Gharsat Amal - Hematology Oncology Medical Student Module 1

In this interactive, question‑and‑answer style lecture for medical students, Dr. Hassan Bencheqroun reviews the fundamentals of hematology and oncology with a focus on leukemia, lymphoma, and related myeloid disorders. He emphasizes the clinical importance of a single laboratory cutoff—20 % blasts in bone‑marrow or peripheral blood—as the WHO definition that separates acute leukemia from other hematologic conditions. The lecture distinguishes acute myeloid leukemia (AML) from acute lymphoblastic leukemia (ALL) using lineage‑specific markers (myeloperoxidase positivity, hour‑shaped rods for AML; TdT positivity for ALL) and discusses age‑related incidence patterns. Dr. Bencheqroun explains that leukemia and lymphoma are the same malignant clone presented in different anatomic locations, illustrating this with examples such as CLL/SLL and ATLL. He then outlines the characteristic features of Hodgkin lymphoma (CD15⁺/CD30⁺ Reed‑Sternberg cells, bimodal age peaks, B‑symptoms) versus non‑Hodgkin lymphomas. Critical cytogenetic translocations—BCR‑ABL1 (Philadelphia chromosome) in chronic myeloid leukemia (CML), PML‑RARA in acute promyelocytic leukemia (APL), and MYC‑IGH in Burkitt lymphoma—are highlighted as diagnostic hallmarks that dictate targeted therapy (imatinib for CML, ATRA/arsenic for APL). Clinical vignettes reinforce decision‑making: a 34‑year‑old with 24 % blasts is classified as AML; a 45‑year‑old with massive leukocytosis and full maturation spectrum is CML, progressing to accelerated phase when blasts exceed 20 %. The session concludes with key take‑aways: the 20 % blast threshold, lineage markers, the leukemia‑lymphoma spectrum, Hodgkin immunophenotype, and three translocations that change management.

It's good to be back with you all. We had a little bit of a hiatus, and now we are coming back to restart again these lectures. But the أشكركم for all of the answers that you gave me on WhatsApp. It gave me some topics to focus on that would most help you. It it tuli hematology and oncology and others. Today we are going to start with the hematology oncology, insha Allah. So I hope that you will have a pen and paper and start to think about answers. As we are going through. This is not going to be a normal course. It's going to be in the format of questions and answers. And then if there are any concepts that are not understood, you can tell me on WhatsApp and we can make another video for them. Let me share my screen. Okay, I hope everybody can see this. This is— we need to talk about hematology and some of the problems that, at least when I was in your shoes, I had trouble understanding. And we're going to start first With what's the difference between leukemia and lymphoma? And it's going to be twenty seconds. In twenty seconds, we need to learn one number. فا، if you have someone whom you suspect lymphoma or leukemia because when you see the analysis, the blood analysis, you see that the white blood cell count is elevated, and you do a bone marrow biopsy. When you look at the bone marrow biopsy. You will find that 18% blasts, and when you repeat it, it's 22% blasts. Same patient, same marrow, one number apart. The diagnosis will change. And لما بنشوف ال النسبة البلاست مرة بتكون 18% مرة بتكون 22% ف شو بتظنوا إنه نسبة 20% تحديداً هي اللي مختارة to make a difference between the diagnoses that you have. What do you think happens at exactly 20%? Well, 20% is the World Health Organization, the WHO definition cutoff for acute leukemia. WHO. Okay, so moral of the story: إنه الخلاصة في أمراض الدم ممكن a single lab cutoff can change a patient from let's just watch and wait or we need to do this now, we need to treat now. So these numbers are important. ف why twenty percent blast؟ Is the line. Twenty percent blast is the line because the blast هي خلايا دموية, the white blood cell count. The blasts are immature cells. They have lost the ability. ف it lost the ability to mature, but it kept the ability to multiply faster than normal. It works. It's it's on. بس more than twenty percent the blasts are so much إنه لما بتجاوز عشرين بالمية بيكون زحمة لهذه ال blasts و وهذه لما بتكون زحمة في النخاع العظمي it starts to crowd normal blood cells فهيدا بيؤدي إلى نقص شامل نقص شامل في خلايا الدم لأنه it is replaced by the blasts. So شو الخلاصة؟ Acute leukemia عيشين بالمية of blasts or more in the bone marrow or the blood. The blasts are too immature to work غير ناجة و ولا تستطيع أداء وظيفتها، فبتزاحم الخلايا الطبيعية and it crowds out normal cells. Number three, the results. شو النتيجة هي فقر الدم، أنيميا لأنه the space for red blood cells is taken by the blasts، فأنيميا والأنيميا بتعطي نتيجة اللي هي الإرهاق. وثانياً نوتروبينيا معناتها الخلايا البيضاء بتكون ناقصة فلما بتكون ناقصة بتسبب الـ infection العدوى وثالثة هي الصفائح the platelets بتكون ترومبوسيتوبينيا فنقص الصفائح بيؤدي للنزيف Classic sign, يعني علامة بنشوفها لما when when examine the patient is when you press in the chest, the sternum, هيد العظم. When you press, it's tender. فبيكون في it's painful when you press it. Okay. So that's why 20% is important to differentiate between acute leukemia or not type. Another thing is, what's the difference between acute lymphoid leukemia and acute myeloid leukemia? Okay, how do we differentiate? What's difference between them? The difference between those إنه بتشوف شو السلالة اللي affected by this disease. Is it the هدو كلهم خلايا بيضاء white blood cells. ف when you have السلالة الميالويد lineage versus lymphoid lineage. So hour rods on smear لما بنشوفها بلطاحة الدم and it is myeloperoxidase Positive. ف myeloperoxidase is an enzyme. فلما بتكون myeloperoxidase we know it's myeloid lineage. لما myeloperoxidase negative it's lymphoid lineage. But also children. بيصيب الأطفال أيضاً والـ cytogenetics التغيرات الوراثية للخلايا بالـ DNA cytogenetics فيه translocation 15-17 ولا translocation 8-21 ولا inversion of chromosome 16 على العكس لما بنحكي على الـ lymphoid الـ lymphoid بيكون the lineages lymphoid مفيش قطبان أو rods الenzyme المايلوبروكسيدايس سلبي negative and التوزع لل to the age في العمر بيكون فيه توزع تنائي الذروة الأولى بتكون بين عمر سنتين وخمسة سنوات ولا بعد ستة وخمسين سنة فهذول بنسميها by model. By model معناته إنه في two peaks. The most majority of patients are either children below five years old or adults above sixty-five years old. And what is the risk of the acute lymphoid So we risk stratify by white blood cell count and by the age Now we're gonna have a question. Let's see if we can. Answer: 34-year-old presents with fatigue and easy bruising. Okay, so when you have fatigue and easy bruising for 34 years old, you have to think that there might be something wrong with the components of their blood. What is the most accurate way to classify؟ Is it myelodysplastic syndrome? Is it acute myeloid leukemia? Is it chronic myeloid leukemia in the accelerated phase? Or is it just reactive marrow change? We should just repeat the biopsy in 3 months. الاولى هي myelodysplastic syndrome. التانية هي الـ acute myeloid leukemia، الثالثة هي chronic myeloid leukemia بس المرحلة المتسارعة، والرابعة هي تغيير تفاعلي في نخاع العظم just reactive لأنه they just had a problem infection ولا أي شغلة اللي بتعمل reactive. Five second pause. The right answer is twenty percent is the line. ف it's correct B which is acute myeloid leukemia. B and that's because بس الإجابة A خاطئة لأنه المايل ميالوديسبلاستيك سيندروم بنتعرف عليه لما بيكون أقل من عشرين بالمية مع some dysplasia خلل في التنسج بنسميه ال dysplasia and the patient that we have is over twenty percent line. الإجابة سي خاطئة لأنه ممكن تكون chronic myeloid leukemia with accelerated phase and blast crisis تكون it looks similar بس ما أعطونا history of other symptoms for years before ولا cytogenetic BCR ABL. فلو ما في ما في تاريخ لمرحلة مزمنة سابقة ولا دليل على ال B C R A B L. و number D is wrong لأنه twenty four blasts with our rods is not a reactive finding. Those are blasts. Blasts are very very important to notice. They should not lose their ability to mature. So this needs treatment now. You cannot wait three months. Okay? So moral of the story, الخلاصة إنه قطبان ال hour rods are the acute myeloid fingerprint. Okay? Type. Now let's go to the next slide, and the next slide is we will talk about The ladder for leukemia. فأولاً every leukemia sits in two axes: acute or chronic. هذه تابعة ل speed of multiplication. Okay. الثانية هي myeloid or lymphoid. سلالة lineage. Once you place leukemia on this ladder, most of the behavior becomes Predictable فبمجرد إنه بنحدد مكان اللوكيميا على هذا السلم بيصبح معظم سلوك متوقع acute and myeloid acute myeloid leukemia fast and immature myeloid blasts acute and lymphoid fast and immature lymphoid blasts لما بتكون chronic مزمن مع الميلويد slow mature Looking, ناجة, myeloid cells. والchronic and lymphoid which is CLL slow and mature looking lymphocytes. Let's talk about all this. Why is this lymphoma also leukemia? فلما بنشوف إنه if a disease starts in a lymph node, why would you ever call it leukemia, which is the blood? Lymphoma is in the lymph node. Leukemia is in the blood. Well, because it's the same malignant cell. نفس الخلايا الخبيثة، either بتعيش بال lymph node في العقدة اللمفاوية or it spills into the blood لما بتنتشر في الدم وبنسميها leukemia. The name describes the location, not a different disease. They are a snapshot of where the cancer cell happens to be sitting at the diagnosis. So It's one spectrum. Think of it as one line from lives in the node or lives in the blood. The same cell of origin can present anywhere. The same cell of origin can present along this line and gets a different name depending on where it's caught. SLL is mostly in nodes. CLL and mostly in blood. Acute T-cell leukemia or lymphoma. ATLL اللي هي acute T-cell leukemia or acute T-cell lymphoma. Same HTLV-1 driven cell, that's named by the presentation. Hairy cell leukemia. Technically, it is lymphoid. هي ال hairy cell leukemia هي من ناحية تقنية نفس ال ال الورم، بس it behaves like leukemia. Okay? Let's do a quick quiz. Which is it? Okay؟ Massive lymphadenopathy. With blood counts that are normal, فالعقد اللمفوية تضخمت يعني بشدة and بس تعداد الدم طبيعي. الثانية فيها sky high WBC ارتفاع عائل كريات الدم البيضاء smudge cells on the smear smudge cell هي خلايا مهروسة. في اللطاخة و minimal nodes ما في تدخم بسيط خفيف للعقد اللمفاوية والتالتة in both both lymphadenopathy and high white blood cell with the same clonal cell same lineage فتدخم واضح في العقد اللمفاوية مع ارتفاع كريات الدم البيضاء so let's match it With what we think, node dominance, blood dominance, or both. Number one, massive lymphadenopathy, but the blood count is normal. Node dominance. Number two, high white blood cell count, but the nodes are minimal. Blood dominance. Number three, both. Many patients can show both at different times. So CLL and SLL are the same disease wearing different outfits depending on where you look. Okay. Hodgkin lymphoma is real classic. 15 times 2 equals 32, and that links CD15, CD30, and the bimodal age peaks: 15-35 years old and 55+ years old. فا الـ CD هو ريسابتر بيكون على الخلية وفي منهم أعداد كتير فا في مرات ممكن نفرق بين هالخلية وهالخلية لو اكتشفنا شو الـ CD اللي عليها ف this why CDs are very important for white blood cell count. When we talk about Hodgkin lymphoma, Hodgkin lymphoma بيكون عنده ذروتين اللي حكينا عنهم للعمر between بين خمستاشر وخمسة وثلاثين سنة وبعد عمر خمسة وخمسين سنة والشغلة الثانية إنه CD fifteen إلى CD thirty and في لما بنسميها خلية الريد ستيرنبرغ. Very important to remember. Reed Sternberg cell is CD15 and CD30. 15 times 2 is 30. كمان فيه المظهر الثاني هو ثناء النوات by nucleate appearance nucleus two ثناء النوات وبي بيكون عليه مظهر عينين البوما for owl eye or by nucleate Appearance. Those eyes are what gave the mark its second half. Okay, kin, Hajj kin. Kin معناته twins, like two things, two two kins. Okay, الذروة الثنائية لل العمر and the virus اللي اسمه Epstein Barr virus EBV. It half the cases. ما بنعرف is the EBV the cause or just associated? We don't know, but it is at least present. Let's talk about the Hodgkin's. This is not why they called it Hodgkin, but You know we use HIV increases the risk of Hodgkin. Okay؟ الحاجة الثانية إنه O is owl eye. So how will I read Sternberg cell? Often starting in the mediastinum. The mediastinum هو space between the lungs. فا it can it can move from one node to the other in order. Non-Hodgkin lymphoma skips. Okay. K معناته kidney can trigger minimal change disease. Minimal change disease is not English, is actually the name of a diagnosis of the kidney. How does it manifest? And S is secondary amyloidosis. بنشوف فيه secondary amyloidosis, which is due to AA protein, which is amyloid. فهذول بيساعدوا على التذكر of all of these features for Hodgkin lymphoma في هذه السمات اللي بنشوفها بال Hodgkin. أوكي شو الفرق between Hodgkin versus non Hodgkin؟ 28 year old presents with a painless enlarging neck mass drenching night sweats والعرق غزير كتير for drench night sweats and فقدان الوزن twelve percent weight loss over four months and the biopsy shows large binucleates with prominent nuclei in a background of reactive lymphocytes and eosinophils. ف البيوبسي فيها تنائية النوات والنويات واضحة كتير prominent nuclei والخلفية ال background فيها خلايا ليمفاوية تفاعلية ال lymphocytes وال eosinophils as well okay which immunophenotype do you expect 5 seconds. A, CD15+, CD30+, CD45 negative. B, CD20+, CD10+, MYC rearrangements. C, CD5+, CD23+ with smudge cells. Or D, TDT plus or CD19 lymphoblasts. والإجابة الصحيحة هي fifteen times two nucleus is thirty ليتذكر الواحد. The correct answer is classic Reed Sternberg immunophenotype with B symptoms. The B symptoms هو ال weight loss and the Night sweats هدول بيتسموا B symptoms. It fits the classic Hodgkin lymphoma. Okay؟ We see it a lot in Africa Very aggressive, very bad وال C is wrong لأنه is CD five plus and CD twenty three plus هدول مع smudge cell they describe CLL. CLL is not node dominant it's blood dominant chronic leukemia. D is wrong لأنه فيه DTD plus TDT plus and CD nineteen it describes B lymphoblastic leukemia and lymphoma. And it's seen in a much younger population. Okay؟ Okay. Then we will do the next one, which is Disease passports نحكي على CML والAPL والبركت ليمفوما لسات نحكي عنهم بس we will anchor them we will talk about them more in detail. أولاً الtranslocation اللي اسمها nine and twenty two، the chromosome nine and chromosome two، they exchanged something. We call it translocation، تبادل ولا انتقال. It's very important to remember this because it's going to be very high in the exam. Okay, how do we treat it? We treat the CML with imatinib, which is a tyrosine kinase inhibitor. مع الإماتينيب there is a ten-year survival that goes from twenty percent up to eighty to ninety percent. أوكي كيف نتعرف عليه how do we recognize it we see that it is chronic مرحلة مزمنة في تدخم هائل في التحال Huge spleen, and the white blood cell count is very high. That's why we call it chronically. أن لو كانت acute we would see that it was blasts not mature مو ناجة بس الكرونيك بيكون فيها full maturation spectrum الماتشور والبلاست كلهم بن بن بنشوفوهم. The next one is called APL. 15 and 17, and the result is called PML-RARA. يتغير فيه إنه we وقع a translocation between those two, and that affected مراحل النجش لل ل المايولويد لينيج. how do we manage it? well, we manage it نستعمل الأترا والأرسينيك تريوكسيد. it's not standard chemo alone. it's very specialized. and because it is acute مو مزمنة حادة لازم نبتدي treatment عتو. ف how do you recognize it؟ Multiplication of all of these myeloid cells very quickly. Many of them get destroyed very quickly, and you cause DIC at presentation. DIC is disseminated intravascular coagulation. 14 and it creates what we call MYCIGH فلما تشوفوا الـ MYCIGH تتعرفوا على البركت ليمفوما and how كيف بنعالجه العلاج كيميائي إنتنسيف مكثف وقصير المدة. and you watch for all of the tumor is going to melt. يعني ذوبان بتشوف الورم يعني بيصغر. and as you destroy it, all the enzymes in the cells they spill in the blood and it causes the person to be sick from the treatment not from the cancer. We call that tumor lysis syndrome. How do you recognize it on the histology? You see multiple of these, and we call it starry sky. Starry sky histology. It's linked with EBV, as زي ما قلنا, in endemic form. فين مكان أمكن فيها الEBV مكثف in the population, and Burkitt lymphoma extremely, extremely موشدي جدا fast growing. Let's talk about CML. CML is the best teaching example of precision medicine. هلأ ال oncology is moving into زي infectious disease. لما تكون عندك infection we do Body fluids, and we send them for microbiology. And depending on what we find, we prescribe the very precise antibiotic. Same thing now for oncology. If we can send the biopsy and we can study something specific, we can give something specific for it. And CML is one of those that actually benefits from it. The CML, الحكينا عنا, translocation is between 9 and 22. Or 34 and 11. فقو is the short or long arm of the chromosome. بس it's not unless you are going to be an oncologist. It's not something that I remember always. But I would say that the BCR gene in chromosome 22 fuses with the ABL1 gene. In chromosome nine, and they create Philadelphia chromosome. The fusion protein, which is a tyrosine kinase, is stuck on on position. عناتو إنه when we fuse these two together, it's almost as if you turned the light on. Okay. فإيماتينيب الدواء it blocks the ATP binding sites of that kinase. ATP and when you give that you can prolong the survival of the person CML So we are coming to close this session, and we're going to talk about this patient as a conclusion. A 45-year-old man presents with fatigue, very high white blood cell count, 180,000 cells per microliter. شو ال ال العادي normal For the white blood cell count, six to ten thousand. He has one hundred and eighty thousand with a full range of myeloid cells. Every stage of maturation you see it's on the smear: blasts, promyelocytes, myelocytes, and mature neutrophils. The spleen is massively enlarged. ف هزلمي خمسة وأربعين سنة يعاني من الإرهاق وعنده ارتفاع شديد كتير من عدد الكريات الدم البيضاء مية وثمانين ألف والنورمال هو ستة لعشر تلاف مية وثمانين ألف that's wow فلما بنشوف وشوفنا اللطاقة الدم فشوفنا فيها المجموعة كاملة من الخلايا النخاعية في جميع مراحل النضج البلاست الأورمات ولا الخلايا قبل النخاعية ولا الخلايا النخاعية ولا العدلات الناضجة mature neutrophils والطحال متضخم بدرجة هائلة massively enlarged ف the stage one what is the most likely diagnosis and what is the genetic test to confirm it How do we know? Well, أولاً شفنا إنه في الدم وحتى في ال ال we don't see a lot of adenopathies ما شفناش العقد اللمفاوية so it's mainly leukemia okay بس هي مزمنة لأنه فيها fatigue And all the stages of maturation, so it's chronic CML. How do we confirm? We do the genetic testing and we find Philadelphia chromosome. Stage two, six months later repeat the bone marrow. We're not going to just say you have CML that's it thank you. No, the follow up ست شهور بعديها نعمل bone marrow بس this time شفنا في bone marrow خمسة وعشرين percent blasts. What has happened? And do we change the treatment? Of course. Okay we have to go with chemotherapy even slow chronic leukemia Can cross تعبر خط العشرين بالمية and becomes acute. The rules don't change, but the disease became different. Okay. Twenty percent blasts هي the line between acute and everything else. Number two. AML versus ALL, acute myeloid, acute lymphoid. Acute myeloid هي our rods مع الميلوبروكسيداز هادي myeloid. TDT positive هي lymphoid. Number three, lymphoma and leukemia. It's the same thing, just location, not a different disease. Hodgkin CD15 plus CD30 plus our eye cells. Contiguous spread and B symptoms. And آخرها three translocations that change management: BCR-ABL1, CML; PML-RARA, that's APL, acute promyelocytic leukemia or acute myelocytic leukemia M3; and MIC, El Burkitt's. Okay وإن شاء الله يكون عنا الموديل الثاني and we will go with it and until then الله معكم.
