Dr. Robert Whitfield, a board‑certified plastic surgeon in Austin, TX, explains causes and treatment options for dimpled cellulite—especially on the buttocks—and introduces QWO, an FDA‑tested injectable collagenase treatment for mild-to-moderate buttock cellulite. He contrasts noninvasive (limited effect) versus minimally invasive approaches (Celfina, subcision with fat grafting) and outlines a clinical workflow: in‑person consult, evaluation of skin laxity, marking/photos, a three‑treatment QWO series, and post‑care including ice/compression and adjunct IPL.

This is Dr. Robert Whitfield. I'm a board certified plastic surgeon in Austin, Texas. I'm the owner of Aloes Skin and body by Dr. Whitfield, where we achieve natural results through art, science and innovation. Today I want to speak to you about cellulite. Cellulite is a very difficult problem for plastic surgeons, providers of skin tightening and aesthetics in general. One very complicated factor about this is the differences in areas of the body. So about nine out of 10 women suffer from cellulite somewhere and on the buttocks. Dimpled cellulite is usually a result of fibrous bands. These bands are made up of type 1 and type 3 collagen. And it once again it's mostly women who get the cellulite there. It starts with a surge of estrogen in puberty. It affects all skin types and the number one area again is the buttocks. What are the treatment options for cellulite? Well, when people come into my office, if it was very specific dimpled cellulite, we would have some options for them. Non invasive treatments in my mind have not provided enough of a effect. Minimally invasive treatments like Sulfina and subscision with fat grafting have. And now QWO is simply an injectable that can provide a result. So here is an example of dimpled cellulite marked on the buttock. And I have used Celfina successfully and it uses a reciprocating blade suction tumescent solution. It's kind of more of a process to get a result, but you can get a result with this. And now we have qwo. What's qwo? If you look in the upper left hand corner of this slide, it says collagenase. So what the heck is a collagenase? Well, collagenase has been around forever. It's produced by this bacteria, Clostridium histolyticum. And if you look further at this Wikipedia example, there's Xiflex, Xypex and Alcoa. Xyflex has been around for a long period of time to treat dupuytrens contractures in the hand. So an FDA trial was done with QWO to show that for treatment of moderate cellulite with little to no laxity, we can get results. So instead of the finding Nemo and Dory telling us to just keep swimming, we now have a treatment we can treat. Mild to moderate dimpled cellulite of the buttocks on label. Expect to have a good result. Skin laxity has to be evaluated at the time and recommendations will be made if there's too much laxity. What's a treatment entail? Well it's an in person consultation with me to see if you're a candidate. Dimpled cellulite of the buttocks will be evaluated, not other areas. At this point we'll also evaluate for skin laxity. A treatment plan will be developed to include three treatments for dimpled cellulite of the buttocks. Separate recommendations will be made for skin laxity using products like bodytight, morpheus 8 and fractura. Photos of course will be taken. Areas will be marked. We will discuss these and have you sign off on these photos for treatment. And then after treatment we're expecting you to use Ice, compression, Arnica, Bromelain and other options to decrease bruising and we'll include complementary IPL treatment.
