You Won't Believe What's INSIDE Your Penis (2 Muscles & 3 Nerves!)

A YouTube episode by Dr. Ann (Anne Truong MD) explaining penile anatomy and how it relates to erectile function. The speaker distinguishes smooth muscle (corpora cavernosa and spongiosum) from skeletal muscles (ischiocavernosus and bulbospongiosus), describes three nerve contributions (parasympathetic, sympathetic, dorsal/pudendal S2–S4), and links abdominal and pelvic muscle health to erection quality. The episode also discusses age- and disease-related changes (tunica albuginea thinning, smooth muscle atrophy, venous leak) and recommends lifestyle measures (diet, exercise, sleep, stress management) and community support (Modern Man Club).
Questions this video answers

Do you know that the penis is just not smooth muscle, but it has skeletal muscle in it? That means it has muscles like your biceps and the special muscles of the penis mix in together. So we're gonna dive deep into the unseen network of what really is going on in the penis. It's not as straightforward as you think. Stay. Stay tuned to find out more about what's really going on down there. I'm excited to talk about this episode because I just learned something new as I was researching. I put in a lot of work every time I do an episode for you all by researching, see, what is the research behind all this? And I found that it was something very unique. You know that the fascia of your abdomen, called the linea alba, is connected to the penis. So what that means is that your penis connecting to your abdominal muscles. So if your abdominal muscles are weak, that can also contribute to your penis function as well, too. So I thought I was exciting. So this is the episode that you really need to watch this on YouTube. You need to watch it where you can look at the pictures I'm showing on the screen, because that way you can follow. Follow along with me. And if you're listening to this in the car, watch it again on YouTube because in the picture, you're really going to appreciate your anatomy and the penis anatomy. So in this picture that you see here and the top left is the picture of the penis with all the skin and everything out. So you have the glans here, and you have the corpus cavernosa, which it goes on both sides, but then you also have the curl, the corpus clavenosum, go down, and it looks like two legs on that picture, right? It looks like two legs down that goes along your pubic bone, right here. It goes along the pubic bone and spreads out like this, like a V. Your corpus cavernosum, your penis is not what you see along the shaft here. It goes along your pubic bone and spread out like two legs like that, like into a V. And then this is the corpus spongiosum. That is also a smooth muscle where the urethra, where you go urinate, go through that. So the smooth muscle part is the corpus cavernosum and the corpus spongiosum. And then you have the legs that goes down like a V, like this and that. Wrapping around the corpus cavernosum are two layers that is called the tunica albuginea. And there's one layer that wrap around like this circumferentially, or around in a circle, and there's another layer that goes up and down on the outside. So there's two layers that wrap around the corpus cavernosum. Why is that? Because those layers, the stronger they are, the. The better the firmness of your erection. So it goes in both ways, but they are involved in keeping the erection firm. And you don't have two compartment in the penis. So both of these muscle of the corpus cavernosum, even though there's two, they talk to one another. So you can inject one side. The other side will also have the medication as well. Children to do Chinese talk to each other on the bottom. And this here is the corpus spongiosum. Before I go, talk about what erection is on the part right here, there are skeletal muscle. And the skeletal muscles are called ischiocavenosis. Skeletal muscle that is kind of like sits on top of the crura or the legs of the corpus cavernosum. And you can see that in this picture that I have right here, where the ischiocavenosus muscle runs alongside the helix here in the back, here around the long side and the legs of the curl of the penis. And the bulbospongiosus muscle actually wrapped on the base of the penis, it wrapped around like this. So the ischiocal venosus muscle help with giving you a better firmer erection. And it does help with contraction. It contracts the pleura of the corpus cavernous and it contributes to an erection. And the bulbospongiosis muscle that wrapped around the base of the shaft of the penis help with ejaculation. So it squeezes to help bring out the ejaculate. And I find that's fascinating because we always thought that the ischiocal vernosus was a lot deeper, but it's actually involved along the shaft as well too. And this picture, you can see that right here, depicts it really well and how it connect to the smooth muscle. So the smooth muscle is the corpus cavernosum and the corpus spongiosum. And the skeletal muscle is the ischiocavenosus and the bulbospongiosis. Now, why am I talking about this? What's the difference between smooth muscle and skeletal muscle? The skeletal muscle is like your biceps and your triceps. Those are the muscle that has muscle fibers that goes within it. And it's voluntary. You can contract and relax it. You can control that as well. Versus the smooth muscle. It doesn't have all the different striation, but it is under the control of the autonomic nervous system. So it's controlled subconsciously. You can't voluntarily control that and that it is structured differently, where the smooth muscles are more structured to be more flat. And it doesn't hypertrophy as much as the skeletal muscle. The corpus cavernosum and the corpus fungiosum is controlled by the sympathetic nervous system and the parasympathetic nervous system, which is the autonomic nervous system versus the ischiocavenosus. And the bulbo spongiosis muscle are controlled by the somatic nerve or the dorsal nerve of the penis. The dorsal nerve of the penis come from the pudendal nerve, which come from the spinal root, sacral root 2 and 4. So why am I diving deep into that? I'm diving deep into this because I want to show you the intricacy between your abdominal muscle, which is connected to your penis, and the pelvic muscle, which you can see on here, which are on the base of the shaft as well as on along the pleura of the penis, which is along the pubic bone that are intricately wrapped around the penis as well too. And it does all contribute to the quality quality of your erection and that you have three different hyper nerve astronomy of penis. You have the parasympathetic, which is involved with erection, and the sympathetic, which is involved more with ejaculation, and the dorsal nerve, which is involved with the feeling, the feeling on the skin and on the glands, which is all involuntary. And it's also involved with ejaculation because it contracts the bulbo spongiosis muscle and the ischiocal venosus muscle. So there's a complex interplay between three different type of nerve and two different type of muscle interconnected in all the pelvic muscle and also in the abdominal muscle as well. So that's why there are so many ways that it can affect your erection. Maybe a weak abdominal muscle can affect the quality of your erection, or a weak pelvic muscle can affect your quality of your erection as well. So having said that, like for instance, if you have diabetes, which can affect the nerve and the muscle, that is the risk factor for quality of your erection as well too. When you have ed, when you age, the tunicle albuginea, which wrap around the muscle, it doesn't encircle the whole muscle, There are some parts that are thinner and. And then the smooth muscle or corpus cavernosum also become atrophy and also has scar tissue in it. And then you have incomplete filling of the blood in the corpus cavernosum and therefore is not able to compress the veins that are on the outside of the penis, which you can see here. So when you have an erection, the veins that are on the the outside of the tunica albuginea gets compressed and that's how the blood stays in the corpus cavernosum like you see in the picture here. So that's what happens when you get older. Now that means that you cannot do anything about it. Of course you can do something about it. Right by again, the four pillars of sexual health is diet, exercise, sleep and stress management. So you are just not sol right. There are some things you can do about it. So having said that, I know this may be a little nerdy episode, but I wanted to kind of give you the whole picture that hey, your abdominal muscle, your pelvic muscle, everything are interrelated and it is a complex area down there. Even doctors have difficult time even learning about the anatomy and that there are multiple steps as to can affect the quality of your erection. But I do believe the more you know, the more you know. So information is powerful. So having said that, I hope you enjoyed this episode. It will make me feel wonderful.
