In this talk Dr. David Perlmutter explains that Parkinson's disease (PD) is fundamentally an energetic disorder caused by mitochondrial dysfunction, specifically a defect in complex I activity. He introduces the ketogenic diet—a high‑fat, low‑carbohydrate regimen that raises blood ketone levels—as a way to provide an alternative fuel that can bypass this mitochondrial defect. Perlmutter reviews a small feasibility study (Treatment of Parkinson Disease with Diet‑Induced Hyperketonemia) that placed five PD patients on a strict ketogenic diet, monitored ketosis via blood ketone measurements, and evaluated outcomes using the Unified Parkinson’s Disease Rating Scale (UPDRS) across four domains: mentation/mood, activities of daily living, motor function, and medication complications. The results showed consistent improvements in all participants, with dramatic reductions in total UPDRS scores (46‑81% improvement in three patients). He emphasizes the study’s limited size, advises that patients consult healthcare providers—especially if on diabetic medications—and suggests that dietary interventions may be a valuable adjunct or alternative to pharmacologic symptom management in PD.
