Dr. Robert Whitfield describes six questions he asks before planning breast explant surgery: how much tissue remains, where the nipple sits, how much skin is loose, what scars the patient will accept, if there's enough fat to graft, and whether to do facial work at the same time or later.

Before I recommend a plan after explant, I ask 6 questions. How much breast tissue remains? Where is the nipple areolar complex position? How much skin is truly excessive? What scars will the patient be able to accept? Is there enough healthy donor fat? And should any facial rejuvenation potentially be done in the office or in the OR or later? The plan follows the analysis and informed consent.
