Gharsat Amal - Hematology Oncology Medical Student Module 2

In this bilingual (Arabic‑English) lecture, Dr. Hassan Bencheqroun reviews five classic coagulation patterns (DIC, warfarin, heparin, liver disease, TTP/HUS) and how to diagnose them quickly using platelets, PT, PTT, and fibrinogen. He illustrates the appropriate reversal for warfarin‑related bleeding (vitamin K + four‑factor PCC) and emphasizes the urgency of plasma exchange for TTP versus supportive care for HUS. The session then shifts to peripheral blood‑smear interpretation, describing five key morphologic clues – Howell‑Jolly bodies, bite/Heinz cells, schistocytes, target cells, and sickle cells – and explains the pathophysiologic mechanisms that generate each finding. Next, Dr. Bencheqroun explains serum protein electrophoresis, differentiating MGUS, smoldering myeloma, and multiple myeloma using M‑protein level, plasma‑cell percentage, and the CRAB organ‑damage criteria. A case‑based question reinforces that an M‑spike < 3 g/dL with < 10 % plasma cells is MGUS, requiring monitoring only. Finally, he outlines the hierarchy of transfusion reactions (acute hemolytic, febrile non‑hemolytic, TRALI, TACO, anaphylaxis), stressing the universal first step of stopping the transfusion before diagnosing. Throughout, the lecture uses pattern‑recognition, clinical pearls, and rapid decision‑making algorithms aimed at medical students preparing for exams.

It's a pleasure to be back here with you. I am going to tackle the second module, oncology. The first one, we talked about some concepts on how to differentiate between lymphoma and leukemia, acute and chronic. And so on. What we're going to do today is going to speak about coagulation, about blood smears, myeloma, and transfusion. So let's start. First thing that we're going to be discussing today is there are 5 patterns that we're going to be talking about. Every coagulopathy. So every coagulopathy case boils down to reading four numbers quickly. Four numbers: platelets, PT, PTT, fibrinogen. And once you know the five patterns, you can call the diagnosis. Very quickly as well. So first thing that you talk about is What is this? It's called DIC disseminated intravascular coagulation. DIC it's high likelihood of mortality ما يكون أي شيء طبيعي لو بدأ ابتدأ أحد من الأرقام يكون طبيعي this is not the diagnosis okay everything has to be not normal فشو هم ال five coagulation patterns اللي حنحكي عنهم نحكي عنها إن DIC Warfarin. The warfarin effect is a medication. The heparin effect liver disease. لأنه it is being consumed. Okay? لأنه الاضطحال بياخدهم and it sequesters them. Okay؟ الطب medicine is pattern recognition. Pattern recognition معناتها نتعرف على الأنماط لما بنشوف واحد ولا اثنين فيهم we recognize the patterns and we come to the diagnosis. So let's ask this question. مريض يبلغ عمره ستين سنة بيتناول الدواء اللي اسمه الورفرين for atrial fibrillation and he presents with gum bleeding. Okay. What is the best initial reversal for a major bleed؟ نديله platelets ولا نديله vitamin K and four factor PCC ولا نديله protamine ولا نعطيه plasma exchange؟ Five seconds. You got it right. Correct answer is B. لأن الوارفارين is anti vitamin K, so we need to give vitamin K, but we give also. Four factor PCC. And what is four factor PCC? It has factor two, factor seven, factor nine, and factor ten. هدول العوامل للتخطر الدموي بنعيد للمريض الفيتامين ك وبنعيد للمريض هدول الفاكترز لا يكون تعويض الفاكترز، and it will normalize the coagulation. A is wrong. Why? Because platelets are normal. Okay, it's a factor problem, not a platelet problem. C is wrong. Why? Protamine بتعكس الهبرين. So protamine reverses heparin, not warfarin. Different mechanisms. D is wrong. We do plasma exchange for TTP. لأنه TTP في antibodies and antibodies cause problem. So plasma exchange filters بصفة الدم من هدول الantibodies that cause the problem. So شو الخلاصة؟ We think of vitamin K pathway. Both of them الاثنين الاثنين في حالتين اللي في يكون في يكون يكون في hemolysis بس hemolysis in small blood vessels micro And geopathic small vessels hemolysis. معناتها إنه شضايا هالكريات لما when they pass by the spleen it takes a bite out of them so they become deformed cells وبنسميهم شضايا or شستوسايت. لما بتشوف الهموليسيس والشستوسايت على اللطاخة الدموية أو نسميه you think about microangiopathic hemolysis. Excuse me. Usually we see this diarrhea in children. HUS is frequent in children than adults. No antibody mediated. منشان هيك لو لو كان not antibody, we don't give steroid because steroids بيخفضوا المناعة. So they reduce the antibodies. If there is no antibodies causing problem, why give steroids? The treatment Usually هو supportive غالبا تدعيم طبي and it comes and goes on its own. There is no need for antibiotics. Why? Because the antibiotics are mostly if there is a bacteria that Stays there. This is the toxin of the bacteria that causes the HUS. So uremia, hidrolytic uremic syndrome. We do see hemolysis. بس لما بتشوفوا ال enzyme ADAMTS13 الاكتيفيتي بلو تين پرسنت النشاط تبع الانزيم أقل من عشرة بالمية في كمان أعراض عصبية نورولوجيك سيمبتمز and the TTP is antibody mediated so we could give steroids but the treatment is plasma exchange تبديل البلازما فوراً لا تستنوا الـ ADAMS13, TS13. You have to start right away because it could cause damage permanent or it could kill the patient. Don't give platelets alone. لأنو ال antibodies they destroy the platelets. So why give more platelets only to destroy them? And the more you give platelets and they get destroyed, they clot inside blood vessels. بيتخطر في الأوعية الدموية so they cause problems. Okay? Now let's do a quick game. One word. What's on the smear? If you have round cells In a patient with no spleen, if you see red cells with semicircular bite taken out of the membrane, if you see fragmented jagged red cell pieces, يعني هلالي من الغشاء تبع الخلايا وكأنه عضة fragmented jagged red cells. number four cells with a bullseye target pattern of central pigment خلايا ذات مظهر يشبه الهدف bullseye وآخر واحدة Crescent-shaped elongated red cells. Number one, round cells with a tiny basophilic dot inside in a patient with no spleen. Number two. Red cells with a semicircular bite taken out of the membrane is called bite cells. Number four, لما cells بlooks like bullseye target pattern of central pigment بنسميهم target cells الخلايا الهدفية. And then number five, crescent shape elongated red blood cells Sickle cell. So we we call it sickle cell disease. So when you see the lutacha, the demoya, the smear morphology, you recognize the patterns, and once you've seen it, then it's easier to recognize. The next one is, what is the mechanism behind each? We have identified what it looks like, but we want to know why it looks like that. How will body— how will jolly bodies, hadul baqaya, leftover nuclear fragments the spleen normally removes So if they are there, that means the spleen is not normally working, and that means it's a marker of functional asplenia. A معناته مفيش. Number two bite cells ولا Heinz bodies. Bite cells ولا Heinz bodies means it's denatured the hemoglobin. Spitten out by the spleen معناته إنه خلايا معضوضة هذي لل لما بتكون الهموجلوبين متخربة أو منزوع الطبيعة هاي الطحال بيشوف إنه فيها خلل وبيزيلها so there is a bite عضى by the spleen and it happens لما بيكون في نقص من الجي سكس بي دي ولا في oxidative stress. Number three, schistocytes. معناته إنه red cells they are destroyed and sheared and torn torn apart when they pass through damaged small vessels micro هذول معناتهم اسمه هذول خلايا دم حمراء تتمزق أثناء مرورها في أوعية صغيرة متضررة وبنشوفها في التي تي بي بنشوفها في الـ HUS بنشوفها في الـ DIC ولا بنشوفها كمان في الميكانيكال فالفز لما يكون عندك صمامات قلبية ميكانيكية Sometimes the red blood cells they get destroyed when they hit the the valve. It targets cells. There's an excess membrane relative to the hemoglobin, and that's why you have like a target like هدف. وآخر واحدة اسمها الـ sickled cell، هيدا معناته الـ hemoglobin S، it makes polymer معناته ترتبط بعضها بتندمج ولما بيكون الأكسجين لو، it makes the hemoglobin start to create polymers and الخلايا الحمراء become deformed. They get stuck and they cause occlusion. Let's talk about smear-based diagnosis. You have a six-year-old with sickle cell disease who has a routine smear showing Howell body, Jolly bodies. So طفل يبلغ عمره ست سنوات مصاب بفقر الدم المنجلي إجا لا يعني exam routine exam وعملناه له للطاقة الدم وشوفنا فيها أسسام حول جولي شو معناتها أولا active hemolysis is occurring right now b the spleen lost its normal Filtering function. Number C, the patient likely has a coexisting G6PD deficiency. Or D, this confirms a diagnosis of TTP. We know it's not. The spleen has lost its normal functioning. الثالثة patient has نقص في J six PD أيضا فوق من sickle cell. و number four this confirms بيأكد نتيجة تشخيص TTP. Which one you think? And you are right. Jolly bodies means The spleen normally will take them, and if they're there, the spleen doesn't work. يأما التحال was taken out by surgery ولا التحال has no blood vessel circulation so functional is splenia. Etc. Okay? اللي بنشوفه بالactive hemolysis is we would look at lactate dehydrogenase elevation, haptoglobin منخفضة والbilirubin is spilled in the blood. Okay. Number C is wrong. Why? Because bite cells and Heinz bodies are the ones for J6PD, not Howell-Jolly bodies. And D, TTP is diagnosed with schistocytes and the T thrombocytopenia plus neural findings, not Howell-Jolly bodies. In sickle cell disease So we have to think about vaccines, infections, and this patient is even though this Spleen is not there, is there, has not been taken out by surgery. It's not working. Okay. We are moving from the blood smear to something else. We will be talking about protein electrophoresis. And when we do protein ناخد سيرم ونعمل سيرم protein electrophoresis we want to see that all the protein are at the same normal level. في ذروة Spike طالعة كتير هذه بتعني إنه البروتين the body makes too much of it it doesn't mean always cancer so the question is how much and is anything damaged السؤال هو ما مقدار هالبروتين وهل حدث ضرر في الأعضاء ولا لسه؟ ما في مش ما في organ damage ما في ضرر للأعضاء ما في يعني إيه it's present it's there we don't know what it is unknown significance امغص smoldering myeloma معناتها إنه الميولوم هناك بس الactivity very low زي ما يكون عندك في نار، بس النار هابطة. Smoldering. It's not fire. It's fire that is smoldering. It's there. It could wake up at any moment, but هسا it's giving us peace. Smoldering myeloma معناته إنه ال M protein أكثر من ثلاث جرامات per deciliter، ولا ال plasma cells، ال plasma cells اللي جايين من السلالة of the lymphocyte B, B cells. So plasma cells are ten to sixty percent, but no organ damage. المالتيبل ميلوما معناته إنه ال اللازما سيلز أكثر من عشرة في المية مع كمان الكراب فيتشرز ورح نحكي على شو معناته الكراب ال C R A B أوكي or slim biomarker so let's talk about it كراب معناته C كالسيوم elevated more than eleven R معناته renal impairment, creatinine more than two milligrams per deciliter. ولا الكرياتينين clearance less than forty. A معناته anemia, hemoglobin less than ten. B معناته bone lesions. في one lesion of the bone lytic lesion. So let's repeat. What we repeat is crab معناته ارتفاع الكالسيوم C أكثر من 11 ملigram per deciliter قصور كلوى كلوى and renal impairment A أنيميا فقر الدم والB هي آفات عظمية وهي موجودة lithic lesion أكثر من واحدة على الأقل في التصوير. Any one of this plus 10% plasma cell, we call it multiple myeloma. We need to treat right now. So we go back. MGUS, معناته M protein less than three, plasma cells less than ten, no organ damage. Smoldering myeloma, M protein more than three, plasma cells ten to sixty percent, no organ damage. والmultiple myeloma فيه plasma cells more than ten percent and at least one CRAB feature. Calcium renal impairment, anemia or bone lesions. Let's do a little test. لما عملناه للbone marrow we see six percent plasma cells. So the M protein أقل من three. Bone marrow plasma cells أقل من ten percent. No organ damage. What is the correct answer? Multiple myeloma, لازم العلاج. Smoldering myeloma, close monitoring. MGUS, periodic monitoring, no treatment. Or D, this is a normal finding. Five seconds. And here we put correct, MGUS. Why? Because the M protein is less than 3, plasma cells is less than 10%, and zero CRAB features. It's MGUS. A is wrong because no CRAB feature. This does not meet myeloma criteria. B is wrong. M protein is less than 3, plasma cells is less than 10%. C is wrong. MGUS is a condition and it has a risk. A risk إنه في في خطر حقيقي وإن كان منخفض للتحول إلى ورم نقوي متعدد multiple myeloma بمعدل يقارب واحد بالمية سنوياً لذلك يحتاج إلى المتابعة we need To follow it. M spike is a monitoring trigger ف M spike is a monitoring trigger does not automatically mean cancer diagnosis. Let crab criteria not the spike itself decide if treatment starts. اتصلت بيك الممرضة وقالت لك واللي قالت لك the nurse calls you ten minutes into a transfusion doctor the patient just had a fever and says their back hurts. Ten minutes after you started the transfusion, You have to stop the transfusion immediately. Remember القسم الطبيب first do no harm فالإجابة إن نوقف نقل الدم فوراً ونشخص السبب بعدين ممكن نكمل نقل الدم بس أولاً diagnose before you do anything stop the transfusion so خلاصة الأمر. مورال of the story with any new symptom during the transfusion, the first move is always the same regardless of what you suspect. Stop the blood, then investigate. Let's talk about transfusion reactions. Fever plus back pain. ABO an ABO mismatch number two febrile non-hemolytic تفاعل حموي بس غير الانحلالي ما في هيموليسيس يكون حمى وقشعريرة ولا ما في انحلال دم no hemolysis LDH هابتوجلوبين etc. and it comes from the cytokines from the donor white cells meaning الدم اللي بننقلوه يكون فيه cytokines صادرة من كريات الدم البيضاء للمتبرع بتكون التهاب inflammatory cytokines number three trali transfusion related acute lung injury trali معناته بتشوفي ضيق تنفس مفاجئ sudden respiratory distress pulmonary edema ك and Plasma rich products. Number four is called taco. Okay? And in taco means now can you guess what taco stands for? Taco معناتها Transfusion-associated circulatory overload. في pulmonary edema، في water that comes out in the lungs, pulmonary edema، ارتفاع ضغط الدم hypertension، and زيادة الحجم volume overload لدى مريض قلبه ضعيف. Anaphylactic is when you have an allergic reaction So let's come down to conclusion الحالة النهائية عندنا مريض مصاب a trauma patient and received four units of blood rapidly quickly لأنه Trauma, hey? The fibrinogen low. A D-dimer طالع. Ten minutes into subsequent platelet transfusion, the platelet develops short of breath, and you start to see in the X-ray. Things that are not normal. قلنا what is happening and what organ damage is happening. ف what does the coagulation panel show? الخلاصة إنه trauma is one of the most common real world triggers of DIC. DIC. You have to think about it. Number two, stage two. سؤال الثاني what is happening with This breathing. DIC. And how do I tell it apart? حكيف بصنفه compared to a transfusion reaction to pulmonary? Well, trali means it's acute respiratory distress from donor antibodies without hypertension. عم بيقولولك no hypertension. Okay, لو في hypertension معناته we call it transfusion associated circulatory overload. Okay, and that will be due to give you hypertension. Okay, memorize the numbers not just the names. DIC, warfarin, heparin, liver and TTP and HUS. HUS Usually supportive. You just help your patient. Number three, the smear findings tell a story. How will Jolly splenic not working the spleen? Bite cells J six PD deficiency. Schistocytes And that's a shearing. So it's it means that there is microangiopathy. If we add CRAB criteria, it decides whether it's the treatment starts. Or not. And lastly, any transfusion symptoms stop first, diagnosed second. I hope that you enjoyed this. I hope that you will understand. Send me questions. I will be sending you questions on WhatsApp.
