Dr. Charles Mok argues that testosterone replacement therapy guidelines rely on vague thresholds that don't match how endocrinologists actually practice. A large study found many men treated by specialists had testosterone levels within the normal range, exposing gaps between guidelines and real clinical decisions.

The guidelines for what level of testosterone determines which men will benefit are quite vague. So we look at studies comparing different specialists. Doctors had the most expertise in hormone replacement would be endocrinologists. This is a residency that doctors undertake to specialize in hormone replacement therapy and they generally go by symptoms rather than by an absolute level. In fact, a large study showed that the most likely men that had testosterone levels that were considered normal at gap replacement were by endocrinologist because they had symptoms of low testosterone, even though their testosterone was levels were in this so called normal range. Because we know that normal range is not really helpful. It just means that that's where 99% of men fall.
