Dr. Charles Mok explains when to place breast implants on top of the muscle versus underneath. On-top placement causes less pain and bleeding, looks more natural, and avoids distortion from muscle movement, but requires at least an inch of existing breast tissue. Under-the-muscle placement is reserved for patients with very limited tissue.

Questions this video answers

If you can pinch more than an inch of breast tissue, generally the surgeon will place the implant on top of the muscle.
