Why Round 2 Takes So Long After 40 (Refractory Period Explained)
13 Oct 2026 · 11 min
Dr. Anne Truong explains why the refractory period after ejaculation gets longer after 40. Dopamine and testosterone decline with age, and medications like SSRIs and beta-blockers can make it worse. Stress, low arousal, and vascular stiffness all play a role, though most research comes from animal studies, not humans.

Hello there, modern man. If you look up the male post-ejaculation refractory period, you'll find almost universally agreement that it naturally gets longer as men age, taking anywhere from minutes in the young adulthood to hours or even days later in life. Here's what might surprise you. There is almost no published clinical data directly providing that age alone increases refractory period. Yes, I know, surprisingly, right? Most of the study actually come from rats, believe it or not. The human evidence base surrounding the refractory period is surprisingly very thin. Much of what medicine understand today isn't based upon direct observation trials of recovery time, but on what we know about the brain, the ejaculation inhibition, and the sexual pathways. So, So today I'm going to break down what actually causes the refractory period to lengthen and the brain chemistry driving it and the most common modifying factors that you can follow. Most men think losing peak performance is just getting old. I will tell you it's not. I'm a medical doctor. I'm a sexual health specialist. I specialize in erectile dysfunction. I've treated over 7,000 men with ED over 25 years. Let's talk about the brain chemistry. At its core, the refractory period, meaning the time that it takes for you from post-ejaculation to erection, isn't just physical fatigue in the body. It is driven by a shift in the brain chemistry towards inhibition or inability to get an erection again. What that means is that sexual desire and the penis Reflex capacity are primarily driven by actually 2 chemicals, dopamine and testosterone. So following climax, the brain nervous system shifts into an inhibit state. So when this inhibit tone increases, or when dopamine arousal drops, the recovery window widens. What tipped the scale? First, The arousal and psychological stress. The refractory state requires sustained brain arousal to overcome. Fatigue, high stress, and low erotic stimulation prolong the time needed to achieve the subsequent erection. In fact, continuous sexual stimulation is one of the few interventions experimentally demonstrated to shorten the recovery time. Quick note before we go any further. While the claim that aging Lengthened recovery time rests primarily actually on clinical authority rather than large published trial. So mechanically it makes sense, but as men age, natural dopamine tone decline, testosterone level decrease, also their blood vessel stiffness arise. So 3 factors that contribute to ED also contribute to— Erectile dysfunction. Lengthened refractory period. Third, ED, erectile dysfunction. Often it is described as long refractory period is functionally mild or moderate ED. Yes. So if you have a long refractory period, it is almost synonymous with mild to moderate ED. So if the pelvic blood flow or nerve signals is compromised, achieving another erection after climax Takes longer period of time, even if the desire has returned. So I hear men just say, well, I still have the desire. Why is it taking so long for me to get another erection? This is what happened. The single most important and modifiable cause of prolonged refractory period is medication. Yes. Medications such as serotonergic antidepressant or SSRI, increasing Central serotonin directly raises the ejaculatory time. Sometimes mean is that intentionally raising the serotonin actually treat premature ejaculation, like Paxil, the medication that's used frequently to treat premature ejaculation. Another medication is dopamine-blocking agent, antipsychotic, that antagonize dopamine receptors and therefore reducing sexual desire and prolonging the ejaculatory time. Other common prescription is alpha blockers like tamsulosin, which affect the smooth muscle tone of the penis and also the ejaculatory mechanism. Or another medication, 5-alpha reductase inhibitors like finasteride, dutasteride, or beta blockers, and even chronic opioids use. These all suppresses the brain hormone. Now, I'm not telling you to stop this medication. I'm just saying some of these medications can contribute. However, discuss this with your doctor before you discontinue any medication. So next, let's talk about other metabolic and endocrine drivers as well. So finally, systemic disease play a direct role. Low testosterone or hypogonadism. Testosterone fuels the central sex drive and also the sensitivity in the penis. So when levels are low, recovery slows down. Next, diabetes. Men with diabetes experience higher rate of delayed recovery due to, yes, endothelial dysfunction, reduced nitric oxide, and small— Nerve damage that dampens the reflexes required for another erection. So the third condition is hyperprolactinemia or high prolactin. So high prolactin suppresses dopamine and testosterone production, dampening the sexual function. To summarize, while the refractory period is an entirely normal neurochemical reflex, a sudden And disrupt the lengthening usually point to vascular health, hormone levels, arousal, and possible medication effects. So if you notice sudden changes in your recovery time, discussing this with your doctor and getting blood work done, get your testosterone measured, be evaluated for erectile dysfunction with the sexual health specialist. And if you found this breakdown useful, Make sure you go see a sexual health specialist to discuss about this and not just try to treat it yourself because it could really mean the beginning of erectile dysfunction or maybe the beginning of even diabetes. So get your blood work done, go see your doctor. And if you found this breakdown helpful, click the like and subscribe button because it means a lot for me if you do.
