In this Hindi‑English interview recorded for Diabetes Month, host Dr Preeti welcomes senior diabetes consultant Dr Pankaj (Narayana Hospital, UK‑trained fellowship) to discuss why diabetes is rapidly becoming an epidemic in India and what can be done about it. Dr Pankaj attributes the rise to a complex mix of lifestyle changes (sedentary habits, poor diet, stress, sleep deprivation), environmental pollutants, and especially widespread pesticide and industrial chemical exposure (DDT, glyphosate/glifosate). The conversation moves to how these hidden risk factors contribute to insulin resistance and beta‑cell dysfunction. They then outline the broad clinical spectrum of diabetes, highlighting atypical presentations such as recurrent urinary tract infections, frozen shoulder, neuropathic pain, gastrointestinal disturbances, and variable weight changes. Severe acute emergencies – diabetic keto‑acidosis (DKA) and hyper‑osmolar hyperglycaemic state (HHS) – are described with emphasis on ICU management, insulin protocols, osmolarity monitoring, and the role of dehydration. Dr Pankaj also explains how glucose variability, hormonal swings (cortisol) and mood fluctuations affect disease control, and why patients may experience muscle loss despite weight gain. The interview closes with a call for greater public awareness during Diabetes Month, community‑level prevention (diet, exercise, reducing pesticide exposure), and the value of structured group programs for type‑1 diabetes patients.
