How to Make a Form on JotForm

A short screen-share tutorial by Mira Koska (owner/founder of Ohana OT) explaining how to make a form in JotForm. The video demonstrates starting a form from scratch (classic form), adding common elements (name, email, address, phone, date picker, short/long text, dropdowns, single/multiple choice), using the Properties panel (marking fields required, editing multiple questions, Advanced), enabling payments and e-signature, previewing the form, and receiving submissions by email.
Hey everyone, Mira Koska here, owner and founder of Ohana ot. And this is a quick tutorial on how to make a form in Jotform. So I'm going to share my screen here. Okay, so once you're in Jotform, these are just all my forms that I've already made here. You would go to create form to make a new form and I'm just going to click on start from scratch. But there are lots of templates and things you can do and I'm going to go to classic form. Okay. And once you're in this, all you do is you can click on, on here to get all the form elements. And so here I'm just going to write, going to just type in sample and that's my form heading. And then the really nice thing is there's all these elements that you can just click on. So if you want their name, you want their email, you want their address, phone number, if you want to put the date that they filled it out, there's a date picker. And then if you, you have all these different options so you can do a short text. So maybe you're gonna ask some kind of question and then they would just type a short answer. Maybe they're gonna write a lot more so they need more room to write. So then you type in a question and then they fill out that form. We're gonna make a form pretty soon here that's like a permission form. So we'd probably have some, just a little bit of the demographics on there and and then we'd have just will paste in all the permission information here and we would use this paragraph 1. You can also what I love and I use this in my evaluation on my OT forms. I love the dropdown. You can have lots of different options for choices. I like the single choices. So you type on different things and then they click one choice. You can also do like a multiple choice question. So those it makes it just really fast because I can preset a lot of things that I would normally say and then I could just go click, click, click, click to fill out my forms. So there's lots of different options in here, but basically those are the main ones I use. You can set up to take payments through your forms. The thing I really like here is this E signature form or the part to the form because then they can sign your form and it gets emailed back to you once they fill it out. So that makes it really convenient. I'm going to move this over a little so you can see. So the next thing I want to show you real quick is that for each of these boxes, if you click on the properties right here, you're going to have all of these. I just kind of moved it over because my. I guess I can move my face somewhere else. Let's put me over here. Okay. That way you can see. Okay. So you click on that little Properties button and then you have different options. But one of the ones I use often is this, whether it's required. So yes, the name would be required. You get this little star. So you know that it's a required. So you cannot submit the form unless you've filled out the required items. So I would just click on that for everything that is required. It also gives you like a little bit easier way for dealing with having your oops, let me go back to Properties where you can type in multiple questions right here. It's a little easier way to fill it out. That's pretty much it. And then once you're done, I mean, there's more to this. You can go to Advanced and that gives you some other ways of filling in the choices and everything. But this I'm sticking to kind of like the basics here today. So then what you're going to do is click on preview the form and then that's you're going to get to. I'm going to move my face over here now. Okay, so this, my computer's kind of set to pre fill this. But normally you'd be typing it in, but this is what it looks like when you fill it out. So like I could click on there and I could pick today's date and this would actually have some question in it and I could type in the answer. I could type in, type more because there's a bigger one here. This would have multiple choices normally. And then you would just click on it here. You would just. You just pick the one that you want. Pick the one that you want. I'm just scribbling here. And then submit. And here's the cool thing. It's going to show that the form submitted. Excuse me. And then it's gonna. It'll say thank you. And you don't want to click. Sometimes it takes a minute. Don't click on anything extra. Just give it a second. And then it shows up in your mail. Let me show you what it looks like. And there it is all filled out. Isn't it cool? So that is Jotform in a nutshell. And then this is the link. So if you want to send somebody the form, you just highlight that and you send them the link, and then they can fill out the form. So that is pretty much it. Okay. All right. Have a great day. And that is the end of this tutorial.
