Revolutionary Research: Exosomes and their Impact on ED | Dr. Anne Truong MD

Relationship between Exosome and ED well, Exosome is a small extracellular vesicle. What that means is just a balloon that comes out from a cell. It is very, very small. It's only 30 to 150 nanometer in diameter and, and it's enclosed by fat. And it contains actually interesting molecules such as protein, lipids, DNA, RNA and metabolite. This is what your body produces, is involved in various cellular processes including how cell talk to one another and also in your immunity response. But it's also been used for other therapeutic application as well. So some of the potential use of Exosomes include using it as a biomarker to follow tumors and cancer and track the cancer progression. And then another use is cell free therapeutic agent where exosomes or little small vesicles are loaded with medication or drug or gene and is delivered to the target cell. Because when these little vesicles travel in the blood, it goes to the target cells and the target cells will kind of take it in. And it sometimes can be used to treat diseases such as cancer or nerve diseases and even autoimmune diseases as well. It can also be used as drug delivery. It can also deliver medication to reduce the side effects of a drug. It can also be used for research purposes as well, to see how cells can can communicate with one another and how disease can develop and how drugs work. The research on Exosome is still very, very early. We still don't know what it does, but we do know that it has some therapeutic purposes, but we don't know everything. I think we know at this time probably 1%. And it can revolutionize how we diagnose, treat and prevent diseases. But we still don't know a lot about it. Okay, here are some additional facts. Exosome is that it is produced by all the cells of your body, but it's more in abundant in cells that are cancer cells, that are stem cells and that are immune cells. And Exosome can travel long distances in your body, in your blood, and it communicates from cell to cell and that it can also be found in blood, fluid and saliva. So it makes them a potential source for biomarkers to follow diseases. Okay. However, Exosome is not FDA approved as a drug or a treatment for any type of condition. That is something to know. So if you're considering going to a clinic that uses Exosome, it is not approved by FDA for the purpose of any therapeutic use at this time and is not approved as a treatment modality or as a medication. However, There are several clinical trials underway to investigate the safety and efficacy of Exosome in various diseases at this time. So if you're considering treatment, just know there's no guarantee of safety or efficacy and that Exosome is still new and emerging. I think it has a lot of potential in the future, but we need to see what the research shows, that we need to know more about what exosomes really do at this time. And so these treatments can be unsafe and could be ineffective and they're very costly. So be very weary now. I do not use Exosome in my practice at this time because I still think that we need to wait to find out what exosomes do. And also we have to see their regulatory constraints with Exosome at this time. I have seen patients that has had exosome treatment for their ed, but they don't get any result or have any resolution of their ED symptoms. What I want to point out is that ED is a complex symptom. It is a symptom from a disease. It is not a disease itself. ED is a symptom of high blood pressure, it is a symptoms of peripheral vascular disease, it is a symptom of high cholesterol, symptoms of obesity. It is a symptom of cardiovascular disease. Therefore, treating ED involves a comprehensive approach like my get heart system where we look at six system approach to treat ED from multiple angles. So you can't just treat ED from one treatment like Exosome and ignoring the brain, the emotion, the hormone, the nerve and the blood vessels as well as the muscle. In my get heart system approach we look at the sixth system which is the brain and then the emotions and then the hormone and the nerve and the blood vessels and the muscle and the get heart system treat every facet of that. So that way we can really reverse edge. So just getting one modality treatment with exosome may not do the job. Sometimes simple things such as changing your diet, exercise and sleeping at night, stopping smoking and moderating alcohol will go a long way. So Exosome has a way to go on its efficacy at this time. However, there are two studies, one in 2019 in the STEM cell translational medicine researcher has found that exosome derived from MEEN stem cell could improve ED in a rat model at this time and that exosome has shown to repair damaged tissue in the penis and improve blood flow. But that is in a rat. In another study in published in a journal of Biomaterial in 2020 found that exosome from fat could also improve ED function in a rat model at this time. There's no human study for exosome at this time and in this study it has shown that exosomes improve the production of nitric oxide, which is a molecule essential for increasing blood flow. So there is more research that needs to be done to confirm these findings and to determine efficacy and safety of exosome therapy in human There are some ongoing clinical trials that are being done right now for Exosome4ed. There are clinical trials looking at exosomes in mesenchymal stem cell for ED, as well as from fat as well as umbilical cord at this time. So if you're interested in participating in this clinical trial, you can search for trials on the website of the National Institute of Health Clinical Trial Network. I will post that link on the show. Note so to end to summarize, exosome is a sexy idea. It makes sense because it is a vesicle that is produced by all the cells in your body and it does have some therapeutic implication of repairing damaged tissue and also improving blood flow. But it is still in its infancy, in stages. Only trials have been done in rat models for ED at this time. If you're considering getting exosome, just be very wary and cautious because we don't know its efficacy and safety at this time. I think there are some potential. I don't use Exosome in my practice at this point, but I'm waiting for more research to be done and I'm waiting to see what regulatory agency have to say about it at this point.
