السلام عليكم ورحمة الله وبركاته بتمنى إنه تكونوا بخير وإن الفورمات الجديد لللكشن بتوافق معكم ويعجبكم and this is the third part of the hematology and oncology. So what we are going to be discussing is something that was very difficult for me, which is the smears. They come in exams. They always seem to be confusing. So insha'Allah, بعد هالحصة، and ممكن إنكن ترجعوا لها بعدين مرات عديدة لأنه you will find a lot of good stuff in it. Hopefully after this you will feel much more ready for The slides and the smears that we see. What we will be talking about is the blood smear as well as pharmacology. هدول أسئلة ادتهم لي بالWhatsApp and I hope إن شاء الله تكون موافقة مع Your expectations. طيب، the smear is because sometimes the patients don't tell us anything. ف a patient can hide symptoms, forgets history, downplays pain, but the red cell can't. So the shape of the red cell is almost like a written record of exactly what damaged it: mechanical shearing, ولا يكون فيها كمان يعني الغشاء انفقد ولا تمزق ميكانيكي ولا كمان هجوم تأكسي وهو oxidative attack and sometimes even the bone marrow إنه خال عضمي تحت ضغط شديد under a lot of attack. By the end of this module. You will be able to read this record in under two seconds, insha'Allah. ف أولاً look at any smear image before the label appears. What is your instinct, your gut telling you? Is it a red blood cell problem? A white blood cell problem or platelets problem. And today we will cover all three. Each one has its own logic. So خلاصة الأمر إنه المورفولوجيا the morphology هي معلومة تنحفظ مع فهم لأنه فهمة هي آلية بتقدر الواحد يعيد بناءها مرة تانية بس لو كان just memorizing you will forget. Before we go anywhere خلينا نتذكر شو is how is this hematopoiesis؟ احنا كلمنا في الموديل number two على سلالات الخلايا الدم فتبتدي من stem cell The origin cell, and it can go into lymphoid or myeloid. Lymphoid, هذي سهلة بتروح على lymphoid stem cell, and it's become lymphoblast لساتمان أجا فهذول البلاست اللي حكينا عنهم في الموديل تو. And then from lymphoblast, it differentiates. It differentiates into B lymphocyte, T lymphocyte ولا natural killer. On the other side, هدول بيعطينا ميكاكاريوبلاست، which turns into ميكاكاريوسايت. Blast and cyte معناته نضجت. And then ميكاكاريوسايتز they turn into platelets الصفائح. Or it can go into pro. Pro means early. Pro erythroblast. ومن الpro erythroblast we go into reticulocytes. Or we go into myeloblast. El myeloblast then differentiates into eosinophils, neutrophils, or basophils. Or we go from myeloid stem cell into monoblast. The platelets, erythrocytes, eosinophils, neutrophils, basophils. Monocytes, B lymphocytes, T lymphocytes, and natural killer cells. There are three ways a red cell tells us its story. Learn the families and you will stop memorizing the shapes one by one. هدول الفamilies بتشوف هل الغشاء انفقد ولا ضرر ولا إنعاد تشكيله فلو الغشاء إن فقد membrane is lost معناتها a piece was bitten off or pinched في نعض ولا نقرس جزء من الغشاء هذول membranes they become either spherocytes كروية Or bite cells. Membrane is damaged by shear forces in a small vessel. If the car goes into a street that is narrow, it starts to be damaged. Then we have schistocytes or teardrop cells. Okay. والfamily the third family لو الغشاء انعاد تشكيله بسبب ضغط أو أو مرض مزمن في في واحد من الأعضاء فهي membrane that is remodeled with chronic metabolic or organ stress. Then, تدينا إكينوسايتز لو الكيدني is damaged. Target cells لو في تلاسيميا or liver problem. So ask yourself first: Was something removed? Was something torn? Or was something reshaped? We start with these four. Now you have anxiety. بس وحدة واحدة احنا نشوف الوحدة الاثنين ثلاثة والاربعة finding number one finding number two finding number three and finding number four فبنسأل الRBC خلايا الحمراء هل شو السؤال انشال جزء من الخلية ولا تمزقت ولا تغير شكلها فا نمبر ون بنشوف هالخلايا اللي هون هدول تغير شكلن كمان التانية تغير شكلها التالتة هون والرابعة هون so let's find them الأولى you see الشكل تغير And becomes ما في البياض اللي في ال center there is no center white these are erythrocytes but they are round كروية فسفيروسايتز here بتشوف زي ما دمعة فبنسميها tear drop cell or dacryocyte الثالثة هدول الacanthocytes ولا spur cells بتشوفهم هون. فالبر and spur cells when you see them they look similar. راح نتكلم عنهم separately to make sure that we understand them. Okay? What is the mechanism behind each? Spherocyte. Spherocyte. معناته إنه خلايا كروية وهذه الخلايا it's the membrane is lost الغشاء منه هناك فلما الغشاء يكون إن شاء الله، what do you see؟ We have round. Etenia is teardrop cells. معناتها it it means that it squeezed out of an infiltrated marrow one drop at a time. في fibrosis in the marrow called myelofibrosis ولا مرو إنفلتريشن ولا سيفير تلاسيميا فالخلايا بتنضغط ويطالعة من النخاع وهالنخاع فيه متليف ولا مخترق بب باطولوجي كأنها بتطلع قطرة قطرة الثالثة هي الأكنتوسايتس الأكنتوسايت فيهم irregular spiky projections فبيخرجوا منهم هادول spike بس irregular and it shows that the membrane is abnormal and فيها شوكات بتخرج بس غير منتظمة بسبب خلل في تركيب الدهون من الغشاء للخلية severe liver disease ولا A-beta lipoproteinemia البر سيلز اللي مختلفين على سبر سيلز كمان هدول عندهم شوكات ولا نتوآت بس هدول الشوكات صغيرة ومتنظمة uniform and they are evenly spaced معناته space بينهم is the same بنشوفها في اليوريميا renal failure كيف بنفرق بين spur and burst cell؟ spur cell ولا الـ acanthocytes بتشوفهم هون عندهم irregular spikes بس variable length and spacing And this S معناته spiky and sharp. That means the spleen sequestered them. وبتكون في we see it in severe liver disease ولا أي بتة ل lipoproteinemia فالطحال ب بيكسرها مبكراً مشان هيك بتترافع often coexists with hemolysis مع الحلال الدم الbur cells الأشواك هي صغيرة بتشوف هنا الفرق بينهم. والأشواك هون المسافة منتظمة، فهي الأشواك صغيرة ومتساوية والمنتظمة والحرف البي بيذكركم ببلونت and balanced بنشوفها في renal failure and it gets better with dialysis. Okay. Now, which means that we need to remember S is spur cells. فيهم spikes and B is bur cells, which means blunt and balanced. Okay, now let's go to our first question. Thirty-four-year-old woman has jaundice. And splenomegaly and hemoglobin of 9.1. The smear shows numerous small round red cells with no central pallor. The direct Coombs test is negative. Osmotic fragility is increased. What is the most likely diagnosis? The most likely diagnosis is A, warm autoimmune hemolytic anemia. B, hereditary spherocytosis. C, J6PD deficiency. And D, thrombotic thrombocytopenic purpura. Think about this for a little bit. And the answer is hereditary spherocytosis. أولاً في وجود spherocytes without any polar central Pallor. Elcombs test is negative. So it cannot be hemolysis. Increased osmotic fragility is something we see with spherocytes, which points to an inherited membrane defect. Not an antibody-mediated process. لنذكركم على direct combs test. لو أخذنا blood sample with a patient with hemolytic anemia, بيكونوا هالantibodies اللي they caused the hemolysis on the red blood cell. We add to them a few drops of anti-antibody antibody. لو في أنتبيديز، they will capture them. We connect them, and we see that the red blood cells clump together, agglutinate. That's positive Coombs test. If they don't agglutinate means negative Coombs test. So A is wrong. B is wrong لأنه J6PD بتعطينا bite cells and Heinz body زي ما شفنا في الموديل 2 مش السفيروسايت هدول وقع damage للغشاء and D TTP produces schistocytes معناتها بيتمزقوا for mechanical shearing because they go through microangiopathic damage فخلاصة الأمر أنه وجود السفيروسايد دائماً بيخلينا نفكر من خلال الدم بس بنعمل فحص الكومبس اللي بيحدد إنه إذا كان السبب المناعة والمضادات الحيوية ولا وراثية لما يكون الكومبستست سلبي ولا نيجتيف We go to these بنسميهم رولو agglutination and what we see in the bone marrow. So when when that happened, you start to see rouleaux of red blood cells stacked like coins. And that means the red blood cells clumped Positive and that means cold agglutinin ولا direct antibody على الأقل إنه إنه من المناعة ولا من المضادات الحيوية الثالثة اسمها polychromasia polychromasia means you find خلايا الدم حمراء كبيرة وفيها لون أزرق خفيف زي ما بتشوفه هون هدول مش خلايا حمراء هدول قبل الخلايا الحمراء اللي اسمهم ريتيكولوسايت. So young reticulocytes معناته إنه البون مارو is so speedy wanting to release the red blood cells that it releases the young red blood cells as well. So when you see polychromasia, you see nagin red blood cells with the white inside or monagin مهمة وآخر واحدة هم النوكلييتد ربيسيز هدول النوكلييتد ربيسي قبل ما يخرج الاربيسي بتاخد منه النوكليوس and it takes it out خلايا الدم الحمراء للسادة فيها نوات معناتها إن نخاع العظم دفع الخلايا للدم تخرج قبل وقتها أوكي وبنشوف هذي في السفير أنيميا ولا bone marrow infiltration أنه خال عدم بيدخل فيه مرض ولا هيبوكسيا، أوكي؟ We talked about the red blood cells. هلأ we're gonna talk about خلايا الدم البيضاء، white blood cells نفس القواعد لكن هالمرة بالwhite blood cells. فا number one finding number two finding number three finding and number four finding number four number one is في هدول النقط اللي بتشوفهم هون granules هذي بتسمى toxic granule granny toxic granulation toxic granulation فيها حبيبات غامخة وخشنة عند النوتروفيل. بنشوف غرانيولز في النوتروفيلز بس هدول look at how it looks so dark. فبتشير look at the هدول بنسميهم toxic granulation. التانية صورة تانية بنشوف فيها النوتروفيل and you can see granulation خفيفة مش زي number one بس هذي hyper segmented وبتشوف النوكليوس very divided النوات فيها فصوص أكتر من اللي بنعرفه ففيها خيوط رفيعة كمان وبدل العدد الطبيعي اللي عادةً بيكون ثلاثة إلى خمسة فصوص، هدول فصوص أكتر، فبنسميها Hypersegmented Neutrophil. Our Rods. And our rods معناته إن خلية كبيرة شوف النوكليوس للساتو كبير ما في فصول وغير ناجشة وبعدها في ستوبلازم بنشوف القضيب الرفيع فهذا بنسميه our rods and we spoke about it هذا myeloblast with our rods وآخر واحدة بتشوفوا smudge cells ف زي ما تكونت مسحت ولا تفجرت بدون غشاء واضح أو بدون تفاصيل النوات هدول بتشوفوا smudge cells and we will talk about them in just a little bit. The most dangerous one to overinterpret manata to react to it without checking هو number four. Let's talk about them. Toxic granulation, toxic granulation. بتشوفوا فيها فيها حبيبات غامقة coarse dark granules from accelerated ولا disordered maturation, which means that the النج الخلية كان تحت ضغط التهاب ولا عدوى وبنشوفه هذا كتير في bacterial infection and sepsis. الhypersegmented neutrophils هدول بنشوف أكتر من ست lobes فصوص this happens when you have a DNA synthesis تصنيع الDNA بيكون ضعيف ولا impaired. ولما بتشوف ال hour rods هذا is diagnostic بيأكد ال acute myelocytic leukemia زي ما شفنا في module two and we go back to the twenty percent blast line لو في أكثر من twenty percent versus not. We have to make a confirmatory test before we call it. ف let's talk about this. This is one of the most repeated shortcuts that students think. See smudge cell, call it chronic lymphocytic leukemia. No, that's not always true. If you see smudge cells alone, what do you do to confirm the diagnosis? Sometimes just because you make the cell, the smear, and you put a lot of pressure, it smudges the cells. So that is not the patient, that is our hands. So how do we diagnose? CD23 positive in the B cells and plus an absolute lymphocyte count that is more than five thousand sustained over time. You still is just a clue. لو تشوف smudge cell, you have to confirm with flow cytometry before you give the diagnosis of CLL. The acute myeloblastic leukemia or acute lymphocytic leukemia. Leukemia. How do you differentiate between them? المايالوبلاست لما تكون acute myelocytic leukemia بنشوف our rods which are in damage للنقط granules and we see myeloperoxidase positive وبعدين بنشوف إنه الكروماتين is very fine With a very big nuclei, النواد كبيرة وخشنة وبنشوفها أحسن. And more common in older adults. بالمقارنة مع ال lymphoblast and lymphoblast ما في أو الراض أبداً بس إن ما في myeloperoxidase positive لأنه هذه myelo this is lympho ال lympho بنشوف فيها إنه فيه is TDT positive, and we see the shape of the cytoplasm. The hand mirror shaped, we will see it in just a little bit. It's classic in all T. Acute lymphocytic leukemia. وفي ذروات ذروتين اثنين peak age two to five or second peak more than sixty five years old. So this is the difference again between myeloblast and lymphoblast. On the myeloblast, you have more cytoplasm. The granules azurophilic may be present. The nucleus في Fine chromatin, and the nucleolus is very prominent. It comes from myeloid lineage. And on the other side you have the lymphoblast. The lymphoblast is you have small amounts of cytoplasm, no granules in the cytoplasm. Nucleus is very coarse. He has twenty-six-year-old male with three days of fever and sore throat who has a smear showing numerous large lymphocytes with abundant pale blue cytoplasm that appears to be hugging Surrounding red cells What's the most likely cause of this finding? Bacterial pneumonia with toxic granulation, infectious mononucleosis. Acute lymphoblastic leukemia or chronic lymphocytic leukemia. فحكينا على الchronic lymphocytic leukemia with the BCR and ABL Philadelphia chromosome. حكينا على toxic granulation how it shows. So this is infectious mononucleosis. And now قدمت لكم شغلة تانية باسمير اسمها reactive lymphocytes ولا atypical lymphocytes. A is wrong حكينا عنه لأن ما شفنا الـ granules وبعدين اللي شفنا أنه lymphocyte مو neutrophil. ودي is wrong لأنه normally lymphocytes are small when they are not gene وكمالين with smudge cell not atypical. CLL does not pretend present with fever and sore throats. فخلاصة الأمر إنه ال atypical lymphocytes المورفولوجي زائد an acute febrile illness in a young adult بتخلينا نفكر في مرض حموي حاد اللي هو ال EBV قبل ما نخمم منفكر في اللوكيميا. هلأ نحكي على ال hand mirror clue طيب ال our rod is a needle shape بتشوفوا فيه كمان كتير من ال our rods هون بس بتكون inside the blast and that's because all the azurophilic granules اندمجوا مع بعضهم okay فهذول الإبرات الإبر مكونين من دماج الحبيبات الأزروفيليا وخاص بالسلالة النخاعية اللي هي المايلويد only it's never seen in lymphoid بس الour rods لما تشوفوه always think acute myeloblastic leukemia الour rods are the weapon you see it's the lineage question is not A question. It is myeloid. Okay? فهذول لما بتشوفوهم الـ flow cytometry you can do it to confirm the lineage سلالة. Platelets normally are small, two to three micrometer. اللي بنشوفه هو giant platelets is one, micro platelets is two, platelets اللي clumped together number three, and we will talk about these three things. أولاً this is how a Normal platelets is. بس giant شوفوا how this is big and just large. It's big but not too big. Why do we make the difference between them? Giant platelets is larger than a red blood cell and is seen in مرض اسمه bernardsulie ولا idiopathic thrombocytic Purpura thrombocytopenic purpura. Aldrich syndrome. With delta, لما بنشوف platelets clumping, and platelets clumping معناته إنه antibodies clump the platelets into the tube. This is how it looks like. So a healthy 40-year-old Comes for routine pre-op checkup, and the CBC shows platelets 260,000. What's the norm? 150,000. So platelets نقصين. There's no bleed, no bruising, and a completely normal exam. When you see the smear, the What is the next best step? Do we treat for idiopathic thrombocytopenic purpura? Do we repeat the platelet count بس دلوقتي بن add to it citrate blue top? Do we give platelets transfuse? ولا do we order bone marrow biopsy؟ And the answer is لما بتشوف platelets clumping, that's when three or more platelets clump together. It can give you a false خطأ, false low platelets. Usually you repeat the lab test with the citrate sample to correct the clumping and provide a more accurate platelet count. And typically it's not clinically significant. There's no problem with it. So. Why is A wrong? ITP. Number C is not because if we transfuse platelets, we expose the patient to unnecessary transfusion. Okay? وآخر واحدة bone marrow biopsy is big. Biopsy is big. We need to verify the results before we do anything else. So a low platelet count with a normal exam and clumping on the smear—it's an artifact. Okay, until you prove otherwise. فنعمل نقطة مراجعة هالجزء لما يكون فقدان الغشاء membrane is lost, spherocytes or bite cells. ولما يكون إعادة تشكيل الغشاء, remodeled, spur cells, burr cells or target cells. How do you differentiate spur and burr cells? Spur is irregular spikes. Burr هذول الأشواك منتظمة ومقاييس بناتها نفسها and uniform spikes with same distance between them. We see it in kidney disease or it's an artifact. Okay, our rods always myeloid, hand-shaped mirror always lymphoblasts. When you see smudged cells It suggests to you. It suggests to you. إنه تقترح إنه chronic lymphocytic leukemia بس قبل ما تكون diagnosis on the smear, you have to do flow cytometry to confirm it. وآخر واحدة إنه platelet clumping on a smear with normal exam, you have to repeat in a citrate tube before you think the platelets are low. Is something important, which is methotrexate. A doctor gives high dose methotrexate without leucovorin rescue on schedule. That was supposed to cure the patient starts to destroy their bone marrow. How does it do it? What is leucovorin, and why does the timing matter so much? Leukovorin is the name of folic acid, folinic acid, and it bypasses the enzyme that methotrexate blocks. We're going to talk about how it works and why do we give leukovorin so it doesn't it lessens the side effects coming from it. So how does the folic acid work? The folic acid here, we show for inu, the folic acid is metabolized into at the bottom dihydrofolates using what? Dihydrofolate reductase. So dihydrofolate reductase takes folic acid and makes it into dihydrofolates and makes it into So it reestablishes amino acid metabolism and it makes the membranes and everything else continue while the methotrexate only focuses on this. So you can see. رح نرجع على الماتوتراكسيت in just a little bit. Okay? We will go into anticoagulants. The anticoagulants. There are four drugs: filheparin, Kamla, unfractionated; filheparin, unfractionated, or low molecular weight heparin, or warfarin, or DOACs, direct oral anticoagulants, DOACs. The heparin. How do we coagulate? What causes the tachatur? Tachatur beikun, lamaikun. We make fibrin. Fibrinogen bi biyisbah fibrin. Thrombin. Thrombin بتساعد الفيبرينجين ليكون fibrin and creates All of التخطر الدموي coagulation. Dijil heparin, and it's called antithrombin. فبتوقف thrombin ولما بتوقف thrombin معناتها بتوقف fibrinogen to fibrin so we stop coagulation. A low molecular weight it's happen it activates the antithrombin like heparin does. But it's too small to bind to thrombin directly. So what it does is it inhibits factor 10a. It blocks the vitamin K. والرابعة هي direct oral anticoagulants. It either directly inhibits thrombin, directly inhibits factor 10a. I wanted to give you a reminder of the coagulation cascade. On the left, مش شمال عندنا intrinsic pathway. Okay? The intrinsic pathway بيتنشط is activated with contact, contact of damaged surface. لما يكون عنا damaged surface of the blood vessel, الفاكتور twelve بتنشط للفاكتور twelve a. فاكتور twelve a بنشط الفاكتور eleven into eleven a. فاكتور eleven a بنشط فاكتور nine into factor nine a. فاكتور nine a بنشط فاكتور Ten into ten A. والفاكتور ten A بين نشط البروترومبين لترومبين. That's how we make a clot. Intrinsic pathway. Extrinsic pathway معناته إنه something Is circulating, ولا like trauma or injection of something in the blood. So what happens is this activates بنشت الفاكتور eight للten لل eight a. The factor eight a بنشت the factor ten into ten a, and then we go into the common pathway ten a. Activates prothrombin للpro للthrombin والthrombin activates fibrinogen للfibrin. فهذول الاثنين each one of them is a pathway. And now we're going to talk about مضادات تخطر. الفيتامين ك بتساعد الفاكتر ايت وتيشيو فاكتر على activating factor ten from the extrinsic pathway. فالورفرين لما بنديها it takes away the vitamin K which means it inhibits factors eight tissue factor etc. Whether it's unfractionated heparin or low molecular weight heparin or fondaparinux, هدول بيتسموا antithrombin. 10A and they stop prothrombin from making thrombin. So it's an antithrombin. وآخر واحدة اللي هي direct oral anticoagulant اللي pills اللي منعرفهم هلأ اسمه إدوكسابان، أبيكسابان، بتريكسابان، ريفور، إيفاروكسابان هدول كلون direct anti direct factor ten a inhibitors بيوقفوا نشاط ten a فبيجوا بعد هال pathway كلون so no matter what happens they stop the ten a which means they stop prothrombin to thrombin و they stop thrombin from making Fibrin. Okay. And dabigatran is stopping the thrombin 2A directly, antithrombin directly. I put that summary. The contact pathway or intrinsic pathway starts at factor 12. Tissue factor pathway or extrinsic pathway starts at factor 8. Pardon me, factor seven. I'm sorry. It's V plus two. That's factor seven. Tissue factor pathway starts at factor seven. Will common pathway turn fibrinogen into a fibrin clot? The drug types warfarin blocks factor two, factor seven. Factor nine and factor ten. The DOACs direct oral anticoagulants. They block factor ten A or thrombin directly. While heparin and unfractionated heparin, low molecular weight heparin, is use antithrombin to block the factor ten A and thrombin. When we want to send تحليل طبيعي لنشوف للورفرين الورفرين is the extrinsic pathway. So this is the PT Or if we talk about something that affects intrinsic pathway, we do PTT. So let's talk about this. لما بنقول الورفرين الآلية هي إن it blocks the vitamin K epoxide reductase، ف it reduces factor two, factor seven, factor nine, factor ten and protein C. بس أكثر الهلف لل العوامل and كيف نعمل المراقبة ال management monitoring بال INR ولا ال PT ال INR is important because it's extrinsic pathway okay وال interactions التداخلات عندها ال warfarin كتير interactions multiple drugs food antibiotics amiodarone Aspirin, vitamin K rich greens. If you eat something that that has a salad with vitamin K, it will interact with warfarin. And if you have warfarin, how do you reverse the warfarin? You تعكس الآلية تبع الورفرين by giving vitamin K. بس كمان إما بنعطي fresh frozen plasma FFP or nowadays بنعطي ال four factor PCC. ف if you have somebody bleeding and they were on warfarin you want to replace by giving factor four vitamin K and Four factor PCC. What about the DOACs? Direct oral anticoagulants. The DOACs هم أحدث the newer أحدث مضادات للتخطر الدموي. فتأثير سريع بداية تأثير سريع faster onset but reversal requires the right antidote so the mechanism is direct thrombin factor 2a inhibitor you can go back and see direct factor um dox 10a And reversal agent اسمه الاداروسيزوماب. So, اداروسيزوماب is a monoclonal antibody. M A B is monoclonal antibody? Any medication that has MAB هي monoclonal antibody. So idarucizumab, immediate reversal, ladabiratran. And it has— it needs a kidney. So if your kidney is not working well, you have to dose adjust. El rivaroxaban, el apixaban, hadul direct 10A inhibitors. 10A inhibitors, the reversal agent is something similar to 10A. Isma? So اندكسنت ألفا that's a reversal لو في واحد عنده ريفاروكسابان and they have a head bleed ولا they have GI bleed to reverse you give them اندكسنت ألفا. Okay? And reversal is vitamin K with four-factor PCC. شو هدول ال four factors two, seven, nine, and ten. We always give it to mechanical heart valves or valvular atrial fibrillation, and there are multiple food and drug interactions. Direct anticoagulants dox. No routine monitoring. They are predictable. We do not need These are preferred for non-valvular AFib and pulmonary embolism, blood clots to the lungs, blood clots in the legs. They have very few interactions, but you have to dose adjust in kidney problems. Let's see if we can test ourselves. Seventy-year-old with mechanical mitral valves. Is currently on warfarin with a therapeutic INR. Her son, a medical student, asked if she should switch to direct oral anticoagulants. Yes, direct oral anticoagulants are safe and effective for all indications requiring anticoagulation. No, direct anticoagulants are contraindicated in mechanical heart valves. She should remain on warfarin. Yes, but only if the INR becomes difficult to control. Or it doesn't matter. Both drugs work identically. What do you think? دراج بي بي معناته إنه دواكس are contraindicated in mechanical heart valves. They're not. They're contraindicated. Okay? A non-valvular atrial fibrillation and VTE. والسي إز رونج لأنه each one of the drugs acts differently عندهم آليات مختلفة تستهدف نقطة مختلفة للـ cascade. aspirin, clopidogrel and absciximab like GP2B3A inhibitors. Irreversibly inhibits COX-1 and it blocks the thromboxane A2 production for the lifespan of the platelets P2Y12 ADP ريسبتور is blocked. Okay? معناته إنه you need you don't you cannot bring the platelets together. Make sure that the person who we're going to give clopidogrel to we test their CYP2C19. Activation. لو كان عندهم ال unfortunately they will not respond to clopidogrel زي ما بتقول resistance والتالتة G2P2B GP2B3A glycoprotein 2B3A inhibitor زي ما كلمنا على أبسيكسيماب هذا بيبلك بيوقف بيمنع ارتباط الفايبرينوجين الفايبرينجين تو فايبرين الارتباط للبايندينج تو ألاو فور بلايتليتز تو أغرغيت هذا بيمنعه. so I wanted you to know the mechanism again. فبندي أسبيرين to block the inhibited to inhibit the platelet activation and aggregation. Clopidogrel بيشتغل على ال P2Y12 receptor ولما بيوقف بشكل غير معكوس which is irreversible when you irreversibly stop the ADP from binding to the receptor you stop platelets activation and aggregation. تايكاغريلور has a reversible receptor blockade of the ADP, which causes the same thing, which is platelet activation and aggregation. Okay? أوكي نرجع على الكيمو حكينا على الكيمو بس only with folic acid now let's talk about the chemopharmacology اللي قلتولي عليها إنه بتدوخكم وصعبة كتير ليتذكر الواحد ففي أربعة عائلات من الآليات اللي بيكون فيها أدوية العلاج الكيماوي. Alkalating agents, antimetabolites, spindle poisons and anthracyclines. Okay, so cyclophosphamide is an alkylating agent, damages the DNA directly. It's it gives a toxic metabolite. And it can be Okay؟ So they they mimic the normal building blocks and they block the DNA synthesis. El methotrexate بيتعكس باللوقوفورين. Will six melt captopurine unfortunately interact with allopurinol? You have to remember this. بالvincristine وبالvinblastine they insert themselves and they block the microtubules. All of the tubules, هاد القضبان اللي بيكونوا they they form the شكل تشكيلة الخلية they block that assembly. Okay? The paklitaxel block the microtubules disassembly. فالمايكروتوبول لما بيكون بينبني هدول الفينك ألكالويدز المايكروتوبيول لما بيكون بينهدم عشان يبنوا واحد تاني الباكليتاكسل ستوبزت فبيكون المبنى مو سوليد. so opposite mechanism same target هدول spindle poison وآخر واحدة هي الأنتراسايكلينز الأنتراسايكلين بتدخل في DNA وكمان بتولد free radicals and free radicals they cause oxidative stress and destroy the cell from within. Okay, so the more you put the anthracycline, the more بينزاد على بعضه and it causes damage and kills the tumor. But it can cause damage to the heart. So I wanted you to know this. Let's talk a little bit more about it. The more you talk about it, the more you remember it. Alkalating agents, direct DNA damage. تاني ال antimetabolites. It it تعمل شكلة على أنه إن مكونات البناء الطبيعية and blocks the DNA synthesis. Metotrexate is reversed by leucovorin, as we saw earlier, and 6-MP interacts with allopurinol. Spindle poison thiomethanine. The vinca alkaloids block the assembly البناء للقطبان, the microtubule. The taxanes block the disassembly الهدم. لاتتذكروا الفينكريستين كريسبس النورس، so it causes neuropathy الفينبلاستين بلاستس، explodes، the blast bone marrow، so myelosuppression Is the problem with vinblastine? الاثنين they block the microtubule assembly during the mitosis when the cell is trying to subdivide. Okay. Elvin Kristin tiji ma cocktail combination la chop ABVD C H O P A B V Vin Kristin. Specifically because the toxicity profile does not overlap with other bone marrow suppressing partners. Elvin Kristin and Elvin Blastin and the bone marrow. So let's talk a little bit. This is a question to see if we can help ourselves. If you need to stop, طيب السؤال هو مريض بياخد a patient on maintenance six mercaptopurine. And let's go back to see mercaptopurine is antimetabolite six MP. They're taking six mercaptopurine for acute lymphoblastic leukemia. And the person starts them on allopurinol because of gout, uric acid, while keeping the six mercaptopurine dose. No adjustment is needed. The drugs don't interact. B reduce the sex mercaptopurine dose significantly لأنه الألوبيرونول blocks its breakdown. Number three, increase six mercaptopurine to maintain efficacy و number four, stop allopurinol permanently. It's never safe with any chemotherapy. عندكم ها reduced six mercaptopurine لأنه ندي six mercaptopurine and because the tumor is starting to be destroyed, the DNA starts to be destroyed, and now you have more uric acid. ندي له الالوبرينول بس الالوبرينول interacts with the breakdown of six mercaptopurine شو معناتها؟ معناتها إنه six mercaptopurine will have higher dose now that it's not metabolized. Okay. So, allopurinol inhibits xanthine oxidase. It's the enzyme that breaks down 6-mercaptopurine without a major dose reduction, typically to 25% to 33% of the original dose. The patient develops severe life-threatening مايلو سبراشن. Okay. A is wrong. لأن one of the most dangerous drug interactions in hemopharmacology is allopurinol and six mercaptopurine. You have to know it. C is wrong. لأن increasing the dose will make it more problem. D is wrong. لأن allopurinol must be used when it's needed. It's not an absolute contraindication. A patient stable on 6-mercaptopurine for years is admitted for an unrelated Gout flare. The covering intern continues the home six MP and starts allopurinol at full dose without checking for interactions. What did the intern miss and what happens next if uncorrected? ف six mercaptopurine dose need major reduction the moment allopurinol was added regardless of how long the patient has tolerated it. So sometimes we give ف for example في erythropoietin هيدي بت to stimulate RBC production بتحفز إنتاج خلايا الدم الحمراء تستخدم في الكيموثرابي induced ولا حتى renal failure induced anemia okay you have to use it cautiously when you have active cancer الثانية هي بstimulate بتحفز إنتاج ال الخلايا البيضاء نوتروفيلز اسمها فيلغراستيم or GCSF. فيلغراستيم stimulates neutrophils production, but the timing matters. Avoid giving it in the first 24 hours immediately before or after chemo to prevent immature chemo vulnerable cells. شو كمان حكينا على الفوليكاسد رح نحكي كمان عليهم وهم هدول علاجات نقص مكونات الدم فالحديد والبيت والف والفوليت كل واحد منهم بيعالج نوع معين من فقر الدم لكن لكل واحد فيهم فخ مهم في الممارسة وهي each of these have a specific type of anemia بس each one has a distinct trap you have to pay attention to it iron we give iron لما بيكون نقص في ال iron كاملا ولا نقص في الهضم لل iron ولا الامتصاص malabsorption المشكلة إنه تتراسايكلين وال Will proton pump inhibitors both reduce oral iron absorption? You have to keep the iron and these you have to take them separate times. So When you give B12 for replacement, you have to make sure that it's done early so you don't have irreversible neurological symptoms. Well, folate replaces the folate, but if you replace it without checking B12 first, you will have an anoxic Them normal, and you will not know B12 is low. So you have to check both B12 and folate, and if you find them both, it deal B12 awlen. Then you can give the folate. This is coming to the end. So we have here. 58-year-old male on chronic warfarin for a mechanical aortic valve is admitted with GI bleed. The INR instead of 2 to 3 that is normal now is 6.8, 3 times more. He's also on 6-mercaptopurine for a history of ALL and was recently started on allopurinol for gout by an outside clinic without any dose adjustment. We know that now, don't we؟ حكينا عنه الورفرين is anti vitamin K فبنديله الvitamin K والfactor 4 PCC 2, 7, 8 Nine and ten for reversal. هذا اسمه major bleed, so we must take this first before we do anything else. حتى لو عنده صمام، we go back to it after we stop bleeding. Okay? On labs, his white count is dangerously low. What is the second problem? Six mercaptopurine was never reduced. When aloperinol was added, causing severe myelosuppression from unmetabolized six mercaptopurine. The correct step is At the clinic, allopurinol was started. We should have cut the six mercapturine dose twenty-five to thirty-three percent. I hope that you enjoyed this. The RBC shape fall into three mechanisms. Families. Membrane is lost. Membrane is sheared. Membrane is remodeled. Tenian. The hour rods, myeloid always. Smudge cells suggests tecterach CLL, but you need flow cytometry confirmation. لأنه the preparation of the smear can cause smudge cells without the patient having anything. ثلاثة patient platelet clumping platelets clumping وهو تجمع الصفائح مع الفحص بيكون طبيعي normal physical exam you have to repeat in citrate two before treating as a real thrombocytopenia لازم نعيد العد في أنبوب الستريت قبل ما نعتبر إنه الصفائح ناقصة حقيقياً. So warfarin needs INR monitoring and vitamin K PCC reversal. Anti-ten A factors. They have specific antidotes. El dabigatran has idarucizumab. Will apixaban? Will will rivaroxaban? And then lastly, always check for interactions on every home medication. Remember. 6-mercaptopurine and allopurinol is the classic dangerous example. I hope you enjoyed this session. I hope that you found what you're looking for. I look forward to the next things. And now I'm going to be sending you this week multiple questions in order for you To see what you can do, how you can integrate a lot of this stuff. Don't be afraid. Okay, so we have to repeat things in order for us to remember them and remember them while understanding the mechanism. Allahumma arham shahada al muslimin. Allah yekun faun kun. I am so pleased. لو سمحتم send me question. لو you need understand more, I will incorporate it in more sessions. Between now and then, until then,
