This Journal of Clinical Neuroscience case report (2018) describes ketamine infusion used for refractory status epilepticus (RSE) across a retrospective series of nine patients and details one 72-year-old woman with aneurysmal subarachnoid hemorrhage (SAH) who developed atrial fibrillation, recurrent asystole and cardiac arrest shortly after ketamine initiation. Of the nine patients, two had cardiac events (one sinus tachycardia; one with multiple arrhythmias/asystole). After stopping ketamine and initiating antiarrhythmic therapy, the patient stabilized. Authors conclude ketamine should be used cautiously in RSE patients with SAH due to potential heightened risk of life-threatening arrhythmias.
