In this episode of the Truth, Lies and Alzheimer’s Show, host Ken Poglia introduces dementia expert Lisa Skinner, who shares two personal case stories to illustrate less‑known behavioral symptoms of Alzheimer’s disease and how to recognize and manage them. The first story, “Birds in the Mattress,” recounts Mrs. Walker’s vivid delusions, hallucinations, and paranoid beliefs about birds, rats, and intruders, highlighting how people with dementia can create elaborate false realities and repeatedly call police. Lisa explains that these experiences are common manifestations of dementia—hallucinations, delusions, and paranoia—and that reasoning alone cannot dispel them; instead, caregivers should acknowledge concerns and gently redirect attention, with medication reserved for severe psychosis. The second story, “Sundowning,” follows Jack, whose late‑day confusion, agitation, and elopement attempts culminate in a dangerous wandering episode that finally convinces his family to place him in a memory‑care facility, where he thrives. Lisa describes sundowning as a pattern of disorientation, anxiety, aggression, and wandering that often occurs in the evening, and distinguishes it from wandering and elopement. She emphasizes that these behaviors are disease‑driven, not intentional, and can be mitigated with non‑pharmacologic strategies, environmental adjustments, and appropriate care placement. Throughout, Lisa stresses the importance of early recognition of these “hidden” symptoms, empathetic caregiver responses, and informed decisions about assisted‑living versus memory‑care environments.
