A clinician explains why you must believe patients who say they can't tolerate a drug, and why starving them into compliance isn't the answer. When you have time, diet works. When plaque is serious, tell them.

If a patient comes back and says, I can't tolerate this drug or that drug, you pretty much have to believe them. You can't force a drug down there. They won't take it. You fool yourself. So I don't know what you tell somebody like that, but I would back off on a diet to malnourish them. Especially if I KNEW I had 10 years to play. If she had coronary plaque to that degree, I'd let her know.
