever. You may not know that, but if you want to create an asset, you have to have systems in place so that you can continue to continuously build without getting burnt out. If you do not have systems in place, you will croak trying to keep up with everyone that does have systems in place that you're like, I just can't compete. I just can't compete at all because a human, one human cannot outcompete, you know, 12 other different humans plus bots plus agents, all things. It just doesn't happen. And in today's world, it just is not it. So if you're not on the forefront of this, then you need to hire someone that is. Like, we have this all built out, okay, that allows you to be more proficient and allows you to do things without it taking as much time, effort, energy, so that you can focus your attention on truly what matters, which is getting the patients the results. And it's not going to take you 2, 3, 4 years to get there. It'll take you, you know, 1 year because you invested in these things that'll— that have shortened that gap. And I think that's, that's key. I don't believe a lot of people, you know, really understand that. Like, guys, I talked to about 30 of you guys just recently, and I was so shocked, so shocked how many providers— I would say 2 out of the 30 honestly understood investment. 2 out of the 30 therapists that I talked to. And it honestly made me so freaking sad for our profession because I feel like they just sold us on a lie.
