Hi there, I heard you need help uploading documents to your Power to Patient Portal. Not a problem. We are happy to help. This quick video is going to walk you through it step by step. First sign in to your account. Go to mdlifespan.com Click Login link in the top right corner of the page. Enter your username and password. Click the Orange sign in button on the left. If you have trouble with this step, please reach out to our client liaison for login details. Now that you're logged in, you're going to click on messages in the menu. On the left side of the screen, click the orange button that says Compose. Please type in your doctor's name for the subject. Please type Medical Records and Date Completed. Click the orange button that says Attach PDF for title. Add a description of the records and the date it was completed. Next to upload, you should click the Browse button and select the document. If you want to send more than one document, please click the white button with orange outline that says Add Document. Repeat these steps until all documents are uploaded. When you're all done, click the Upload Orange button in the bottom right corner of the screen. Double check the name you're sending these documents to, and click the Orange Send button in the bottom right hand corner. Then you're all done. The Longevity Team member will review your documents before your next appointment. If you have any questions, please reach out to your client liaison for assistance.
