Dr. Robert Whitfield explains when he combines fat transfer with breast implant explant surgery in one operation versus staging them separately. He does both together if tissue is healthy with good blood supply and enough viable fat. He recommends explant first, then fat transfer later, if there's inflammation, capsule problems, or skin issues.

Can you do a fat transfer at the same time as your explant? Sometimes yes and sometimes no. Here's how I decide. Simultaneous fat transfers at the same time as explant works when the tissue is healthy, the blood supply is good, and there's enough viable fat to harvest. The advantage is one surgery, one recovery. But if the tissue is inflamed, if there's problems with the capsule, if there's skin issues, staging it is a safer call. Do the explant first, let the tissues heal, and come back later for a fat transfer. The decision is based on what I see clinically, on the quality of the tissues, of course, and the patient's overall health, toxicity burden, and other factors. This Thursday, August 13th, 6 PM Central, I'm going live with 3 patient scenarios, including this one, on Instagram, YouTube, TikTok, LinkedIn, Facebook X, and then privately, obviously, on Dr. Rob's Circle after the fact.
