Systematic review (published online 12 Feb 2014) assessing the effectiveness of NMDA receptor antagonists—primarily ketamine—for control of refractory status epilepticus (RSE) in humans. The authors screened multiple databases (inception to Sep 2013), identified 23 relevant articles (16 manuscripts and 7 meeting proceedings) covering 162 patients (110 adults, 52 pediatrics), and report electrographic seizure responses of 56.5% in adults and 63.5% in children. Evidence is low-quality overall (Oxford level 4; mostly GRADE C), adverse events were rare, and the authors call for prospective trials of early ketamine administration.
