Short educational segment (YouTube) by Dr. Charles Mok explaining when implants are placed under the muscle versus on top of the muscle. The speaker states that on-top (subglandular) placement is now often preferred for patients who have enough native breast tissue (he gives a verbal threshold of being able to "pinch more than an inch" of tissue) because it causes less motion-related distortion, less postoperative pain, and less bleeding and can look more natural. Submuscular placement may still be required when breast tissue is very limited.
Questions this video answers
If the patient can pinch more than one inch of breast tissue, the surgeon will generally place the implant on top of the muscle.
