What really happened to Michael Jackson?

Dr. Nyree Penn, an anesthesia professional, explains how Michael Jackson died from propofol intoxication. The coroner ruled it homicide. Jackson had drug levels matching general anesthesia, but received it at home with no monitoring equipment or rescue drugs present, and benzodiazepines made the depressant effect worse.

Hey Dreamers, you know, unfortunately there's something almost everyone remembers about Michael Jackson's death. Propofol killed him, and that statement is grounded in the official medical record, but it's not the full clinical story. I'm Nyree Penn. I spent almost 20 years in anesthesia, and I really want to explain his case the way anesthesia professionals see it, with respect, with With precision, without the sensationalism, and with medical understanding and knowledge. First of all, the official finding according to the Los Angeles County Coroner, they listed the cause of death as acute propofol intoxication. The manner of death was homicide. The autopsy also listed a benzodiazepine effect as a contributing condition. That distinction matters. Propofol was the immediate identifying factor according to the medical examiner, but we should not rewrite or dismiss the fact that there was propofol found in his system. The toxicology report found propofol along with lorazepam, midazolam, diazepam, lorazepam, lidocaine. These are all benzodiazepines, right? Not every drug detected in his system should be called independently fatal. The clinical important point is that the coroner specifically identified a benzodiazepine effect as a contributing factor. Why is this important? Because when you mix that with propofol, independent of the benzodiazepines, propofol can cause rapid loss of consciousness. It is a respiratory depressant. It depresses breathing. It lowers blood pressure. It changes your cardiovascular function. The current FDA label for propofol warns that benzodiazepines and other sedatives can increase propofol's sedative and respiratory effect. In plain language, taken together, it has a compounding effect. The coroner's anesthesia consultant wrote that propofol concentration was similar to what we see during general anesthesia for major surgeries. Think about it. In the operating room, what surrounds the patient? You have an anesthesia professional, you have a continuous infusion, oxygen, blood pressure, EKG, pulse oximetry, capnography to detect whether or not you're breathing, airway management, airway equipment, emergency equipment, emergency resuscitating equipment. All of this is in the operating room, not in someone's bedroom. The coroner then concluded that the standards were not met for this type of drug that was given in a non-hospital setting without the appropriate medical indication. There was no infusion pump at the scene. The monitoring that was recovered was not in use where the patient was found. The equipment and readiness required to resuscitate and rescue a patient was not found. From sleep apnea, obstructive airway, cardiovascular collapse—those measures were not in place, unfortunately, and that was the result. This is why I would not describe this simply as a story about a dangerous medication. Propofol is used safely every day for the. Approval of anesthesia and sedation procedures. By the way, this message is not supported by propofol. They are not a sponsor in any way, shape, or form. However, this is what we use at Insomnia Sleep for our safe anesthesia professionals, by our safe anesthesia professionals, in a safe manner, in a safe clinical setting. Because safety depends not exclusively on the anesthesia It depends on the dose, the patient, the selection, continuous observation, psychological monitoring, airway expertise, and immediate rescue capabilities. The drug and the system cannot be separated. Only anesthesia providers should be providing this drug. So let us address the question that many people still have: Why was Michael Jackson requesting propofol? It was. Meant, so he was on propofol, apparently, according to the California appellate opinion. He had a history of using propofol dating back to the 1990s. And his belief was that the intervention reliably worked for him. Unfortunately, in this particular manner, it was not upheld. And his belief in that was intervened and that reliably worked for him for a while. So I will say propofol sleep is not the same as natural sleep. However, propofol does induce the same neurological and restorative benefits seen during restorative sleep. Propofol doses do seem to induce neurobiological and EEG features of sleep. And research has shown that propofol induces sleep slow wave activity. REM-like waveforms. We have studies that show that there are important differences, including altered slow-wave organized and spindle behavior. And although there's been very limited, limited clinical research exploring propofol and propofol protocols for refractory insomnia, at Insomnia Sleep, we have developed a framework to give clinical anesthesia in a safe Way with anesthesia professionals to safely induce neurorestorative sleep, the type of sleep that Michael Jackson was looking for. And I do believe if he was around today, he would have a song about us. I also do have to disclose that propofol has not been indicated or approved for the use of insomnia or any other sleep disorders. At Insomnia Sleep, we use the off-label use of propofol to induce neurorestorative sleep. So the larger human history and the real, the bigger story, the larger human history and human story is really sleep desperation. When someone like Michael Jackson is under extreme pressure for so long, it's understandable that they cannot sleep. And they begin to believe that only stronger sedation will work. More sedation is needed. And so the risk is not just one medication, it's the escalation of another dose, another medication, another prescription, another provider, alcohol, the over-the-counter use of another product or substance, the outside use of a safe and controlled anesthetic is a consideration. So we are not surprised that Michael Jackson used propofol for sleep. However, the procedure was done Outside of a safe environment with the appropriate medical professionals that it requires. So for you, if this sounds very familiar, please do not abruptly stop your med— your sleep medication or your benzodiazepines. Do not change your sleep medication in any way because of the information I'm disclosing now. The appropriate discontinuation is necessary because you cannot just abruptly stop these neurological type of medications.
