Matt Kelly explains how carnitine converts to TMAO based on your gut bacteria. A balanced microbiome produces little TMAO, but an imbalanced one creates more. Berberine, an herbal extract, may block this conversion.

But it turns out if you read most of the research about TMAO and carnitine, that it's not just that the carnitine automatically does that. It's when the carnitine goes into a digestive tract, cuz you're taking these pills orally, goes into a digestive tract that has an imbalance of good flora. So the microbiome is off. You have a much higher level of carnitine creating TMAO formation. If we have really good balance of microbial flora in the GI tract, we don't make that, much TMAO or none. And there was some research that came out a few years ago that showed that if you take the herbal extract berberine, which we see used a lot for some infections and diabetes and all sorts of other stuff, but if you take berberine, it actually shuts down the formation of TMAO from carnitine.
