Doctor Helps Man with Erectile Dysfunction LIVE

In this live‑streamed medical session, Dr. Charles Pereyra, M.D. treats a 32‑year‑old patient, Brian, for erectile dysfunction using a multi‑step regenerative protocol. The procedure begins with a topical numbing cream specially formulated to penetrate penile skin, allowing the patient to remain fully conscious and conversational. After confirming adequate anesthesia, an ultrasound Doppler exam performed by fellowship‑trained Dr. Nieto documents baseline penile blood flow. The core therapeutic intervention combines platelet‑rich plasma (PRP) with a mesenchymal stem‑cell (MSC) exosome solution, delivering roughly 90‑110 billion exosomes per vial. The mixture is injected into the penile corona and surrounding tissue while the patient reports minimal pressure‑type sensations and no pain. Throughout, Dr. Pereyra emphasizes safety (Betadine sterilization, sterile technique), patient comfort, and the experimental nature of the treatment. The session concludes with a discussion of expected benefits (improved vascular health, regenerative effects) and a scheduled three‑month follow‑up to reassess outcomes.

Oh my God, yeah, now it— this is crazy. I don't feel, I don't feel anything, dude. Seriously, this is good stuff.
3, 2, 1, and nice little poke. So let us know how you feel.
Not bad at all.
It's actually going right in right now. Can you feel it going in?
I do.
Does it hurt at all?
Not at all.
Feeling good.
All right, I'm, uh, I'm going to go get dressed now.
Hey, Dr. Charles here, and I'm incredibly excited to bring the research-type started to begin during my time at Cornell University, all through medical school and residency, and hopefully now to bring it to a patient with a successful result. Brian is about to embark on the first stage of this journey, which involves taking platelet-rich plasma, or PRP, to concentrate growth factors. After this step, we'll introduce our MSC exosome solution and combine it with this platelet-rich plasma solution, not only to add a scaffolding and viscosity, but to increase the volume so that the entire male organ can receive the beneficial effects.
That one vial has 100 billion? 110 billion. 110 billion. Ooh, 110. I can feel it.
It is incredibly difficult to determine the total amount of exosomes in any solution. That's why in our lab we only utilize facilities which have multiple safety checks as well as quality checks to determine the approximate number to the closest possible number of these small exosomes. So an exosome, for context, is about 1/1000th the size of a cell, which means it is extremely small. And you need multiple modalities to even determine if you have an exosome in your solution. And based on what we're seeing, there's approximately anywhere from 90 all the way up to 110 billion exosomes in just one vial of our stem cell solution. Again, I know this is experimental, but a lot of patients do seek to go and look for these kinds of wellness treatments, including men's health and even total body. And one of the things that Brian did here today was look to do an IV wellness treatment. Which is a very high dose, uh, of our MSC exosome formula designed to help naturally boost your own body's regenerative capabilities. This is something I've personally done and I really enjoyed, and it's something Brian is experimenting with for the first time. I've seen in my personal benefits VO2 max change, I've seen my HRV change, and I've also noticed a qualitative change in my endurance when performing activities.
And so I'm super excited to see Brian's results Yeah, I'm super excited to see what kind of benefits I'll see physically. I've been working out and, you know, trying to work on my fitness as much as I can. So I feel like having this as an extra boost is definitely gonna be a big benefit. So super excited to see the results.
The next step is gonna be applying the numbing solution. So we'll all step outta the room. You actually get to apply that yourself. Oh, great. So there's no awkwardness with anyone in the room. I don't have to do it for you.
It's not gonna be filmed?
No, we don't have to film it. It's okay. You know, we charge extra if you want us to film it. That's it.
Oh, great.
So we'll get that set up. Brittany will show them how to do it. This is the solution right here. So this is a very unique numbing solution that took a lot of research to get something that could actually penetrate the skin to the level where you have enough numbness where you really don't feel the sharpness of the needle going in. And it's just more of a pressure sensation, if that. And so it's a very unique solution in itself. It has 3 different factors in it. Some of them have actually 4 different factors in it. And it's able to be applied by the patient and they can do it themselves in the privacy of the room and it really absorbs into the skin. And afterwards, wipe it off, and we really have minimal interaction with this. It's all about the patient, and we teach the patient exactly how to do this for the most effective result.
So I'm now on the phase of applying the local anesthetic. I already did that. I'm not gonna show you guys, but, uh, yeah, you can see it's been about 10 minutes. I was told to wait about 20 minutes, so I was told to just massage it in for a little bit, so I'm gonna continue doing that for a little bit. But it seems like it's working because I don't I'm starting to lose sensation in my— Dress? I've got my little gown on. So yeah, I was just thinking we could like test it with staples. Let's do it. Oh my God, dude, this is crazy. I don't feel anything, dude. Seriously? Yeah. Like I— Dude, it's so weird, dude. It's so weird. It's an impressive product. Yeah, no, it definitely works for sure. But Like I'm touching it down there, I don't feel anything. Yeah, well, that's what you get, man. Industry-leading in everything that we do.
Yes, sir. Incredible.
Yeah. So I kind of want to just get it done right now. I think I'm ready. I think it's good.
I'm ready to go.
Let's get the doctors in.
Let me crack on.
And that was a pretty, pretty crazy experience because I've never had my penis numbed before like that, which is of course, you know, how it should be because we're going to be injecting a needle down there. So I knew that it was working instantly because I was applying the cream and then Over time, over like the course of about 10 minutes, I started to lose all feeling. I still— it's still pretty numb right now because the effects are still residual after that. Yeah, that's when I knew that it was obviously working. So that's when I felt confident to let the doctors know that, that I'm ready to get started. So I don't feel anything. I think I'm ready. Yeah, this numbing cream is good stuff.
Powerful, right?
It's really powerful.
I've been numb for 6 hours.
Great.
All right. So you're feeling very numb?
Yes. Very numb down there.
The next step is we're going to have our ultrasound fellowship-trained specialist who's here, Dr. Nieto, and he's going to do an ultrasound of the penis, which is a qualitative assessment of the flow inside, making sure you have good blood flow. Then we'll take a video of it. We can compare it from now until after. Just a qualitative visual approach.
Cool. Yeah. Awesome.
All right. So we're going to sit down and get started.
So this is the Doppler ultrasound, right? Okay. So even though it's not erect, it'll still get a good gauge of—
It's still a blood flow.
Yeah.
I mean, if it had no blood flow, you would just die. Like you need some blood flow.
Right, right, right.
Right. But to like truly, truly assess, you would need an erection.
But to induce an erection, you would have to inject you with something. I see. Or arouse you, which is weird.
So one of the qualitative measures that we use is an ultrasound Doppler. And really all that does is assess the penile blood flow via ultrasound. We had our specialist, our fellowship-trained doctor, Dr. Nieto, here to help us and guide us through the entire flow. And we were able to actually see the arterial flow of the penis in real time, which is really cool. That's something you can take a look at qualitatively, and you can also measure the velocity to get a rough measurement of how good the blood flow is at that given time, and then compare it if the patient wants to come back in 3 to 6 months. All right, that's it. Let's go to work.
Does this hurt?
No, no, you must be very numb. Very numb.
This is, uh, very strange that I don't feel anything.
So I'll let you kind of shift in here. We can do this.
Overall, everything looks good.
Yeah, everything looks great, man. You got a healthy male organ.
Awesome.
You're doing great. You're— how old are you?
32.
32?
Yeah.
So with our patient Brian today, super courageous to be filmed. And one of the really cool things about having Brian here is we got to see his facial expression during the procedure, right after being numbed. And he was actually able to hold a full conversation, very minimal pain, uh, really just a pressure or a slight pinching sensation for him versus that pain you would expect from having a needle in such a sensitive part. And the cool thing about that is that numbing medication lasts for some time, meaning even after You go home, you're still a little bit numb, and then by later on in the evening, the medication starts to wear off and you don't really notice sensation that much at all. What we're doing is we're having Dr. Nieto now clean the entire male organ with a very strong cleaning solution called Betadine. This makes sure that the area is sterile. Obviously, when you're injecting any instrumentation around any organ or any breaking the skin surface, you want to make sure it's as clean as possible and sterile as possible. So we actually do two passes with the Betadine solution. This has a little bit of a color tint to it. Doesn't harm the pigmentation of the skin, but definitely helps with the sterilization process. We're going to do it. We'll count down from 3. We go, you know, 3, 2, 1, and nice little poke. So let us know how you feel.
Not bad at all.
It's actually going right in right now. Can you feel it going in?
I do.
Does it hurt at all?
Not at all.
All right. So it's happening in real time. And the solution is really something we enjoy using for our patients because of this. You can sit here and have a conversation with me.
Yeah.
Without being in pain. And it's a big part to be comfortable during this actual procedure.
Yeah, I definitely felt something, but it was not painful at all. Yeah. Felt like almost like kind of the sensation when you have to urinate. Kind of felt like that. Yeah. Yeah. Interesting. Yeah.
It's really important to us that the patients are comfortable, not just having an effective procedure, but having a safe procedure and a procedure that the patients can actually go home with same day and not have to worry about being in extreme pain. Just a lot of pressure.
Yeah.
Putting a lot of pressure.
That one actually, this one feels less than the last one.
Feels even less.
Yeah.
It's getting better.
The right side hurts a little bit more.
Yeah.
I definitely felt more on that right side. Yeah.
So right now we're injecting into the corona, which is the most sensitive area of the male organ. And how you feeling, Brian?
Yeah, totally fine. Yeah.
Able to have a conversation, which is exactly what we want to be. That is the goal, conversational and void of any pain. 5, 4, 3, 2, 1, done.
All right, not bad. Overall, right now I feel good. Uh, I feel like almost accomplished, although I didn't really do anything besides just walk in the door. But I went through a procedure which was pretty daunting, you know, like obviously no one wants to get needles down there. But, you know, I was so curious about this procedure and the benefits that I could have got from it. So I'm now, I'm just like, I'm glad that The hard part is over and— well, the hard part isn't over, I guess that's to come. So yeah. Yeah.
Yeah. So follow-up is in 3 months.
3 months. Okay. Got it.
