Thursday Night Live about the NOCUT

Dr. Robert Whitfield explains his no-cut approach to facial rejuvenation for patients experiencing rapid volume loss from GLP-1 drugs like Ozempic and Wegovy. He combines tiny sub-chin incisions, microcoring, radiofrequency microneedling, and fat transfer to restore fullness without visible scars, with recovery measured in days rather than weeks.

Hey everybody, just hopping on here. Gotta get all the setups going, so just give us a moment. Just double-check everything, make sure it's fine. Yep. All right, all the streaming equipment is working. Just wanna double-check, how's my audio? Just make sure everybody can hear me okay. Hey, Danielle, just let me know that you can hear me okay. And we have a— Hello. Talk to give about the No Cut. I know it's busy. It's a World Cup night, as is every night seemingly going forward. Many of you know I'm a huge fan. We went to watch the US men's national team last Friday in Los Angeles at SoFi Stadium. Amazing experience. Got to take my family. Definitely one of those moments you you won't forget. So if you can watch or pick up a match here and there, that'd be great. You'll learn something. It's really— we have a very nice setup. Team's great, working hard. So should be good. Sorry, I'm trying to get all my cameras adjusted. And so we have the final of my 3 talks tonight. Just gotta pull it up. And then we'll get going. Everybody's, everybody's just hopping on, so we'll give it a minute. Okay. How's everybody doing on TikTok? I have Artis, Sharon, Geraldine, Gilda, Lori, Linda, Stacy. And on Instagram we have Jennifer, Gab, Shelley, Danielle, of course, Maury, Mallory, Jesse, Maria? Well, I'll share this. We won't start right now. We will— Yeah, just so everybody knows, do not be rude. I will block you and delete you. This is an educational talk, so I won't tolerate any of that ever. Please share this with your friends. This is going to be about the no-cut face and neck lift. Skin removal, skin tightening without scarring that's visible. We're gonna dive into things regarding Ozempic phase and the use of GLP-1s as well. So, everybody hop on. We have about— gosh, I can't tell how many I have on Instagram. It doesn't really tell me. I have over 250 on TikTok, I guess. I think that's correct. Mm-hmm. Hard to tell. Very hard to tell. Oh, thank you, Danielle. Yeah, everybody can go and join the circle live to watch this as well. Danielle's heading over there now. Afterwards, we'll have a live Q&A in my private community, drrobcircle.com. It's free to join. Danielle and Leah keep telling me eventually it will be unmanageable in there, but right now it's very simple. And pretty uncomplicated, there's no obligations in there. Keep you up to date. I think this week I've been so busy since I got back from the World Cup that it's been a little bit more challenging for me, but nonetheless, it's all good. All good. I am gonna double check something real quick. Quick just to see, um, one thing. Um, is anybody live on, um, is anybody watching on Substack? I can't tell. Bloody hell. Oh, okay, I see. So I don't have a subscriber chat. That's funny. Gee, I don't know if y'all know this, but I go live on Substack as well as the other platforms, and I post a lot there because I don't get censored on Substack. All right, we'll give the talk in a minute. That's just funny. That's too funny. Cracking myself up, I know. It's the way it is right now. Give me a break. All right. I believe we're just about ready to go. And yeah, I can't tell what's going on right there. I'm just trying to double-check to see what's like happening right now. I feel like I'm not seeing everything I thought I would see. But maybe I'm wrong. Trying to go to a couple profiles, just double-checking things for a minute. There we go. Okay, perfect, yes. Just was having a little moment there, I was unsure about where we were streaming. But just bear with me, I'm making sure we're live everywhere. That we're supposed to be. And yes. Yes, yes, yes. Yeah, I don't see it live on Facebook. Is anybody live on Facebook? Usually my manager Jen chimes in here and lets me know if she's seeing me or not. No. All right, I feel like we're having a Facebook moment, which is not something I wanna be bothered by, but all right, if anybody's watching on Facebook, please chime in in the comments. I wanna make sure that it's running on Facebook. Yeah, y'all gotta get more active in the chat. There is no place offering what I offer, so, and nobody has what we have in the office, post-op. So you'll want to stick around. Might be having a Facebook problem and that's why I can't see it. All right, well, I think that's part of the issue. So we're gonna get rolling. Got about a 10-minute talk to give and then we'll go answer a few questions and go live in drrobcircle.com. So it's free to join. So once I get done with the talk, I'll take a question from Instagram. And a question from TikTok, and then we'll shut everything down and go into Dr. Rob's Circle and have a private Q&A. You can ask me anything you want. I'll stay on as long as you need answering questions. Okay? All right, let's get rolling. Well, welcome everybody tonight. Um, it's not something that we talk about or discuss, but in medical, uh, jargon, the Ozempic phase has now become a phenomenon. "And if you've used GLP-1 medications, Ozempic, Wegovy, Mounjaro, Zepbound, first I wanna say something that most of the providers aren't telling you." And what y'all did to do that is a testament to your discipline. Whether you're dealing with a problem with weight loss or you're dealing with a metabolic problem or dealing with inflammation that was creating problems for you from a lifestyle standpoint, or you're having trouble with binge eating, things that you just you couldn't handle on your own, that has given you an avenue to something that's really worked. And that's not a small thing. I actually never thought anything like this would come to the marketplace while I was alive doing medicine because the most difficult thing and looking— you gotta look and listen to this. The most difficult thing to change is someone's behavior. Always. When you lose this weight though, and you lose it rapidly, as GLP-1s can cause weight loss faster than most anything we've seen before outside of weight loss surgery, your body doesn't lose the fat evenly. The face is one of the places that it's most obvious, and facial fat isn't obviously just padding, it's structural, it helps hold your face in a better, vertical position. It's what gives you a more youthful appearance, a more rested appearance, an appearance in your, we'll say, 20s and 30s, which are very much in the rearview mirror for me. I'm 56 now, just so everybody knows. When the fat disappears quickly, the structure underneath really can't compensate or adjust to that. The skin doesn't have time really to tighten gradually as it may do with slow natural aging or slow weight loss. So, what you're left with is more of a hollowed-out appearance. And that appearance usually manifests itself by the cheeks looking more sunken, more— or I think of the depth of the fold being deeper. And the skin looks like it's hanging rather than resting. Many of my patients tell me the same thing, that they've solved some of these metabolic issues with their body, but now they look in the mirror and they feel older than they actually feel because they feel much better. Mm-hmm. So that disconnect is very real and it's not in their heads. So tonight let's walk through kind of how we're addressing this and what we think are some really useful ways to do this that don't leave visible incisions. I, if you know, traditionally try to avoid as many incisions as possible to help with visible scarring. And in fact, I don't do traditional tummy tucks anymore because I really can't stand the scar. I came up with something called the Smart Tummy, and that procedure doesn't leave any visible scar. So I, once again, I like to use a combination approach, usually of tech in this instance, and, um, I don't rely on one option. And the, the reason the no-cut facelift came into existence was a multitude of factors in terms of what is non-surgical or minimally invasive treatments for facial rejuvenation or facial aging. And, you know, we're now faced with volume loss instead of just skin laxity. So now we have a structural problem with volume and we have a skin laxity issue. So now we have a couple different things. Mm-hmm. That are going on. And it's easy if we have an isolated neck problem, which is the first thing that usually becomes lax when you lose weight. 5 to 10 pounds is always gonna cause that. And we do that with the ZOOM Lift by MyElevate. That is a very straightforward, I mean, efficient procedure to do under local or twilight, depending on the patient's preference. Mm-hmm. And it's done through just a very simple small incision under the chin with a series of tiny, almost like needlestick incisions. And there's nothing visible when it's done. So the line of sight doesn't show anything. The jawline is improved. Everything is hugging around the jaw. And that is an easy way to improve or sculpt the jawline. And there's no visible scar and you can't see any of the access points. And so that was easy for necks. So, okay, that's an isolated neck procedure, but now we have, we'll say, some facial laxity. And facial laxity has to be assessed individually. There's no like global one-size-fits-all technique. So we used to combine radiofrequency microneedling and FaceTite, and that would be a nice combination. That would tighten the tissues, and really that combined with the Zoom Lift was what really was the impetus and led to the no-cut facelift. So moving forward, we swapped out microneedling. You can still do that, but here when we add a, a device, uh, from Satellis called Ellacor, it's a microcoring device. So now you can take skin out without a scar. I did this for my patients JJ Virgin, Dave Asprey, and others. And this Ellacor device is a microcoring device What it does is remove small slivers of skin that are less than a half millimeter, and that allows the, the skin to heal and tighten without leaving visible scarring. That's it. It's so small it doesn't leave a scar. And so depending on the individual patient, then we have to assess, are we going to add some volume back with fat transfer, which we can do? Um, we then want to know, all right, are the you know, from a weight standpoint, goal weight, are they, what dose of GLP are they on? Like, what is our, what is our endpoint for that? And that's gonna vary patient by patient. So I would say that is going to be another, you know, discussion point with each and every individual care plan. And so when you put all of those together, that was basically the no-cut facelift. And now we're going to, add volume when necessary via fat transfer for restoration when needed. So, and it's going to be needed more typically in patients as they age because the skin won't tighten as much. As we get older, the collagen and elastin in the skin goes down, it doesn't go up, and that leads to less snap in the skin. So when you lose weight, you don't tighten up as much. So I've called that the no-cut facelift and neck lift, and it was built specifically for the situations where we need this tightening procedure, but we don't want visible scarring. And now we add fat into it based on the treatments people have received with GLP-1s and the loss of volume in the face. Now we have an even more— it's a more powerful procedure. Now the recovery can be much, much shorter than traditional surgery. Patients can be back to normal activity really quickly. And it's the visual, now the face will look almost like sunburn after about a week. The little holes fill in very quickly. And there are lots of different adjuncts you use depending on the patient's preferences. We have natural healing factors. There's always exogenous factors, exosomes. There's all sorts of tools and tricks you can apply topically or you can aerosolize to improve the wound healing efficiency. Okay. So this is a way to solve, I think, probably the biggest problem I've seen, um, just exponentially increase over the past 5 years. And, you know, short of a, a huge uptick in facelifting, which I believe that is, is obviously going to happen as well because I'm giving a non- or minimally invasively executed way to take care of this. So that is and continues to be what I strive for, to find ways to limit visible scarring, whatever we do. And so if everybody's, you know, interested in learning more, we're gonna take a few questions and we're going to Dr. Rob's circle and answer those in a more private fashion there. And so I'll take a question here from TikTok. Let me see. Let me see if there's a question. TikTok doesn't really tell you there's a question, it just tells you that people joined. Hmm. Oh my gosh. All right, he's got a question over here. I'm going to Instagram looking for a question. Okay. Gab, what you should do is go get on a discovery call set up with my clinic and we can talk about that problem. But it's not what I'm talking about tonight, okay? Does anyone, anybody wanna talk about the face tonight? My goodness, y'all. This is hilarious. Oh my gosh. All right, y'all, there's not even a question here. Hi, Heather. Hi. All right, let me, Let me see what's going on in my private community. Let me see who's joined. Let me see. All right, who do I have? Let me just— All right. Danielle, did you go in the circle? Gosh, I can't even speak. Let me see. All right, I'm going into the circle. So you go to drrobbscircle.com. It's free to join, or you can go to my bio and click it. It's free to join. That's where we're gonna go, all right? I'm gonna type it in here. Don't let me down, Instagram, come on. All right, there it is. Hi, Danielle, awesome. Hey, Leslie just joined, I think you missed the talk. Hi, Leah. AJ, Madonna. So I want y'all to head, Heather Armstrong, oh my gosh, Heather liked everything, that's awesome. I want Heather Armstrong to go into drrobcircle.com. Can somebody type that on, how do you type that? Enhance voice. So stupid, I don't even know how to do this. TikTok baffles me, man, I swear. So nuts. I don't know how to manage it. Hi everybody. TikTok is out of control. I don't know what to do with it. Poll, GIF vote, super fan box, viewer wishes. Can I just literally comment? Oh, I can, look at that. Wow. All right. We are going to Dr. Rob's circle for private Q&A. Okay. Now. So get yourselves over there. Come on, let's go. I'm gonna answer that in just a minute, Leah. I feel like I got to leave TikTok and Instagram on because they're like the most interested parties here. The other ones, I don't know. We'll see. All right, hold on. I'm gonna move y'all. I'm gonna leave you on a while longer 'cause you've been super interesting tonight, but I'm gonna put my folks in the middle here. And let me see. I gotta turn off some things because I'm gonna stop streaming on some of these other platforms here. I just have to go into, There we go. Okay. So I'm just gonna remove some of the other platforms now because we're gonna stop streaming on those. And we'll stay on TikTok, Instagram, and my Circle for now.
