In this solo podcast episode, Dr. Anne Truong (MD) warns listeners about the systemic effects of sildenafil (Viagra) and tadalafil (Cialis), especially when combined with antihypertensive drugs such as losartan and amlodipine. She explains how these PDE‑5 inhibitors increase cyclic GMP, causing vasodilation throughout the body, which leads to common side effects (headache, nasal congestion, dizziness, blue‑vision) and can dangerously lower blood pressure in patients already on blood‑pressure medication or nitrates. Truong stresses that escalating doses (e.g., 100 mg or 200 mg) signal worsening erectile dysfunction (ED) and increase the risk of severe hypotension, fainting, or even death. She pivots to a “root‑cause” philosophy, identifying sleep deprivation, low testosterone, chronic stress, obesity, poor diet, and vascular stiffness as primary drivers of ED. She outlines a lifestyle‑first protocol: Mediterranean diet, daily moderate exercise, stress reduction, adequate sleep, and limited alcohol, arguing that these measures can lower blood pressure, improve vascular health, and often eliminate the need for medication. Truong promotes her commercial offerings—the Modern Man Club (online “Get Hard System”) and an in‑office “Get Hard Protocol” in Virginia that combines stem‑cell therapy, shockwave, Botox, hormone optimization, and lifestyle coaching for a $16,000 investment. Throughout, she provides URLs (noedman.com, sexualhealthformenpodcast.com, eatfornoed.com) and encourages listeners to share the episode on social media, subscribe, and download a free guide on common ED mistakes. The episode blends medical education, risk mitigation, lifestyle counseling, and marketing of paid services.

What will happen if I take 100 milligrams of sildenafil, right? So this kind of aligned with what I was saying before about when you have ED and you take ED medication, when you take it, you end up worse off than you were. Okay. So what will happen if you take 100 milligrams of sildenafil? 100 milligrams is a pretty high dose. Sildenafil is generic for Viagra. So how these medications work is that it increases a chemical called cyclic GMP, which then increases nitric oxide and therefore increases blood flow to the blood vessels in the penis. But it also increases more blood flow everywhere too, meaning to the lung, to the sinuses, and everywhere else. So that's why when you take it, you have congestion and you have dizziness because it opens up blood vessels. It will lower blood pressure. When it lowers blood pressure, and if you're already taking blood pressure medicine, it drops your blood pressure. Therefore, you have dizziness, you have headache, you have blue vision. And then you have the nasal congestion as well. So it does lower your blood pressure, especially if you're already taking blood pressure medication. The higher the dosage, the more the severity. And honestly, if you find yourself increasing the dosage in your Viagra, that's not a good sign either. It's a sign that your ED is getting worse. If you're going to take Viagra, start at 20, but then you find that you need to go more, then you really need to address the root cause. Oftentimes, If you treat the root cause, you don't need sildenafil. Okay. So the higher the dosage of sildenafil, the more chance that you're going to get more side effects. One of the common side effects is nasal congestion, blue vision, headache, dizziness. Sometimes you can pass out too, because your blood pressure drops. And you'll have like back pain as well. But the most common is the headache, the congestion, sometimes back pain. But just know that if you take it, you're really not addressing the cause of ED. Sometimes the cause of ED could be that you may be Maybe you need to sleep more consistently at nighttime, and fixing that will improve it. Maybe your ED is due to low testosterone, and then just taking a little bit of testosterone will fix that as well. And then also, maybe it's due to stress, and then alleviating the stress will also fix the ED, so you don't need medication. So, to answer your question, Milton Hunter, the higher the dosage of sildenafil, The more the side effect. I'm not anti-medication overall. I just want you to treat the root cause of ED so that way you don't have to rely on medication. So that way you have a reversal of your ED and you can continue to have sexual activity to the day you die, not be dependent upon medication. Just be careful when you're taking high dose of sildenafil. I've heard of some people will take 200 milligrams of sildenafil. That's just way too much. When you take that much, your blood pressure is going to drop. And sometimes you hear people passing out or even death taking Viagra because the blood pressure dropped. The blood pressure dropped tremendously. And therefore you will pass out. That's when you have like the dizziness there. So find yourself taking ED medicine, take a step back and find out why you have ED. Sometimes it's as easy as sleep and stress. And all you have to do is just change your diet and just walk. So next question is, I have high blood pressure and take medication for losartan and amlodipine. What happens when I'm on it and take either Viagra or Cialis? Well, losartan and amlodipine, if you're on 2 medications, that means your blood pressure is not being well controlled, and that's why you need to be on 2 medications. But what causes high blood pressure in the first place? Let me explain to you what high blood pressure is. So imagine your arteries, right? Your arteries are the pipe that when your heart pumps out blood, it goes through a vessel. A vessel is like a pipe. Think of it like a pipe from your heart It goes out, right? That's your artery. The artery is lined with muscle, it is lined with fibrous tissue, it's lined with collagen. So when blood is delivered from the heart out, that's artery. And then the heart pumps out the blood out with oxygen. So what high blood pressure means? High blood pressure means that the arteries, when blood's being pumped out through the heart, the arteries, which are the blood vessels, are stiff. They're not flexible like this. They're stiff. So it's not accommodating. So therefore, when the Blood vessel gets stiff, there's pressure as the blood flows through it. That's what causes high blood pressure. High blood pressure mainly due to you being overweight and also not exercising. When you're not exercising, your heart doesn't work as hard and your artery doesn't stretch as much, so it becomes like kind of more stiff. So when you're not exercising, And then when you're overweight because you're not following a healthy diet, when you're overweight and when you're not exercising, that causes the blood vessel to become stiffened, to become not flexible, and that's what causes high blood pressure. So high blood pressure means that there's tension in the blood vessels. Okay? So that's why you take losartan, amlodipine, and what those medication does is that it relaxes the muscle around the arteries. So that's why it makes the arteries more flexible, relaxes a little more, and that's why it lowers your blood pressure. So what happens when you're on it and you take Viagra? Because those medicines lower your blood pressure. When you take Viagra or Cialis, it lowers the blood pressure some more because Cialis and Viagra, what they call vasodilator, they open up the arteries, so therefore they actually lower Blood pressure. So you got to watch your blood pressure when you're taking Viagra or Cialis. And that's one of the side effects, such as dizziness, headache, and nasal congestion. So you got to really watch it. And in fact, if you take nitrate, you shouldn't be taking ED medication as well. And you've got to watch your blood pressure because you may drop and you may get dizzy and pass out. Okay? Always discuss This with your doctor first. Okay, how are you going to treat this? Well, your goal is maybe exercise 30 minutes every day or every other day, get on the Mediterranean diet. When you get on the Mediterranean diet, you're going to lose weight. You exercise, you're going to lose weight. In fact, your blood pressure will be better controlled so that way you don't need to take losartan, amlodipine. And losartan, amlodipine are actually medication that does not affect erection as much compared to like a beta blocker. So your goal is to eat better and lose some weight so that way you can bring down your blood pressure. And then when you bring down your blood pressure, then you may not need Viagra or Cialis because when your blood pressure is high, that's going to cause decreased blood flow. Because when you have high cholesterol, it can also contribute to high blood pressure as well too. But we do know with proper diet, weight loss, and exercise, blood pressure Does come down. Stress also increases blood pressure too, so you got to be aware of that. My endpoint is be cautious and check your blood pressure. There are blood pressure cuffs at home that you can wrap around the wrist. You can measure your blood pressure, follow it along, because you should expect probably a drop in your blood pressure while taking Cialis or Viagra. But I'm challenging you to, instead of taking the medicine, try to work on reversing your high blood pressure. By eating better, by exercising, by sleeping and controlling your stress. Give yourself a 90-day, 3-month timeframe. Just follow the Mediterranean diet. You can get the Mediterranean diet at eatfornoed.com. Download that Mediterranean diet and exercise. I mean, walking 30 minutes. You don't want to walk? Go on YouTube, do Tai Chi for Fat Loss. There's a video for 25 minutes and it's free. Or just go out and do yard work, up and down the stairs. Be really very, very cautious with alcohol. Alcohol is probably one of the common causes of ED. And I'm not saying don't drink alcohol. You can drink about 2 drinks a day or 14 drinks a week, and it shouldn't affect your erection. So I challenge you to work on lowering your blood pressure instead of just taking the medication. That is what you need to be looking at instead of trying to take Viagra or Cialis. Okay, Modern Man, if you're struggling with weak erection, low energy, the food you eat could be the big part of the problem.
