In Korea, if I don't wear makeup, I often get comments like this: You look tired.

Do you need to go to the hospital? You look sick.

But in the US, I would never say this because it can sound a bit rude in English. If I thought someone looked sick or tired and I was genuinely concerned, this is what I would say instead: Is everything okay? Or more casually: You're good? Yeah, why?
