The Immune Signal Most Oncologists Get Wrong

In this presentation Dr. Dino Prato, NMD, founder of Advita Medical Centers, argues that the primary reason many cancers recur after surgery, chemotherapy, or radiation is the neglect of a critical immune signal: damage‑associated molecular patterns (DAMPs). He explains that when cancer cells die they release DAMPs, which act as SOS flares that can either activate a protective immune response or, if released in excess by overly aggressive treatment, drive inflammation, mutation, and resistance. Prato advocates an immune‑centric, precision‑oncology approach that (1) uses micro‑dose, metronomic chemotherapy to modulate DAMP release, (2) maps tumors with a thousand‑plus molecular markers—including DNA/RNA sequencing, transcriptomics, and spatial immunology—to guide therapy, (3) employs adoptive immunotherapy and immune profiling to “teach” the immune system, (4) monitors disease with liquid biopsies (circulating tumor cells, ctDNA, methylation scores) and inflammation markers (CRP, IL‑6, TNF‑α), and (5) supports the immune system through lifestyle changes (sleep, antioxidant‑rich diet, stress reduction) and integrative agents (curcumin, boswellia, omega‑3s, low‑dose naltrexone, fasting). Throughout he provides concrete tips for patients to ask their oncologists about immune activation, DAMP modulation, and advanced monitoring, positioning the immune system as the first and last line of defense against cancer relapse.

Have you ever wondered why after surgery, chemo and radiation, even after doing everything right, cancer can still come back roaring back like a stubborn weed? It won't die. What if the answer isn't another drug or supplement, but it's hidden in the switch inside your body's immune system that decides whether relapse will occur or you'll recover. It's like a two edged sword. It's called dams. Damage associated molecular patterns is something that almost no one is talking about and is completely missed in cancer treatment. And yet we have found that to be so Critically important. I'm Dr. Dino Prado, founder under Advita Medical Centers. For the last several decades, my team and I have helped cancer patients who've failed some of the top hospitals across this country. And today we're going to talk about why treatment works and fails. And it all has to do with immunity. And that's for most cancers. So let's go into this. Here's something most people never hear. Every treatment, whether it's chemo, radiation, or even a natural therapy, it works. But it only works if it has an ultimate effect on your immune system and your immune system's memory. It can kill the cancer. But ultimately it's the immune system that has to do the cleanup the memory and teach the rest of the body about the cancer. So when cancer cells die, they release this danger molecule or signal called dams. Damage associated molecular patterns. These are like the SOS flares that wake up the immune defenses. So number one tip one, as we're going through this, I'm going to give some tips here to help you. Ask your oncologist or your integrative doctor. How's my treatment plan being optimized for immune activation, not just tumor shrinkage. But is there an answer? Are you missing something here or are you just going after a short term response? And I'm not Talking about a PD1 inhibitor which only work on 19% of the patients long term, according in oncology. But I'm talking about a proper immunotherapy that's in place for your treatment as part of your algorithm of care. So before we move on, in my clinical experience, it makes all the difference in the world. In immune centric care, that means your immune system is working. So let's continue. Damps are a switch. Your body has this hidden communication system. When cancer cells die properly, they throw off an immunogenic cell death. They release these damps in harmony. It can be a two edged sword. If there's too much chemo and radiation, which we See in standard oncology all the time. It's basically the standard. It can build resistance and mutation. That means the damps get released so strong that it causes the cancer to grow. This is the key. It's all about having the right targets and the right dosing. If you learn nothing else in this episode, understand this key that it has to be at the right dose. That's why I like microdose metronomic chemotherapy that is targeted for the patient. DNA, RNA transcriptomics, immune spatial biology. Looking at 1000 plus markers and given a microdose because we don't get those side effects and the dams get turned on correctly. But you may ask me what about those patients who responded great to standard of care high dose chemotherapy. They were the fortunate ones that their immune system rebounded. But we don't see this unfortunately with late stage cancers. And that's why I'm talking about it because most late stage cancer patients, they have this problem going on. The treatment is too much, too much mutation resistance and the chemotherapy wipes out the immune system. And then the damps are too strong which is pushing inflammation. So these waves we see like calriculin that are signals that send out to the immune system, go out and eat the cancer. ATP signals come from here. Recruit immune help HM GB1 alert danger calling in the T cells. These are all micro damps that are sent out in the body. That's how your body learns to find and destroy the hidden cancer cells that can't even be picked up by imaging. So lifestyle and therapy choices. So let me give you another little tip here. That can reduce chronic inflammation. So some of the things you got to do is get sleep at night. That's really important. So here I'm talking about the healthy population because I think these are things we can do to decrease inflammation and create a strong immune system. Antioxidant rich diet red reduce all the seed oils, all the garbage food that is refined, carbohydrates and sugars and move to farm to table organic eating healthy foods reduce stress and help your body interpret the signals correctly. Think of this as tuning your immune system and your radio. But when we're talking about precision oncology, we need to follow a five step system which I think has made all the difference for our patients. Number one, we want to map out the cancer with all of the details. The thousand plus markers, not the 24 markers that are seen in standard oncology where they typically use a tumor marker, a biopsy and imaging. So instead we want to look at a thousand plus markers and DNA, ngs, transcriptomics, immunospatial biology. We want to prime the body's immune system to release damps in the right concentrations to stimulate the immune system and not over stimulate inflammation and resistance. This is so important and we can do that with immune profiling because we can see the damps and we can see the switches that are turned on. So using things like genetically targeted fractionated chemotherapy at micro doses, we can get them past the blood brain barrier if we need to. We can send stress as we kill the cancer cells, release these molecules of information, capture it, and now the immune system knows what to do. In fact, we use that same information to grow and expand the immune system against cancer. We're training the cells, the adoptive immunotherapy to go after the cancer and do its work. Block all the inflammation, block all those signals so that we can now see the body heal. Now we can use on label, off label phytotherapeutic nutraceuticals, all combined along with cellular biologics to customize us for each patient. But this continuous is so important. CTC, CT free DNA methylation score so we can see leading indicators before the cancer would ever return. This is why patients aren't getting the correct cancer treatment today. Not only are they not getting the targeting they need, but they're also missing out on adaptive monitoring so they can spot the cancer before it returns again. So here's another tip. Ask your doctor about liquid biopsies so that you can spot circulating tumor cell ct, free DNA and methylation scores. If your doctor does not have that kind of testing, find a new doctor. Immune phenotyping Understand the spatial biology around the tumor and the immune profiling. Look at the methylation panels. These will give you hidden information on how to overcome relapse. So why is the old playbook backfiring? Standard oncology we've seen for years uses high dose. The system provides tremendous stress to the body and it releases uncontrolled damps damage associated molecular patterns. This causes collateral damage of inflammation, DNA repair loops, meaning more mutations, more resistance, and it wipes out your immune system. Even low dose or natural, even integrative clinics are usually missing this. They don't work in the area of dams. This is where the pulse part of treatment is so important. Because when you're doing the chemotherapy, you want to pulse the damps, the damage associated molecular patterns at the right concentrations and in the right formula so that we can help the immune system do its work. This is precision medicine and it works by engineering what your immune system normally does over 10,000 times a day, your healthy immune system will delete cancer cells. So when your immune system is working correctly, we want to keep that going so that the body can get into remission. Because the immune system is the first and last defense against cancer. Next tip. Understand inflammation. So markers like C react active protein IL6TNF alpha. These are resistant pathways in treatment. High levels during or after treatment often means the damps are misfiring. Isn't that interesting? You can run simple markers to find if your damps are misfiring and this isn't being done in oncology. Then you can add simple integrative treatments like curcumin, like Boswellia, like omega 3s and fasting and changing the diet and decreasing inflammation. Low dose naltrexone. A series of things we can do that can shut down inflammation alongside care. If the markers are over elevated and this is important, every patient's map is different and their needs are going to be different so we can control their damps and get the cancer to be killed all at the same time. It's going to make a big difference. Cancer doesn't just respond to treatment, it responds to signals. Those signals can train your body whether it's going to relapse or it's going to have memory for victory. That's the difference. If you can take one thing from this episode is start asking how is my treatment communicating with my immune system? Because when you align your therapies, your nutrition, your treatment, your mindset, everything in the medicine around this question, you can move from fighting cancer to teaching your body to win. I hope this episode was helpful and helped you see the power of the immune system and how what we've been given by God is what we need to heal. We just have to do it in the right way. Give it the right information and your body can do the rest. May the Lord bless you on your journey to healing. Sam.
