Podcast monologue by Dr. Charles Mok explaining non-pharmacologic endoscopic alternatives for weight loss. He describes the Orbera gastric balloon (endoscopic placement, saline-filled, ~6-month duration, typical weight loss 20–50 lb, reversible) and an endoscopic gastric sleeve (internal stapling, less invasive than open surgery, same-day discharge, typical weight loss ~20% body weight, removable). He contrasts these options with GLP-1/GIP drugs (semaglutide, tirzepatide) and open bariatric surgery as alternatives for patients who cannot or do not want medication or open surgery.

Foreign. Questions we get is what can I do to lose weight in the era of the drugs such as Wegovy and Ozempic, the semaglutide drugs, or Tirzepatibes such as Mangernal and Zeppelin? What can be done if I don't want to use these medications? And diet and exercise is thus far failing? And I don't want to go through bariatric surgery on my open surgical procedure. So there are some options that we offer. One is called a gastric balloon. And the one we've been using for about seven years is the Orbera gastric balloon. This is a balloon that goes in endoscopically, meaning it goes, you get sedated and they put a scope down your mouth into your stomach. And a balloon gets inserted with the scope and inflated with saline, and the fill tube is removed. And the balloon is designed to stay in for about six months and make it feel full so your stomach's got a certain size. And when the balloon takes about half that size, creates fullness. And the person tends to lose on average, 20 to 50 pounds of weight is typically lost. But we've seen it up to 100 pounds of weight loss. It's designed to go for six months. And that six months baboon is taken out in the same fashion with the endoscope. Now, the downside there balloon is that it's not permanent. However, it gives you that edge of changing your lifestyle where you would start getting used to ordering smaller portions, being full sooner. And it has a potential to change your microbiome, which leads to less craving of excessive eating. There's another procedure called an endoscopic endoscopic gastric sleeve. And this is a endoscopic procedure, and it's closer to a surgery. They actually use staples to make the stomach smaller, so the stomach is folded in and stapled. So it's a gastric sleeve procedure done internally through the mouth. But it doesn't have the risk. It has less risk than traditional surgery because we're not opening you up, although you are still sedated for it. And this is a procedure that can be done and you're back home the same day. There's no recovery time at all. And people typically lose more weight, about 20% of their body weight, as loss of the gastric sleeve. Now, this is essentially more of a permanent procedure, but it is removable. So the gastric sleeve sleeve staples can be removed if there's a problem. We haven't really seen that be needed much, but that was designed to be a reversible procedure. These are services people can do if they're struggling with weight loss and traditional means haven't worked and they don't want to do bad bariatric surgery and they do not want to use medications Perhaps they've had problems with the medications or they just made too expensive. The surgery in the long term tends to be cheaper less expensive and some people have side effects from the medications which they can't use them is the generally very well tolerated some people can't tolerate them. Dr. Medications so the gastric balloon fills the stomach up with a balloon there's less space so you feel satiety or fullness sooner so you eat less food and the weight comes off. The gastric sleeve pinches the stomach smaller. It's internal endoscopic gastric sleeve it makes the stomach smaller. It doesn't have as much capacity and these are less invasive than traditional surgical procedures and alternatives to weight loss medications when diet and exercise are not not cutting it for you. So thank. You.
